A gauge indicating the pressure in the system shall be provided at each section stop valve and at a central station. The gauge indicating needle for water-mist stop valves for sections deck 10; fire zone 3 and deck 5; fire zone 4 were found broken such that the gauges did not provide an accurate pressure reading. 74SOLAS01 II-2/12.2.3 50ac

The marking must enable passengers to identify all the routes of escape and readily identify escape ecits. All escape route signs and fire equipment location markings shall be photoluminescent material or marked by lighting. A section of low location lighting approximately 10 feet long was extinguished in the vicinity of door 3-2-007. 74SOLAS97 II-2/28.1.10 50ac

Stairways, doorways and corridors shall not be lass than 900mm in clear width. Multiple chairs were stowed in the passageway in the vicinity of door 4-2-005 such that the width of the passageway was less than 900 mm. 74SOLAS97 II-2/28.1.10 50ac

Fire doors in main vertical zone bulkheads and stairway enclosures, designed to re-open sufficiently allowing a clear passage of atleast 0.75 m but no more then 1 m. Fire screen door 3-5-015 opened more than 1 m after contacting an object. After contact the door remained fully open and did not attempt to close again. 74SOLAS97 II-2/30.4.2 50ac

Fire doors in main vertical zone bulkheads and stairway enclosures shall be self-closing and shall have an approximately uniform rate of closure of no more than 40s and no less than 10s. PSCOs observed the following doors failing to close: 02-1-016, 02-7-014, 08-7-006, 08-6-003, and 3-4-006. 74SOLAS97 II-2/30.4.1 50ac

The construction of all doors and door in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel or other equivalent materials. PSCO observed galley FSD 12-7-006 and engine room emergency exit FSD 30-06-009 did not fully latch. 74SOLAS97 II-2/30.2 / 10c

Fire protection systems and fire fighting systems and appliances shall be maintained ready for use. PSCO observed FST-04-6-01 P and FST 05-6-07 P latches on doors fail when attempted to be opened. Two different members could not be open FST 04-6-01. The handle on FST 05-6-07 fell off when attempting to open doors. 74SOLAS20 II-2/14.1.1 / 10c

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Vessel reported a complete loss of propulsion of the starboard azipod when shifting from hand steering to aziman mode while mooring up in Sitka Alaska. 74 SOLAS 04 II-1/26.1

07105- The condition of all doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors or situated. 07120- In addition to the emergency lighting, the means of escape, including stairways and exits, shall be marked by lighting of photo luminescent strip indicators placed not more than 0.3m (300mm) above the deck at all points of the escape route including angles and intersections. The marking must enable passengers to identify all the routes of escape and readily identify escape exits. PSCO observed low location lighting not illuminating at locations: deck 11 - outside hair salon, deck 5 – IVO art and photo gallery, deck 5 - IVO kids department near adventure ocean discovery, deck 3 – IVO FSD 03-6-005 port side. -74 SOLAS (97)II-2/28.1.10 -17ac

PSCO observed hydraulic oil leak in PORT & STBD azipods directional control valves. -74 SOLAS (97)II-2/15.4 -17ac

07105- Fire doors in main vertical zone bulkheads and stairways enclosed, designed to re-open upon contacting an object in its path shall re-open sufficiently to allow a clear passage of at least 0.75 m but no more than 1 m. PSCO observed crash bar on FSD 04-5-004 did not reopen upon contact. -74 SOLAS (97) II-2/30.4.2 17ac

07105 - Fire doors/openings in fire-resisting divisions - Fire doors in main vertical zone bulkheads and stairway enclosures shall be self-closing and be capable with closing with an angle of inclination of up to 3.5 degree opposing closure. PSCO observed FSD 02-5-021 unable to close due to bent/damaged door. 74 SOLAS 97 II-2/30.4.1 / 17c

11101 - Lifeboats - All lifeboats shall be provided with a rudder and tiller. When a wheel or other remote steering mechanism is also provided, the tiller shall be capable of controlling the rudder in case of failure of the steering mechanism. PSCO observed the emergency steering assembly on Lifeboat Tender #9 was inoperable. 74 SOLAS 97 III/41.7.2 / 17c

07101 - Fire prevention structural integrity - Spaces are classified according to their fire risk as shown in categories (1) to (14) below. Fan room deck 8, MVZ 3 is a category (10) space where storage of combustibles is prohibited. PSCO observed storage of air filters and insulation. 74 SOLAS 97 II-2/26.2.2 / 10ac

Except for hatches between cargo, special category, store, and baggage spaces, and between such spaces and the weather decks, all openings shall be provided with permanently attached means of closing which shall be atleast as effective for resisting fires as the divisions in which they are fitted. PSCO observed FSD5: 03-7-01 1, 03-7-025,03-7-013, 02-3-011, 30-5-005, 1-6-035, 1-6-22,05-7-009, 04-1-015, and 30-5-004 failed to close. 74 SOLAS 97 II-2/301 - 17C

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. PSCO observed crew members assigned to lifeboats 2,4, and 6 were not familiar with fuel shut off procedures. 14 SOLAS 20 III/19.2.1 - 17C

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation 11-1/42 or 11-1/43. PSCO observed 05 extinguished embarkation deck lighting on the port side of the vessel. 74 SOLAS 97 III/11.4 - 10C

07106-Any required automatic sprinkler, fire detection and fire alarm system shall be capable of immediate operation at all times. PSCO observed inoperable smoke detector DK00.03.25.007 in guest main laundry space. -74SOLAS(97)II-1/12.1.1 / 17c

07120-The means of escape, including stairways and exits, shall be marked by lighting and photo luminescent strip indicators. PSCO observed approximately 10 feet of low location lighting leading to the main entrance to the spa was inoperable. 74SOLAS(97)II-2/28.1.10 / 40c

In addition to the emergency lighting required by regulations 11-1/42 and 111/11.5, the means of escape shall be marked by lighting or photoiuminescent strip Indicators placed not more than 0.3 m above the deck at all point of the escape route including angles and Intersections. Deck 2, aft, in vicinity of ECR, emergency lighting was blocked by provisions/spare parts stored In the corridor.

Every ship of 400 GT and above, and every ship that is certified to carry 15 persons or more, shall carry a garbage management plan which the crew shall follow. This plan shall provide written procedures for collecting, storing, processing and disposing of garbage ... Garbage was being stowed in zone whfch was not permitted by the garbage management plan.

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other than power-operated watertight doors and those which are normally locked shall satisfy the following requirements: the doors shall be self-closing...FSD 11-4-001, FSD 02-7-006 would not fully latch once released.

Any required fixed fire detection and fire alarm system with manually operated call points shall be capable of immediate operation at all times. Call points Dk.04-07.68 004 and Dk 04-07.68 007 are inoperable.

Indicating units shall, as a minimum, denote the section in which a detector has operated; smoke detectors 04.02.07.36 and 04.02.07.35 are indicating each other's posiiton IAW 74 SOLAS (2001) II-2, 13.1.6.

Def No. 1 (Code: 0695) MES system party shall be trained by participation in a full deployment of a similar system into the water. During the abandonship drill, the PSCO questioned the system party whom exclaimed they have received training but have not participated in a full deployment in the last (3) years.
