The owner, charterer, master, or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 122.510, when applicable, the duties listed in the station bill required by § 122.514. Did not conduct drills during annual exam. Perform crew drills in the presence of attending MI prior to the carriage of passengers. 46 CFR 122.420(a)

Doors in A-Class fire control boundaries must meet the following additional requirements: (1) A door in a bulkhead required to be A-60, A-30, or A-15 Class must be of hollow steel or equivalent material construction, solidly filled with approved structural insulation, and capable of meeting the requirements of an A-15 Class bulkhead" MI observed fire door between machinery and engine room not meeting fire control boundary with 1/2 inch gap under fire door. All inlets and discharges led through the vessel's side shell shall be fitted with efficient and accessible means...for preventing the accidental admission of water into the vessel either through such pipes or in the event of fracture of such pipes. Inspectors found drain pipe near galley preparation station port side wasted and missing section to the through hull fitting. Through hull fitting had no valve or means to stop water ingress from sea. Pipe drain led directly into ships void space/galley preparation area. Submit repair proposal prior to repair to local OCMI. 46 CFR 56.50-95 (a)(1)

Flaking and scaling of hull steel next to the Keel was observed in Void 4 (Freshwater Void), and under the Galley in the bilge two frames forward of access hatches. descale and take pictures of cleaned steel.

Deck railing on the 02 deck both port and starboard side were found with rusted stanchions. Repair.

Aft port lazarette to port fuel tank approx 10ft by 10ft steel hull plate was identified by vsl rep to be below threshold and will be cropped and renewed. Call outs for crop, fit up, back gouge, final and NDT required.

"A vessel that is exposed to salt water more than three months in any 12 month period... must undergo a dry dock examination and internal structural at least once every two years..." Vessel lapsed its DDX/ISE inspection. Complete Credit DDX and ISE prior to carrying passengers. 46 CFR 115.600 (c)(1)

"Exercises must be conducted at least once each calendar year, with no more than 18 months between exercises." Vessel has not conducted a security drill or exercise since 2019. Prior to carrying passengers conduct security drills and exercise. 33 CFR 104.230(c)(1)

The intent of this part is to insure that each vessel is provided with safe,readily available and sufficient firefighting equipment of the type necessary to extinguish any fires which are likely to be encountered in normal operation. Adequate emergency lighting automatically actuated upon failure of the main lighting system shall be fitted along the line of escape in vessels having lounge areas below the main deck. Emergency lighting in galley was not operational.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. MI found unauthorized power strip connected to the surge protector, overloading the power strip.

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following:(1) The location of emergency exits, survival craft embarkation areas, and ring life buoys;(2) The stowage location(s) of life jackets;(3) Either:(i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, or(ii) That passengers may contact a crew member for a demonstration as appropriate,(4) The location of the instruction placards for life jackets and other lifesaving devices;(5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master; and(6) If the vessel is operating with reduced manning or equipment requirements in § 176.114 of this chapter. Vessel's passenger safety orientation script did not include all of the required information. 46 CFR 185.506(a), 16c

All portable fire extinguishers installed on vessels shall be of an approved type. Portable fire extinguisher on the bridge lacked appropriate bracket to qualify it as a type approved marine fire extinguisher. 46 CFR 181.30-5, 16c

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. No portable extinguishers are being subjected to monthly inspection. 46 CFR 176.810(b), 16c

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Terminal end of conductor in the pump area of the engine room bare and not in enclosure. Provide enclosure or remove conductor, located in pump area below engine room.

Door frames shall be of rigid construction, and shall provide at least a 1/2 inch door stop at the sides and top. Gap spacing in excess of allowance and does not provide a 1/2 inch stop. Replace in accordance with 46CFR 72-25

The discharge valve on the fire main is leaking and needs to be repaired. Cite: 46CFR181.310

Flexible fuel hose on stbd main engine is chafed and needs to be replaced. Chafing material needs to be removed from fuel hose on both main engines. Cite: 46CFR182.720(e)

Galley hood suppression system needs to shut off power to appliance. Pressure system is not wired. Cite: 46CFR

A vessel's hull must not carry current as a conductor, ensure ground found on port heater is rectified. Cite: 16CFR183.370(a)

C/R to good metal, bottom plate and frame in stbd side of galley bilge crawl space approximately at the midsection of the space as discussed and marked.

Permanently repair the generator drainage pipe in engine room space.

Replace float light battery.

Submit repair proposal for proper installation of shore tide connection.
