The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Sector Honolulu received report that subject vessel has one inoperable generator, damage to props, and is pumping sewage directly overboard. Vessel is prohibited from movement until an inspection is completed by marine inspectors to determine suitability for service. 46 CFR 176.840

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structure examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Vessel unintentionally grounded, damage to hull and propulsion suspected. Vessel is prohibited from carrying passengers and is authorized to make a one-time transit directly to Phoenician shipyard for emergency drydock inspection. 46 CFR 176.600(d)

The minimum capacity of a the fire pump shall be 50 gallons per minute of a pressure of not less than 60 pounds per square inch at the pump outlet. Unable to provide adequate proof of pressure due to wasted and blown fire main pipe. Replace wasted section and conduct fire main test to the satisfaction of the attending marine inspector.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules... White masthead light and anchor light do not illuminate when energized. Restore to operational status to the satisfaction of the attending marine inspector.

A vessel on an Oceans, Coastwise, Limited Coastwise, or Great Lakes route must carry - Six hand red flare distress signals... 04 red flare signals onboard found to be expired. Replace with approved red flares to the satisfaction of the attending marine inspector. Until resolved, vessel is limited to operation to Lakes, Bays, and Sounds.

A vessel must comply with the structural design requirements of one of the standards listed. 01 large pit measuring 5/16 in in depth located in the port transom within the lazarette. Submit a repair proposal to the satisfaction of Marine Inspector.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. 02 life ring buoys on board vessel have deteriorated Type II retroreflective material. Replace the retroreflective material on the main deck port life ring and upper deck light ring to return both rings to compliance to the satisfaction of the attending marine inspector.

Electrical equipment must be installed to protect from electrical hazards. Gangway winch wiring found with splices. Replace wiring.

A vessel must comply with the structural design requirements of one of the standards listed. Areas in the overhead of the lazarette found with several pits, exhaust flange in lazarette was found with rust and corrosion, cooling water pipe for exhaust in way of previously mentioned flange was found with weeping from welds and swim platform found with separation of wooden planks from main structural support. Address all areas by completion of next drydock.

Fire hose shall be 1 1/2 inches in diameter and 50ft in length and be lined commercial fire hose that conforms to UL 19. Excessive leaking was found from both fire hoses. Replace hoses with UL 19 conforming hoses.

The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Approved Coast Guard Marine Safety Center (MSC) QFA/DVTPs for the vessel’s ZF MICROCOMMANDER processor propulsion engine control system Throttle feedback potentiometer failure test failed, two manufacturer procedures were not submitted for review, and approved DVTPs require revision. 1) Provide ZF's determination of the witnessed result for the throttle feedback potentiometer failure; 2) Add the two missing manufacturer procedures to DVTPs; 3)Submit revised DVTPs to MSC for approval; 4)Conduct a retest of the throttle feedback potentiometer failure. Number 1,2,and 3 are to the satisfaction of the attending marine inspector. Number 4 is in the presence of the attending marine inspector. 46 CFR 176.700(d)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Engine room bilge has oil soaked diapers, excessive oil/water in bilge, and accumulation of oil on the machinery and their foundations. Remove oil soaked diapers from bilge, clean bilge of the excessive oil and water, and wipe running oil off the machinery and foundations to the satisfaction of the attending marine inspector by August 2, 2024. 46 CFR 176.830(b)

The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Approved Coast Guard Marine Safety Center (MSC) QFA/DVTPs for the vessel’s ZF MICROCOMMANDER processor propulsion engine control system test failed and two manufacturer procedures were not submitted for review. 1) Repair faulty alarms and failed stbd bridge wing throttle head; and 2) Conduct pretesting of approved DVTPs and missing DVTPs prior to requesting Coast Guard MI attendance to witness testing. 46 CFR 176.700(d)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The #1 generator raw water sea strainer valve was seized and could not be operated. Install new valve prior to carriage of passengers. 46 CFR 182.100

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. A red hand flare distress signal may be substituted for any of the hand orange smoke distress signals. Vessel has eight red hand flare distress signals and is required to have 4 hand orange smoke or red hand flare signals in order to meet the regulation. Vessel restricted to a Lakes/Bays/Sounds route or 30 min voyages. 46 CFR 180.68

