At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Ladder utilized during man overboard drill was too long. Bottom of ladder was below waterline creating an unsafe condition to crew. Shorten ladder or provide alternate arrangements for recovery of man overboard to satisfaction of attending marine inspector.

For portable fire extinguishers, the inspections, maintenance procedures and hydrostatic pressure tests required by NFPA 10, (incorporated by reference, see § 114.600 of this chapter) with the frequency specified by NFPA 10. Where at least monthly manual inspections are conducted, the date the manual inspection was performed and the initials of the person performing the inspection shall be recorded. Vessel could not provide proof of required monthly inspections. Provide satisfactory evidence of inspection record keeping to satisfaction of attending marine inspector.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Light for the STBD ring buoy was damaged, had expired battery, and deteriorated line. Replace to satisfaction of attending marine inspector.

Each lifejacket must be approved under approval series 160.002, 160.005, 160.055, 160.155, 160.176, or 160.255 in subchapter Q of this chapter, or other standard specified by the Commandant, all of which require retroreflective tape. The vessel's stock of life preservers was found to have retro-reflective tape that was substantially degraded. The retro-tape on life preservers should be replaced with tape meeting the type I material standard found in subpart 164.018 of this chapter. Limited to daylight operations only.

Each lifejacket must be approved under approval series 160.002, 160.005, 160.055, 160.155, 160.176, or 160.255 in subchapter Q of this chapter, or other standard specified by the Commandant, all of which require retroreflective tape. The vessel's stock of life preservers was found to have retro-reflective tape that was substantially degraded. The retro-tape on life preservers should be replaced with tape meeting the type I material standard found in subpart 164.018 of this chapter. 46 CFR 180.25-5

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment under all practical conditions. Bilge suction line in lazarette not drawing suction. Repair to an operational condition to the satisfaction of attending marine inspector. 46CFR182.25-1

Exhaust shall be gas-tight throughout its length and shall be properly supported by non-combustible hangers or blocks. STBD exhaust manifold in vessel's tank room had an exhaust leak which left a pile of soot on the deck. This issue needs to be addressed/repaired. 46 CFR 182.20-20

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. STBD fuel shutoff valve did not close/seat all the way, which could allow fuel flow after actuated. 46 CFR 182.20-40(b)(3)

Each small battery installation must be located in a well ventilated space and protected from falling objects. Both MDE battery storage covers were missing stanchion/legs which allows natural ventilation and prevents the battery cables from being bent/pinched. This cover issue needs to be addressed/repaired. 46 CFR 120.354(b)

Where rigid nonmetallic material is permitted for use in piping systems, the following restrictions shall be adhered to. An acceptable metallic shutoff valve is installed adjacent to the through deck or through bulkhead fitting. This valve shall be operable from above the bulkhead deck. If two valves are installed, one on either side of the bulkhead, the valves need not be operable from above the bulkhead deck provided immediate access to both is possible. Where both plastic and metallic pipes are used in a bulkhead penetrating system and where the two materials exist entirely on opposite sides of the bulkhead, the required shutoff valve shall be installed at the bulkhead in the metallic part of the system, and may be locally operated provided immediate access is possible. The PVC piping that penetrates the aft bulkhead tank room, and connects to a metallic pipe in the lazarette, is missing required valves. 46 CFR 182.40-1(a)(1)(ii)

On a vessel on a protected route, a watertight coaming with a height of at least 75 millimeters (3 inches) must be provided under each weathertight door in a cockpit or a well. Two 2" holes were found in aft door coaming.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. One wingnut was found securing cables to the pilot house battery.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Galley microwave was plugged into surge protector portable cord.

Sea inlets and discharges, such as in closed systems required for the operation of main and auxiliary machinery, shall be fitted with a shutoff valve located as near to the shell plating as practicable. Both main engines and both generators are fitted with non-integral keel coolers with no shutoff valves installed. Shutoff valves to be installed prior to 2025 Hull Exam credit. 46 CFR 56.50-95(d)(1)

The minimum capacity of the power fire pump shall be 50 GPM at no less than 60 PSI. Fire pump output was less than 60 PSI.

Fuel shutoff valves shall be installed at the fuel supply lines. Remote fuel shutoff on starboard side failed to operate.

Life preservers shall be marked with the vessel's name. Not all life preservers marked with the vessel's name.

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection & one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank shall be operable from outside the tank compartment, preferably from an accessible position on the weather deck. Penetrations and openings in watertight bulkheads must have a means to make them watertight. MTWB between engine room and lazarette has an unsealed penetration on the upper port side. 46 CFR 171.114

All lights, receptacles & switches exposed to the weather shall be watertight & on vessels operating in salt water, shall be constructed of corrosion-resistant material. The second deck exterior lights were found with fractured plastic covers. 46 CFR 183.05-35(b)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The lifejacket stowage locations on the main deck doesn't match the number of lifejackets stored in that location. 46 CFR 122.604(f)

At next hull exam make permanent repairs to 02 inserts that were allowed as temporary repairs after relocating the main engine exhaust. 04 inserts were made on the aft side shell plate of the lazarette (02 gen/02 main eng). The temp inserts referenced are located outboard. Permanent repairs must cover two frame bays and extend doen to the chine or to good metal.

Repair exhaust leak on the port side in lazzarette for main engine.
