An inspection, either general or partial depending upon the circumstances,shall be made when any important repairs or alterations are undertaken. Steering is malfunctioning due to electrical communication errors. Repairs require review and approval from the attending Marine Inspector prior to the carriage of passengers; however, passengers may be aboard but not while underway.

All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering) of this chapter. 46CFR57.05 Port side / failed davit / hinge backing plate / welded in place by non-certified welder. 3rd party to conduct pt (dye pen) on 6011/45* fillet.

Bow thruster is inoperable due to sheared pin.

Flex coupling between chain drive #1 and chain drive #2, starboard side, failed. 19NOV2024: added note (**15NOV2024) to 835v; ABS approved interim part - flex coupling - installed and tested, in house, and approved via video clip (ABS & USCG). ABS stamped/approved part to follow. Lead time unknown. Action 50C open to extension.

All piping, valves, and fittings shall meet the applicable requirements of sucbchapter F. Firemain valve leaking @ station #27.

All doors, except those normally kept closed, shall be capable of release from the control station and from a position at the door. Door inoperable.

Automatic bilge pumps must be provided with bilge high level alarms that anunciate in the machinery spaces and @ a manned location. Bilge alarm IWO galley/theater void is inoperable. 46CFR70.20-1 sends to 62.35-10.

Each lifejacket must be marked with Type I retro reflective tape approved under 164.018. One Child lifejacket requires new tape - otherwise, fully functional.

Following a loss of propulsion vessel's crew conducted repairs to the propulsion system. Repairs require review and approval from the attending Marine Inspector prior to the carriage of passengers.

General emergency alarm signal must: (a)Be located in passenger and crew quarters where they can alert persons in spaces where those person may be maintaining, repairing, or operating equipment, stowing or drawing stores, or equipment, or transiting, such as public spaces, work spaces, machinery spaces, workshops, galleys, emergency firepump room, bow thrusters, storage areas for paint, rope, and other stores, underdeck passageways in cargo areas, steering gear rooms, windless rooms, holds of roll-on/roll-off vessels, and,except those that are accessible only through bolted manhole covers, duct keels with valve operators; and (b) Be audible in the spaces identified in paragraph (a) of this section with all normally closed doors and accesses closed; General alarm bells in theater zone and complimentary/adjacent deck below are inoperable. Repair as needed.

46 CFR 58.01-35: Auxiliary machinery vital to the main propulsion system must be provided in duplicate. An electrical short in vessel's propulsion drive circuitry rendered the vessel incapable of operating in normal mode, preventing the duplicate system requirement required by the regulations.

Halon flexible connections between cylinders/disribution piping must be inspected and tested in accordance with NFPA 12A, paragraph 6.3.1, except that hydrostatic testing must be performed every 12 years instead of every 5 years. Vessel has no record of hydrostatic testing. Test or renew.

Fire Safety Door #18 needs seal replaced.

Ventilation damper inside closet of cash control room is inoperational.

Fire Damper forward of the pilot house has seized.

Water was found in fuel due to the raw water cooler on fuel return line having hole in tubes.

COI expired. Complete COI exam.

Provide certification for potable water tank coating and drinking water.

Provide certification for paddle wheel bearing installation.

Submit for MSC or OCMI approval periodic safety test procedures (to include missing steering system and emergency internal communications test procedures)

Install a chlorine pump for approved sewage system capable of 2 GPH and ensure timing delivers wastewater at 1-2 Mg/L (PPM) of chlorine concentration.

Install new chain fall (open face, longer chain) or other manual launch system for life boats.

Obtain Permit to Proceed from USCG NOLA

Replace/Renew 04 Hydraulic pressure gauges for the steering pumps. Gauges not reading.

Replace hinge and latch for lifejacket stowage container on Texas deck/Starboard side.

Bow Thruster Field Excitor Board Blowing Fuses on startup.

When testing the Halon fixed fire extinguishing system it was discovered that the Halon bottle for the Emergency Generator space was empty and had completely discharged. You must refurbish or replace the fixed fire extinguishing system with an acceptable (CO2)system to the satisfaction of the attending marine inspector. Authorized to operate the vessel IAW plan for alternative arrangement.

Main bilge system priming/relief valve leaking at the stem. Engineering department instructed to monitor leak and report any changes. Item is already on work list to be addressed at drydock.

Air compressor in main engine room has a small leak.

Port and starboard chain drive pumps and filters leak. Repair leaks on pumps and filters.

Crack in deck located inside coaming around starboard engine. Drill stops at each end of crack and weld crack.

Starboard aft (fantail) void near paddle wheel has water inside. Pump out void and inspect/clean gasket surface of hatch cover.

Wiring for port and starboard generator needs to be located and terminated in a junction box.

Found various holes in deck behind ice makers in the storeroom below galley on starboard side. Bilge has suction port and high level alarms. Smaller holes are for condensation lines that drain into bilge, but larger holes serve no purpose. Repair larger holes and lower the bilge alarm inside the bilge.

Area around emergency escape hatch in lower storeroom below galley on starboard side has stacked boxes blocking the emergency escape signage. Relocate boxes and provide means to ensure nothing is stacked in front of sign.