(a) Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency; (b) The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Vessel personnel installed an electric stop solenoid for the center engine without submitting plans. Submit the following to the attending Marine Inspector for review and approval: 1) Elementary one line diagram of power system; 2) Voltage drop calculation; and 3) Solenoid manufacturer data. Once the alteration proposal is approved by the attending Marine Inspector, complete the installation and test in the presence of the attending Marine Inspector. To be completed by August 10, 2024. Until the deficiency is resolved, the center engine is only to be operated in an emergency or when determined necessary by the master to ensure safe navigation of the vessel. 46 CFR 176.700(a)(b)

Vessels shall be provided with toilets and wash basins in accordance with Table 177.30-5(a) except that vessels operating on short runs of approximately 30 minutes or less need not be fitted with toilets or wash basins. All toilets and wash basins provided shall be fitted with adequate plumbing. Vessel's Type III sewage holding tank is wasted and is not permitted to retain sewage. Submit a Type III sewage holding tank repair/replacement proposal for approval to the attending Marine Inspector. Once the repair/replacement proposal is approved by the attending Marine Inspector, complete the repair/replacement of the Type III sewage holding tank to the satisfaction of the attending Marine Inspector. 46 CFR 177.30-5 (a)(b) Old T

At each inspection of a vessel, the owner or managing operator shall be prepared to conduct tests including the following: Inspection and operation of through hull fittings and sea valves. Make all sea valves within 6 inches of the waterline and below available for inspection. 46 CFR 176.802(a)(2)

During the examination all accessible parts of the vessels underwater body and all through hull fittings including the propellers, shafts, bearings shall be made available for examination. Remove bottom paint and make hull available for inspection. 46 CFR 176.610 (a)

An internal structural examination consists of a complete examination of the vessel's voids and ballast, cargo, and fuel oil tanks. De-water CO2 room and make available for inspection. 46 CFR 176.610(b)

The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Provide to the Coast Guard Marine Safety Center (MSC) - 1) A QFA of the vessel’s ZF propulsion engine control system (throttle) system limited to the loss of power to the control system. (2) A propulsion control drawing with an installation/operational test protocol. This test protocol is an abridged document to show compliance with 46 CFR.184.620. Upon MSC approval,the test must be witnessed by the attending MI. 46 CFR 176.700(d)

Each circuit breaker must meet UL 489 or other standard specified by the Commandant, and be of the manually reset type designed for: (1) Inverse time delay; (2) Instantaneous short circuit protection; and (3) Switching duty if the breaker is used as a switch. New ZF MICROCOMMANDER processor circuit breakers are 10 amp UL 1077. Replace with 10 amp UL 489 circuit breakers to the satisfaction of the attending MI. 46 CFR 183.380(m)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Provide from the manufacturer or manufacturer's authorized representative a attestation letter stating: (1) Installation configuration of the six additional control cables instead of the bridging bar is permitted. (2) That 20 AWG is permitted to be used as control head cable in lieu of the manufacturer's required 18 AWG control head cable. If not permitted install system in accordance with the manufacturer's instructions to the satisfaction of the attending MI. 46 CFR 176.700(a)

Penetrations and openings in the watertight bulkheads must have means to make them watertight. Bulkhead 19 penetrations on the stbd for new control cables are not watertight. Make water tight to the satisfaction of the attending MI. 46 CFR 171.114(a)(2) Old "T"

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The new ZF MICROCOMMANDER processor wire from the battery to the circuit breaker for the stbd engine is excessively chaffed. Replace wire in-kind to the satisfaction of the attending MI. 46 CFR 176.800(a)

During the deficiency check on May 19, 2023, the attending MI sighted the stbd main engine with excessively oil soaked exhaust insulation heat wrap. Remove oil soaked insulation, repair leak, operationally test for exhaust/oil leaks, and renew insulation to the satisfaction of the attending MI. 46 CFR 176.830(a)

A device must be provided to automatically shut down engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. Activation of the fixed CO2 system did not automatically shut down stbd engine. Replace solenoid. 46 CFR 181.410(b)(10)

A visual and audible alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. High water level alarm in rudder room was not wired. Rewire and demonstrate proper operation. 46 CFR 182.530(a)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Man overboard drill not conducted. Conducted MOB in the presence of the attending MI. 46 CFR 176.808(g)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel’s hydrostatic release mechanism is expired. Replace hydrostatic release. 46 CFR 180.64

Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test shall be logged or otherwise documented, as applicable. Operator was unable to provide log of EPIRB testing. Provide log of testing. 46 CFR 185.25-20

Each ship transporting more that six passengers for hire equipped with a radiotelephone station must carry a radio operator who: holds a marine radio operator permit or higher class license. No crew on-board had a valid radio operator permit. Provide proof of permit. 46 CFR 184.502

The drill entry shall include the following information: Date of the drill and training; and general description of the drill scenario and training topics. Vessel replaced anchor light to use as stern light. Provide operable anchor light. 46 CFR 183.420

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Motor/pump coupling on port side engine room is missing guard. 46 CFR 177.35-15

Lifesaving equipment must be in serviceable condition. All three life floats were determined by the Marine Inspector to be not fit for service and removed during COI inspection 26OCT2021. Life floats have still not been replaced. Resolve by date or COI will be amended with "NOT MORE THAN THREE MILES FROM SHORE." 46 CFR 180.200(c)

Exhaust piping shall be led to the point of escape with a minimum number of bends or elbows. It shall be gas tight throughout its length and shall be properly supported by non-combustible hangers or blocks. In engine room, exhaust piping was missing support bracket. Install new bracket. 46 CFR 182.15-20(a)

This part prescribes regulations governing the design and construction of marine sanitation devices. Type III tank in rudder room inlet pipe had excessive pitting. Renew at next dry dock. 33 CFR 159.1

Where flexibility is necessary a section of flexible metallic hose may be used. Non-metallic hose may be used provided it is especially adapted to resist the action of oil, acid and heat, and has a wall thickness sufficient to prevent collapsing or panting. Rubber wire reinforced exhaust hose in rudder room was found worn. Replace with approved flexible non-metallic hose as described above. 46 CFR 182.15-20(c)

Main distribution panels shall be of the dead front type where voltage to ground is in excess of 150 volts A.C. or 250 volts D.C. Switchboard in engine room had opening, allowing access to panel internals. Cover opening in switchboard. 46 CFR 183-10-15(f)

Electrical equipment used in the following locations must be drip proof: A location normally exposed to splashing or water washdown. Four inches of standing water sighted underneath the bridge console. Remove water and resolve source of water ingress. 46 CFR 183.210(a)(2)

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. This section applies to each vessel that fits into any one of the following categories:(1) A vessel of more than 65 feet length; all vessels must have permanent draft marks at each end of the vessel. Vessel does not have aft draft marks as required by the stability letter. 46 CFR 185.30-1(a)(b)(1)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessels alcohol test strips are expired. Replace alcohol test strips. 46 CFR 4.06-15

The employer shall provide an Employee Assistance Program for all crewmembers that includes both education and training on the effects and consequences of drug and alcohol use. Supervisory personnel must receive at least 60 minutes of training. No evidence of supervisory nor crew training on the effects and consequences of drug and alcohol use. Provide evidence of such training to supervisors and crew. 46 CFR 16.401(b)

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, equipment. Vessel’s centerline engine not operable. Provide repair plan timeline to the satisfaction of the attending Marine Inspector. 46 CFR 177.10-1

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel missing the second radio required by the Bridge-To-Bridge Radiotelephony Certificate. 46 CFR 184.502

Whenever damage or deterioration to hull plating or structural members that may affect seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. During the damage survey, following discovery of unapproved repair progress by MI, it was determined that the damage did not effect the seaworthiness of the vessel. This vessel may continue to operate, however they must effect permanent repairs at their next drydocking to receive credit drydock.

