Vessel conducted a temporary repair to hull below the weather deck to facilitate watertight integrity of hull so passengers could be carried following inspection on 03APR2024 and cleared item #1. Repair was considered temporary and a permanent one shall be completed. Repair by either re-fastening port light to hull penetration or closing the hull penetration with the same wood material as hull shall include: verification of condition of port lights if chosen method, detailed process and list of materials that will be utilized to re-attached port light to hull or sealing/decommissioning hull penetration with wood. Once repair proposal is approved repairs can commence and will be verified by attending Marine Inspector. Guidance for wood repairs can be found in NVIC 7-95 or other recognized good marine practice publications.

Each port light must be constructed so that it can be secured watertight. In the engine room port bulkhead there was port light without the proper closing devices and in the same bulkhead had another port light missing altogether compromising the hull’s watertight integrity.

Provision shall be made by means of plugs, covers, etc. to prevent the admission of air into the space protected. Engine room dampers do not fit as designed. 705: 7 days

Separation of machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. The bulkhead separating the E/R from the accommodation space has a hatch cover with a missing gasket affecting the vapor tight integrity of the space. Additionally, a trash compactor installed in the same bulkhead had light penetrating around the outer edge of the trash compactor, thus creating an open space for the passage of vapor.

Each hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or other devices to prevent its loss. Hatch cover to aft accommodation space contains no hardware to secure hatch or attach to deck to prevent overboard loss.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Outlet receptacles in engine room missing outlet covers.

Flexible or portable cords such as SO, STO and similar types cannot be used for permanently installed wiring. Remove all portable extension cords currently in use throughout the vessel.

Each ring life buoy must be marked in clearly legible block capital letters with the vessel’s name. The orange ring life buoy was not marked with the vessel’s name.

All wiring and cables shall be suitable for marine service and for the particular installation used. Electrical cable for A/C power located on overhead near starboard fuel tank not marine grade.

The material used and the minimum thickness allowed in the construction of independent fuel tanks shall be as indicated in Table 182.20-25(a)(1). MI unable to inspect Starboard fuel tank due to widespread scaling rust on top of tank. Remove scaling rust and make starboard fuel tank ready for re-inspection.

Nozzles, flanges, or other fittings for pipe connections shall be welded or brazed to the tank. The tank opening in way of pipe connections shall be properly reinforced where necessary. Where liquid level indicating gauges are installed, they shall be of heat resistant materials, adequately protected from mechanical damage and provided at the tank connections with devices which automatically close in the event of rupture of the gauge or gauge lines. Fuel level indication not of suitable material and valves to allow fuel to flow into sight glass require manual means of closing.

A vessel less than 100 gross tons operating on exposed or partially protected water permitted to have an opening port light below the weather deck shall have an inside, hinged dead cover. The port lights did not have inside, hinged dead covers. Submit plan for installing dead covers or any documentation that the vessel does not have to comply with the requirements.

On passenger decks of a ferry or of a vessel on an excursion trip the following must be installed: Bulwarks; Chain link fencing or wire mesh that has openings of not more than 4 inches in diameter; or bars, slats, rail courses, or an equivalent spaced at intervals of not more than 100 millimeters (4 inches). Provide plan prior to installation to Marine Inspector for the installation of rail courses that meet the standard listed above.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. MI Observed excessive items being stored in the engine room creating unsafe fire load condition.

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a mannner as to avoid chafing and other damage.Electrical wiring within the engine space was found to be supported with plastic zip ties.

46CFR181.410(a)(3): Fixed gas fire extinguishing system design and installation must be in accordance with the Marine Design, Installation, Operation, and Maintenance Manual approved for the system by the Commandant. 46CFR181.410(d)(3): Piping, valves, and fittings must be securely supported, and where necessary, protected against damage. Piping for the fixed CO2 fire extinguishing system within the engine space was found to be supported with plastic zip ties.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. An electrical outlet located in vicinity of the interior steering console was found to have the hot and neurtral wires reversed.

Each Inflatable liferaft required on a vessel by this part must have a capacity of 6 persons or more. Vessel is required to have a 50 man life raft. Prior to inspection vessels life raft failed inspection at avalon rafts. Owner purchased new raft and is waiting for it to arrive by mail. Owner provided invoice for new raft.

All cables and wires must be protected by metal coverings or other suitable means. Junction box for a missing light has dead end wires hanging from it. Provide means to remove or cover dead end wires.

A vessel must carry a first aid kit approved under 160.041 or equivalent. Vessels first aid kit had very few items and was not to 160.041 standards. Provide a first aid kit IAW 160.041 or equivalent.

six life jackets found to have wasted retroreflective material. Replace retroreflective material.

Vessel missing EPIRB Log. Provide an EPIRB Log with monthly EPIRB tests documented.

Bildge in compartment aft of engine space contained excessive oil. Remove oil from bilge.
