Fire protection systems including structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions shall be kept in good order so as to ensure their required performance if a fire occurs. Each vessel must be manned IAW its COI. An AB was removed from the vessel due to an injury sustained during a lifeboat drill. Vessel must have the appropriate number of ABs per its COI prior to embarking on an international voyage.

In cargo ships of 5,000 gross tonnage and upwards there shall be a self-contained emergency source of power, located above the uppermost continuous deck and outside the machinery casings, to ensure its functioning in the event of fire or other casualty causing failure to the main electrical installation. The main steering gear and rudder stock shall be capable of moving the rudder from 35° on either side to 35° on the other with the vessel at its deepest loadline draft and running at maximum ahead service speed, and from 35° on either side to 30° on the other in not more than 28 seconds under the same conditions. During test of vessel’s steering, it was discovered that the rudder angle indicator was not capable to move 35 degrees stbd and stopped at 33 degrees starboard. Repair and prove operation of ship's main steering in wheelhouse and steering room is capable to place the rudder hard over left (35 deg.) in both modes using No. 1 and 2 steering pumps.

Fixed gas fire-extinguishing systems equivalent to those specified in paragraphs 2.2 to 2.4 shall be approved by the Administration based on the guidelines developed by the Organization. The vessel’s low-pressure CO2’s duplicate refrigerating plant is unable to automatically start up upon failure or stoppage of the unit in operation. Provide proof that the installation and operations of the vessel’s CO2 low pressure fixed firefighting system is in accordance with the applicable Class rules which is specified under the vessel’s CSSC certificate.

Cargo ships shall carry one or more totally enclosed lifeboats complying with the requirements of section 4.6 of the Code of such aggregate capacity on each side of the ship as will accommodate the total number of persons on board. (LSA 4.4.2.2) - the number of persons having an average mass of 75 kg, or 82.5 kg (for a lifeboat intended for a cargo ship), all wearing lifejackets, that can be seated in a normal position without interfering with the means of propulsion or the operation of any of the lifeboat's equipment. Port and starboard lifeboats lower seating forward of helm found with cable interferences in way of seating arrangements. Provide USCG approved drawings for seating arrangements.

Replace or repair the following to the satisfaction of the Class surveyor: a) Repair supply vent exhaust in upper flat in bow thruster room with visible crack to as original. b) Repair starboard side electrical pipe stuffing tube with wastage and holed located above the aft mooring station. c) Repair and preserve fuel vent pipe with excessive corrosion around manifold connection forward of ladder well on bow to as original. d) Remove corrosion on metal hangar support on MOGAS fuel line located at aft starboard mooring station for further inspection and repair. e) Make weather tight manhole access covers with missing bolts on the forward and aft bulkheads in starboard GCC compartment to prevent damage to high voltage electrical panels inside from wind/rain. f) Replace missing bolts on manhole cover located on upper most part of aft bulk located in Bosun’s store. Crew believes the manhole cover does not access a void or space due to its location. Verify in vessel’s records or drawings.

All electrical apparatus shall be constructed and so installed that danger or injury in ordinary handling shall not exist. Repair overhead junction boxes with exposed wiring in fireman storage locker and hydraulic room in forward bosun locker.

Every radio installation shall be clearly marked with the call sign, the ship station identity, and other codes as applicable for the use of the radio installation. [name withheld] radios were with the vessel’s Call Sign and MMSI.

An immersion suit of an appropriate size complying with the requirements of section 2.3 of the Code shall be provided for every person on board the ship. During demonstration of donning immersion suits, a crew member was observed wearing their assigned immersion suits that was not of an appropriate size for his height/weight. Ensure all persons on board are provided with an appropriate size immersion suit.

Photoluminescent exit sign with right arrow located in surge berthing points directly to the bulkhead lockers. Relocate photoluminescent exit sign with arrow point to the exit door or place additional photoluminescent arrows leading to the exit door.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. It was self-reported that Lifeboat No. 3 charging plug is cracked and an order has been made to replace it. It was discovered by the attending Marine Inspector that Lifeboat Nos. 2 and 4 plugs are also cracked and Lifeboat No. 1 pin in plug is bent. Replace plugs for Lifeboat Nos. 1, 2 and 4 to the satisfaction of Class.