Immediately after the addressing of resultant safety concerns, the owner, agent, master, or person in charge of a vessel involved in a marine casualty shall notify the nearest Sector Office, Marine Inspection Office, or Coast Guard Group Office whenever a vessel is involved in a marine casualty consisting of: (3) Loss of main propulsion or primary steering, or any associated component or control system, that reduces the maneuverability of the vessel. During an inspection of a vessel in Kewalo Basin the attending MI's observed staging on the bow of the inspected vessel PRINCE KUHIO associated with an in-water repairs. A Coast Guard (CG) marine inspector (MI) visited the vessel and was told by the C/E the vessel control linkage at the reduction gear had detached causing the bridge propulsion controls to become inoperable resulting in the allision of the PRINCE KUHIO with the Kewalo pier structure. Passenger carriage is not permitted until an MI attends the vessel to conduct a damage survey, vessel owner/operator provides a repair proposal for CG review/approval, and repairs are completed to the satisfaction of the attending MI.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Effect repairs to restore propulsion plant to operate as designed prove proper operation of plant to attending Marine inspector. Your vessel is prohibited from transiting any port within the sector Honolulu COTP zone without the assistance from towing vessel(s) of adequate horsepower and maneuverability. Such towing vessel(s) shall be under the direction and control of an individual holding a Merchant Marine Credential (MMC) authorizing them to engage in assistance towing under the provisions of 46 CFR Part 11.482. 46 CFR 182.200 (a)

At each inspection for certification the fire hose shall be subjected to the available fire hose pressure. The port fire hose leaked when subjected to the pump output pressure. Provide a new fire hose. 46 CFR 176.25-25(a)(1)

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Missing double hose clamps on raw water intake. 46 CFR 182.410(d)(1)

All machinery and equipment must be installed and maintained in an such a manner as to afford adequate protection from personnel injury. The ensure prevention of personnel injury the vessel Captain shall provide a written policy stating that the vessel is normally unmanned and to verify the space is clear of personnel prior to the activation of the fixed extinguishing system. 46 CFR 126.25-1 46 CFR 182.100

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity. Vessel's secondary generator was not in service. Bring adequate secondary source of power into working condition. 46 CFR 183.310(a)

The owner or managing operator or a qualified servicing facility shall conduct inspections and tests as applicable for fixed gas fire extinguishing systems and must provide evidence of the required servicing to the marine inspector. Vessel's fixed gas fire extinguishing system servicing was outdated. Bring system into compliance. 46 CFR 176.810(b)(2)

An adult life jacket must be provided for each person carried on board a vessel. Acquire serviceable life jackets are required for all passengers. Multiple life jackets were found un-serviceable, total POB is limited to 125. 46 CFR 180.71

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals and six hand orange smoke distress signals. Vessel missing 06 orange smoke flares. Vessel is restricted within 3NM from a harbor of safe refuge until the correct number of distress signals are acquired. 46 CFR 180.68(a)

Hazardous electrical practices, including standing water underneath the pilothouse electronics, and hazardous configurations discovered in the pilothouse and engine room. Bring vessel's electrical configuration into compliance. 46 CFR 183.200(a)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position or perform the duties and functions of a position, required by the vessel's COI. Marine employers may form or otherwise use sponsoring organizations, or may use contractors to conduct the random chemical testing programs. Vessel missing DAPI policy, crew and supervisor training, and letter of regulatory compliance from the consortium. Provide all requirements. 46 CFR 16.230(a)

Machinery, equipment, materials and arrangements not covered by standards in this chapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The submersible bilge pump servicing the forward compartment has plastic piping that penetrates the aft bulkhead. A positive stop valve with remote reach rod that is accessible by the crew members must be provided on the galley side of the bulkhead. 46 CFR 176.800(a)

On a vessel of more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The fire pump onboard did not exceed 20 psi of discharge pressure. Provide proof of 60 psi discharge pressure of the fire pump in the presence of the Marine Inspector. 46CFR181.300

The Bridge-to-Bridge Act and the regulations of this part apply to the following vessels in the navigable waters of the United States: Every power driven vessel of 20 meters (65.6 feet) or over in length while navigating. The vessel is prohibited in carrying passengers for hire until a valid Bridge-to-Bridge certificate is onboard the vessel. 47CFR80.1001(a)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with 47CFR80.59 once every five years. The vessel does not have a valid FCC radio safety certificate onboard. The vessel is limited in operation to no more than 1000 feet from shore until a valid FCC radio safety certificate is onboard. 47CFR80.903

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The EPIRB hydrostatic release was expired. The vessel is restricted to no more than 3 NM from shore until the hydrostatic release is replaced. 46 CFR 185.740(b)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of 80.1101(c)(2). VHF radio missing MMSI and LAT/LONG for DSC operation on bridge operating station VHF radio. Provide proof of VHF radio containing MMSI and LAT/LONG for DSC operation to the Marine Inspector. 47 CFR 80.905

