Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire-fighting systems and appliances that they may be called upon to use. Vessel is in ROS status and must conduct required drills once full crew is onboard. (SOLAS II-2/15.2.2.1)

A examination of the smoke cabinet found some of the air flow filaments and lights are not working. The fire detection systems shall be kept in good order so as to ensure their required performance if a fire occurs. Prior to loading and cargo demonstrate proper operation of the smoke detection cabinet to the RO.

Several of the oil/fuel containments are missing the drainage plugs. A ship of 300 gross tons of more must have a fixed container and a means of draining of removing discharged oil of hazardous material from each container of enclosed deck area without discharging the oil of hazardous material into the water. Prior to refueling operations provide proof to the USCG that the containments have the properly installed plugs.

Collection of oil from any pump, filter or heater that may contact a heated surface shall be prevented. Oil leak from gasket on #1 generator port turbocharger discovered during inspection. Repairs shall be made to correct leak and prevent fire hazard. 74 SOLAS II-2/33(a)(iv)(2)

Annual survey of machinery shall be to ensure that main & auxiliary machinery & electrical installations, along with their control systems are in satisfactory condition & fit for service. The following alarms/sensors were discovered to be either inoperable or in an error state during the inspection. 1. Inoperable high bearing temperature sensors for #1 and #3 SSDGs 2. Oil mist detector failure alarms on Port ME #7 and Starboard ME #4/#8 Alarm systems shall be repaired and made operational. 74 SOLAS (2020)I/10(b)(iv)

All inlets & discharges in the shell plating shall be fitted with arrangements to prevent accidental admission of water into the ship. Noted pinhole leak in forward port seachest vent line. Suspect area shall be repaired. 74 SOLAS II-1/14(i)(i)

Vessel has class notation for automation, and is approved for minimally attended machinery space operation. However, the starboard main engine clutch control panel air control diaphragm is leaking by and spare parts are not available in a timely manner with an unknown lead time. Concern is that if the starboard main engine were to receive a safety shutdown alarm when operating on both main engines, the starboard clutch clutch may not de-clutch on the shut-down causing the starboard main engine to be driven by the port main engine, resulting in potential damage. Therefore, when the vessel is arriving and/or departing port, or in areas that restricts the vessel's maneuverability, the Captain shall maintain a full engine room watch to ensure the starboard main engine de-clutches when required. This will remain in effect until the outstanding condition of Class, issued by ABS, has been cleared.

When CO2 is used as the extinguishing medium in cargo spaces, the quantity of gas available shall be sufficient to give a minimum volume of free gas equal to 30 percent of the gross volume of the largest cargo compartment in the ship which is capable of being sealed. Vessel reported 16.5tons of 25 tons of LP CO2 on board. The largest space protected is required to have a minimum of 20.7 tons. Prove proper quantity and proper operation of LP CO2 system prior to getting underway and to the satisfaction of the RO.

MAIN OF SOURCE OF ELECTRICAL POWER SHALL CONSIST OF AT LEAST TWO GENERATING SETS. VESSEL REPORTED (02) OF (03)GENERATING SETS CURRENTLY INOPERABLE.

Return OWS discharge and sludge piping back to original. Verify OWS piping matches approved drawing.

Replace OWS overboard discharge valve to in-kind. Valve found leaking.

Replace starboard seachest valve to in-kind replacement. Valve was found leaking at valve body.

Repair aft lowering hook on all six lifeboats. Vessel is restricted to 82 total persons allowed until repairs are made to the satisfaction of attending USCG and ABS surveyor.

Repair engine control fire door to auto close properly.

ABS Approved Periodic Automation Test Safety Procedure dated 14 July 2012 was found to be inaccurate and insufficient for Automation Testing. Submit an updated PTSP to ABS Engineering prior to completing Annual Automation Survey 4.

Shaft alley bilge discharge pipe has multiple soft patches. Make permanent repairs to the satisfaction of the attending ACP ACS.

Forward forepeak cargo access hatch is not watertight. Make permanent repairs to restore integrity of the boundary to the satisfaction of the attending ACP ACS.

Fore peak tank ventilation duct is wasted and allowing ingress of water. Make permanent repairs to the satisfaction of the attending ACP ACS.

Starboard engine fly wheel is missing top guard. Make permanent repairs to the satisfaction of the attending ACP ACS.

Overhaul all ballast system reach rods as many are not engaging valve stems. Make permanent repairs to the satisfaction of the attending ACP ACS.

Retroreflective on life jackets is not approved. Make permanent repairs to the satisfaction of the attending ACP ACS.

Generator fuel supply lines need support clips. Various support clips were missing and broken. Make permanent repairs to the satisfaction of the attending ACP ACS.

#4 Lifeboat gearbox is mislabeled for foreword/aft. Make permanent repairs to the satisfaction of the attending ACP ACS.

The company is responsible for ensuring that adequate resources are provided. Vessel SMS Medical Inventory 4.8 requires medications to be replenished before their expiration dates. Vessel requested medicine January 2017 and re-requested June 2017. Requested supplies have not reached vessel and some medication are expired. Company to investigate cause and determine corrective action. The Recognized Organization (ABS) is to attest to the verification of corrective action and conformance to ISM Code A/3.3.

Planned Maintenance Program onboard documentation is not available as required by ABS Rules Part 7.14.13.5. Provide required onboard documentation to the satisfaction of the attending ACP ACS.

Explosion proof light is missing plug. Make permanent repairs to the satisfaction of the attending ACP ACS.

Bilge foam distribution piping branch lines are rotted where they connect to main line. Make permanent repairs to the satisfaction of the attending ACP ACS.

#3 Lifeboat limit switch prevents boat from being raised. Make permanent repairs to the satisfaction of the attending ACP ACS.

ABS Approved Periodic Automation Test Safety Procedure is inaccurate and insufficient for Automation Testing as stated in ABS OSR 877. Vessel to operate with fully manned engine room (one licensed and one unlicensed crewmember) at all times until updated test procedure is reviewed and approved by ABS.

Vessel prohibited from operating under minimally attended machinery space status until ABS condition of class satisfied. (see conditions on COI) Maintain fully manned machinery operation until cleared.