Covers for the several junction boxes on top of gray water tank located behind ice makers in starboard storeroom below galley are missing screws and have wires exposed. Replaced screws and tighten down junction boxes.

Overhead (ceiling) in starboard side storeroom below galley has several wasted spots in ice maker area. With ABS approval vessel is permitted to temporary patch areas with steel plates of the same thickness after properly cutting back to good steel. Permanent repairs must be completed at next drydock.

Several fluorescent light fixtures in engine room overhead are missing covers. Replace light fixture covers.

The Class "A" fire door leading into the starboard side storeroom below galley has a broken window. Replace window in kind (wired type).

Service for halon fire extinguishing system needs to be completed. Schedule servicing.

The gauge for the portable fire extinguisher for the starboard rescue boat indicates it needs recharging. Recharge or replace fire extinguisher.

The aft range lights on the top of the wheel house and the anchor lights on the stern of the vessel are inoperable. Prove proper operation of these navigation lights prior to getting underway between dusk and dawn or in low visibility.

Spin valve for toilet supply water line is leaking in lower E/R space. Make permanent repair to spin valve.

RPM gauge for starboard engine is not working. Repair of replace RPM gauge.

Emergency generator remote fuel shutoff cable for 30 gallon diesel fuel tank broke when attempting to activate. Replace corroded cable for remote fuel shutoff.

Emergency generator has small exhaust leak. Repair exhaust leak on diesel engine for emergency generator.

Port diesel-electric engine has a small exhaust leak. Repair exhaust leak on port engine.

During hydrostatic test of fire hoses two (2) hoses were found to be leaking.

During test of fire door releases it was found that FSD 8, 13, 14, and 15 did not operate properly, #13 also was not labeled properly.

Found emergency battery connections for navigation equipment improper.

Found CO2 hose reel in machinery space with a 2008 service tag.

Ensure and demonstrate proper operation of double doors located fwd main and upper decks, IAW 46 CFR 72.05-25 (6)(7)

Remove abandoned wiring, port main motor tachometer. Advise CG MI of status of port / stbd motor tachometer operation.

Provide deadman sticks IVO all switchboards.

Remove abandoned wiring IVO main boom.

Properly wire electrical equipment on hurricane deck in accordance with 46 CFR 111.60.

Calculate electrical load and determine proper overcurrent protection for electrical equipment on hurricane deck.

Free-up fire dampers on hurricane deck on forward blkd below pilot house windows.

Renew flood lamp to starboard bow (safe refuge area) main deck.

Place spanner wrench on Fire Station #15.

Make permanent repair to port MDE turbocharge oil seal (weeping oil).

Repair audible alarm on navigation light panel in pilot house.

Renew polypropyline lines for bridge wing ring bouys.

Prove proper operation of fire screen doors on forward end of Victorian Theatre.

Prove proper operation of emergency escape hatch from ice room to galley and provide T-handle in galley by hatch.

Conduct annual servicing of 50# CO2 hose reel system in engine room. Company provided supplementary additional portable CO2 fire extinguishers as temp. replacements until servicing company arrived later this date.

Two fire hoses were noted with pinhole leaks during hydrostatic testing. Hoses were destroyed on sight and replaced with on board spares.

Automatic ventilation louvers in power ventilation system in engine room did not close tightly due to dirt/grease build-up. Louvers were cleaned and lubricated and retested sat.

Noted 2 general alarm bells in main salon in need of adjustment. Bells were adjusted and retested sat.

Numerous portable fire extinguishers were not labled to identify proper location on board the vessel. Extinguishers were labeled prior to the completion of the inspection.

General alarm bell in engineering control room not audible, although general alarm can be heard from other bells. Make repairs and prove proper operation. Bell adjusted and tested, sat.

Port fwd outside fire door from upper deck in victorian theater out to weather deck did not close properly when tested. Door closing mechanism adjusted to allow for proper operation.

Make permanent repairs to General Alarm (bells) main deck, starboard/midship and port/aft (galley).

Make permanent repairs to forward main deck door/s, between accomodation space and wx dck, IAW 46CFR72.05-25(b)(7)(i).

Update Fuel Oil Transfer Procedures, Identify PIC and update phone numbers.

Renew (4) wasted, worn, and punctured life ring buoys.

Calibrate, electronic rudder angle indicator (engine room).

Starboard Main Engine; Repair overspeed-trip solenoid, actuator for intake air, butter fly valve. Although overspeed shutdown is operating as designed, and engine is being secured, the "flapper" solenoid must be repaired or renewed.

Install stuffing tube. Sewage Space; bulkhead penetration, forward bulkhead, where chlorine injection system tubing penetrates bulkhead.

Fire door # 3, failed to close upon loss of power. Fire door #3 shall remain closed at all times while passengers are aboard or designate galley supervisor responsibility of attending to this fire door while passengers are onboard, in-writing (crew WQSB).

Fire Door #3 failed (twice) to close upon loss of electrical power. Make repair to magnetic release mechanism. Door to remain closed, while passengers aboard.