All vessels of more than 19.8 meters (65 feet) in length must have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights.” Restore the starboard green navigation light cover to the satisfaction of the Marine Inspector. 46 CFR 183.420

The stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. The vessel did not have an up-to-date Stability Letter onboard. Post the stability letter to the satisfaction of the Marine Inspector. 46 CFR 176.306

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts & publications IAW 46CFR184. Vessel was missing all required charts & publications. Provide proof of charts and publications onboard. 46 CFR 184.420(a)

Connections must be made to battery terminals with permanent type connectors. 04 wing nuts were discovered on the port battery installations. Replace wing nuts with proper mechanical fittings to the satisfaction of the Marine Inspector. 46CFR183.350(d)

Gauges to indicate cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. Main engine cooling water discharge temperature and lubricating oil pressure gauges were inoperable. Repair gauges and provide proof to the satisfaction of the Marine Inspector. 46CFR182.20-5(b) "Old T"

Exhaust piping shall be gas tight throughout its length and properly supported by non-combustible hangers or blocks. The port diesel generator exhaust cooling has a failed hose clamp. Repair the failed hose clamp to the satisfaction of the Marine Inspector. 46CFR182.15-20(a) "Old T"

Complete but simple instructions must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Schematic diagram is missing; provide and post schematic diagram. 46CFR181.20-15(d) "Old T"

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR.” Emergency exit labeling for engine room is peeling off. Relabel engine room emergency exit. 46CFR185.606

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Several fuel oil filters were missing strainer bowls. Install strainer bowls IAW manufacturer’s requirements. 46CFR182.455(b)(6)

To avoid water and exhaust gases from ingress into the engine room, install caps in both generator exhaust drain valves. 46 CFR 176.25-1(a) “Old T”

Black water tank flex hose is missing double hose clamps at tank connection and aluminum pipe observed wasted. Remove hose and inspect inlet side of pipe for wastage. Notify [email withheld] of findings and submit repair proposal for Coast Guard approval if required. 46 CFR 176.25-45(a) “Old T”

At each initial and subsequent inspection for certification, re-inspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Center main port wet exhaust hose is weeping and the hoses are wasted. Starboard main engine flex hose leaking. Replace flex hoses and install double hose clamps where required. 46 CFR 176.25-50(a) “Old T”

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arrestors. The flame screens shall consist of a single screen of corrosion resistant wire of at least 30x30 mesh. There are holes in both port and starboard fuel tank flame screens. Replace flame screens. 46CFR182.20-35(d) “Old T”

It is intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Repair leak above service compressor for heads. 46CFR182.01-5(a) “Old T”

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. The vessel did not have a current crew list of individuals employed onboard the vessel, and enrolled in the drug testing program. Provide a valid crew list to the satisfaction of the Marine Inspector. 46CFR16.230

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with 46 CFR 4.06. The vessel had no method of conducting serious marine incident alcohol testing within 2 hours. Provide proof of alcohol test strips onboard to the satisfaction of the Marine Inspector. 46CFR16.240

Provisions must be made for closing all supply duct cowls or scoops for a space protected by a fixed gas extinguishing system. The vessel's 02 generator intakes on the main deck did not have a means of closure. Provide a means of closure to the satisfaction of the Marine Inspector. 46CFR182.465(h)

All electrical equipment and circuits must be clearly marked and identified. The port switchboard at the operating station is not labeled, and has unidentified circuit breakers. Clearly mark and identify the circuit breakers IAW 46CFR183.220. 46 CFR 183.220(d)

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. The kitchen area has 120 VAC outlets which are missing faceplates. Replace the faceplates to the satisfaction of the Marine Inspector. 46CFR183.200

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Numerous dead end wires were discovered under the console at the operating station. Remove or properly terminate the wires to the satisfaction of the Marine Inspector. 46CFR183.340(g)

Each pressure type wire connector and lug must meet UL486A. Numerous unauthorized automotive type connections were discovered under the console at the operating station. Replace the connections with proper pressure type wire connectors meeting UL standards. 46CFR183.340(i)

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. A power strip was discovered under the console at the operating station servicing unknown electrical equipment. Identify the electrical equipment being serviced, remove the power strip, and ensure proper circuit protection to the satisfaction of the Marine Inspector. 46CFR183.380