The purpose of this regulation is to contain a fire in the space of origin. For this purpose, the following functional requirements shall be met: the fire integrity of the divisions shall be maintained at openings and penetrations. Repair door frame in cargo pump room to prevent the passage of smoke and fire into trunk access also an emergency egress to the satisfaction of the attending Class surveyor.

Repair holed and wasted supply air/exhaust ventilation ducting in cargo pump room access trunk to as original to the satisfaction of the attending Class surveyor.

Fuel cargo TLIs in cargo pump room were inspected with a tag labeled not working since YR2020. Vessel’s approved cargo transfer procedures requires use of cargo TLIs during cargo fuel operations. Vessel to repair cargo TLIs and prove operation to the attending Class surveyor. Prior to cargo operations, vessel to provide alternative procedures for sounding fuel cargo tanks in accordance with vessel’s SMS and have reflected in vessel’s cargo fuel transfer procedures to the satisfaction of the Class surveyor.

Each general emergency alarm signal must be an electronically-operated bell, klaxon, or other warning device capable of producing a signal or tone distinct from any other audible signal on the vessel. Prove operation of general alarms in way of vertical members with written numbers 116 and 127 located on the starboard side in No. 1 cargo hold.

Replace missing sign for to lifeboats in forward passage stairwell on 01 deck with new.

Repaint faded “To All Lifeboat Station” metal placards on the bridge port and starboard side bridge aft exterior bulkhead to as original. Recommend additional lifeboat station sign painted with an arrow on the aft most exterior bulkhead in way of the bridge.

Any part of such information may be provided in the form of audio-visual aids in lieu of the manual: muster at the assigned station. Per vessel’s muster procedures for abandon ship, vessel crew muster at the aft main deck for accountability before proceeding to assigned lifeboats. USCG Marine Inspectors discovered multiple muster station signs located inside the house and on aft exterior bulkhead with arrows directing persons to the port and starboard lifeboats as the muster stations. Update muster station signs to guide all persons to the designated muster station that is located on the aft main deck as per vessel’s approved emergency procedures.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Replace sun damaged and weathered ring buoy lines with float light attached with new and USCG approved lines.

Training manual shall be provided in each crew mess room and recreation room or in each crew cabin. Provide a training manual in the officers and unlicensed crewmembers in the recreation room.

Update Training Manual to reflect the approved life savings and arrangement plan: a. Life jackets and immersion suits stowed in the engine room (not included) b. Life raft/Lifeboat diagram: i. Life raft #9 for 25 persons (not 20) ii. Life raft #4 for 25 persons (not 20) iii. Diagram does not reflect the port 73 manned Lifeboat as a rescue boat c. Equipment Lists for the lifeboats: total rations and water are for 99 persons. Update to reflect each 101 and 73 person on P/S lifeboats.

Drills shall, as far as practicable, be conducted as if there were an actual emergency. Prior to departure, fire drill must be completed again to the satisfaction of the attending Marine Inspectors.

Electrical installations shall be such that electrical services essential for safety will be ensure under various emergency conditions. Prove operation of red lights in overhead near each exit in surge berthing.

74 SOLAS (2020) Chapter II-2, Part D, Reg. 13/2.1: Unless expressly provided otherwise in this regulation, at least two widely separated and ready means of escape shall be provided from all spaces or groups of spaces. Remove all equipment/gear blocking secondary means of escape from the inside of the KRB/MCMC workshop on 63 Level deck.

SOLAS 74 II-1/29.a.i: The auxiliary steering gear shall be of adequate strength and sufficient to steer the ship at navigable speed and capable of being brought speedily into action in an emergency. Steering gear No. 2 is unable to be brought into immediate service due to electrical issues. Make repairs to the No. 2 steering gear prior to vessel departure.

46 CFR 56.60-25(b): Nonmetallic flexible hoses must be in accordance with SAE J1942 . Emergency diesel generator nonmetallic lube oil turbo supply hoses do not currently meet SAE J1942 standards, install approved hoses or install approved SAE J1942 fire sleeves over existing hoses.

46 CFR 111.01-2(2): Electrical installations on vessels must ensure: protection of passengers, crew, other persons, and the vessel from electrical hazards. Properly lock out/tag out and secure electrical panel in crew laundry room with sign that says "Out of Order" to prevent accidental use and in accordance with vessel's SMS shipboard procedures.