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. The vessel has numerous lighting fixtures which have open electrical plugs. Seal the electrical plugs to the satisfaction of the Marine Inspector. 46CFR183.200

Electrical equipment used in the following locations must be drip proof. Steering feedback unit in lazarette, and bilge alarm float switch. 46 CFR 183.210(a)(3)

Electrical equipment must be installed to protect from electrical hazards. Dead end wires found at the forward exterior of male head. Properly remove or cover to prevent potential shock or fire hazards. 46 CFR 183.200

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Seal opening in aft heat detector for CO2 system. 46CFR183.200(c)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards including fire. An excessive amount of electrical cable was found coiled under the console at the operating station. Remove the excessive coil of cable to reduce an electrical fire hazard to the satisfaction of the Marine Inspector. 46CFR183.200(b)

All electrical equipment shall be drip proof protected. Totally enclosed distribution panel. #2 switchboard in galley has an opening where an old breaker was removed. Blank off opening. 46 CFR 183.10-10(a) “Old T”

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather deck. Cover or close opening in engine room overhead aft on starboard side. Cap old wire stuffing tube on weather deck of forward peak. 46 CFR 176.176.25-5(a)(1) “Old T”

Accessories, such as switches, fuses, and sockets, shall be standard National Electric Code types for the loads to be carried and shall be of types UL-listed. Energizing service air compressor requires opening enclosure to manually close contacts. Replace or repair service air compressor start switch. 46CFR183.05-35(a) “Old T”

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Either remove the breaker on starboard side forward of service air compressor, or place it in an approved enclosure and properly wire. 46 CFR 183.01-5(a) “Old T”

Over current protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. The PA system was connected to an unauthorized power strip. 46CFR183.380

If the vessel carries more than one life float in a group with each group secured by a single painter: (3) Each life float must be individually attached to the painter by a line long enough that each life float can float without contacting any other life float in the group. The 03 life floats were rigged as a group, but were not secured with a single painter. 46 CFR 180.137(e)(3)

Each child size life jacket must be stowed in a location that is separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. 01 Adult PFD was found in the stowage area of the child size life jackets. 46 CFR 180.78(a)(5)

Each EPIRB must be tested monthly, using the integrated test circuit. The EPIRB test shall be logged. The vessel had no log of monthly EPIRB testing. 46 CFR 185.728(a)

The inspection and maintenance of the equipment listed in 46CFR185.702 shall be logged or documented for review by the Coast Guard upon request. The vessel had no log of inspection and/or maintenance of primary survival craft. 46 CFR 185.702(d)

The master shall ensure that the ship is not operated unless a garbage management plan is on the ship. The vessel did not have a Garbage Management Plan. 33 CFR 151.57(a)

Fire hoses shall be 1 1/2" in diameter and 50 feet in length. 1 1/2" Fire hose must: Be lined commercial fire hose that conforms to Underwriter's Laboratories Inc., Standard 19 or Federal Specification ZZ-H-451E. Both of vessels two fire hoses leaking significantly and no longer in serviceable condition. Replace both hoses with approved UL 50 ft length and test in presence of attending Marine Inspector prior to carrying passengers. 46 CFR 181.15-10(c)&(d)

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. During operational test of fire pump, pressure gage at outlet reading 20 psi. Make 60 psi in presence of attending Marine Inspector. 46 CFR 181.10-1 (e)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Vessel did not have the correct number of passengers allowed posted. Make changes to indicate maximum of 60 passengers on the upper deck IAW stability letter dated March 22, 2017. 46 CFR 185.602 (g)

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. Vessel's remote fuel shutoff station not labeled. 46 CFR 185.608

The minimum number of portable fire extinguishers required on each vessel shall be as determined by the Officer in Charge, Marine Inspection n accordance with Table 181.30-1(a). No fire extinguisher at propane grill on stern. Provide 40 B:C extinguisher in the vicinity. 46 CFR 181.30-1 (a)

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Vessels emergency broadcast placard had outdated call sign. 46 CFR 184.506

Except as otherwise provided in this section, rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crew. Railing on stbd side fractured in 2 locations. 46 CFR 177.35-1 (a)

Remove sections of ceiling from main deck overhead for attending marine inspector to see condition of support structure of upper passenger deck. 46 CFR 176.610 (b)
