182.25-10(a) All vessels shall be provided with bilge pumps in accordance with Table 182.25-10(a). Over 65 feet, any number of passengers is required 02 fixed power pumps. Starboard side bilge pump is seized.

182.530(a) On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Port and starboard laz high water alarms were not indicating both audible and visual at operating station.

185.410(b)(1) The monitoring device(s) must: (1) Ensure the wakefulness of the crew in the event that the watchman required in paragraph (a) of this section is unresponsive. Roving watchman system is not alerting all spaces as required.

180.70(c)At least one ring life buoy must be fitted with a lifeline, 60 feet in length. One ring life buoy is required to have be fitted with a lifeline of at least 60 ft.

181.30-10(b) Portable fire extinguisher& shall be installed and located to the satisfaction of the Officer in Charge, Marine Inspection. Portable fire extinguisher port side on the pilothouse has incorrect bracket for extinguisher.

(1) Lighting fixtures shall be constructed in accordance with the requirements of Underwriters Laboratories, Inc. Standard for Marine Type Electric Lighting Fixtures, Subject 595, or shall bear the proper Underwriters Laboratories, Inc. "Marine Type" label. or shall be constructed accordance with the requirements of other recognized testing laboratories. Observation deck port side forward deck light has a bad gasket allowing water to enter. Install new gasket or replace fixture.

176.830(a) At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Refrigerator on 200 deck aft starboard side needs secured for sea.

MSD control cabinet door was not properly secured. 46 CFR 183.200

The minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The pump outlet must be fitted with a pressure gauge. Pump gauge was broken. 46 CFR 181.300

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Stbd engine room electrical outlet on the left side of the ladder was missing the cover plate. 46 CFR 183.340(g)

A space containing diesel machinery must be fitted with at least two ducts to furnish natural or powered supply and exhaust ventilation. Both engine room exhaust fans were reversed and not providing exhaust ventilation. 46 CFR 182.465(c)

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter). Aft 100 deck portable fire extinguisher was past due for annual servicing. 46 CFR 176.810

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Starboard HALON schematic diagram of the system was missing. 46 CFR 185.612(a)

Each item of lifesaving equipment with an expiration date must be replaced during the annual inspection and replaced if the expiration date has passed. Onboard flares were found expired. 46 CFR 185.726(c)

Except as otherwise required by paragraphs (b) and (c) of this section, an existing vessel must comply with the fire protection equipment regulations applicable to the vessel on March 10, 1996, or, as an alternative, the vessel may comply with the regulations in this part. Port HALON bottle was removed for servicing and was not installed at time of inspection. 46 CFR 181.115

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Liferaft hydrostatic release units were past the marked expiration date. 46 CFR 185.740(b)

New installations of fire protection equipment on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection on or after March 11, 1996, must comply with the regulations of this part. System design and installation must be in accordance with the Marine Design, Installation, Operation, and Maintenance Manual approved for the system by the Commandant. Vessel has new installation of fixed fire protection equipment serving the port and starboard engine room. The fixed fire suppression system design and installation is questioned by the OCMI. Provide approval by the Marine Safety Center (MSC). 46 CFR 181.115 (c) 46 CFR 181.410 (a)(3)

All calculations required by this subchapter must be submitted with the plans required by §170.075 of this subpart, Calculations must account for the weight of all loads carried aboard the vessel. Vessel had additional loads on board to include freezers and a canopy; Vessel's stability is questioned by the OCMI. Provide documentation of MSC approval of added loads after the April 12, 2005 Deadweight Survey. 46 CFR 170.090 (a)

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996 must comply with this part. Vessel had new installations of loads to include large freezers and air conditioning units that were questioned by the OCMI. Vessel also had replacement of wire and cable that were of insufficient wire gauge as well as daisy chained outlets from a single breaker. Provide documentation of MSC approval or complete a new electrical one line diagram for approval. 46 CFR 183.115 (c)

A water heater must meet the requirement of Part 53 and 63 in subchapter F of this chapter if rated at not more than 689 kPa and 121 degrees Celsius, except that an electric water heater is also acceptable if it is protected by a pressure-temperature relief device. Water heater relief valves were rusted/seized and needs to be exercised to show proper operation. 46 CFR 182.320 (a)(4)

Provision shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected. Dampers leading to the port and starboard engine room does not fully fit the duct and does not fully prevent the admission of air into the space protected. 46 CFR 181.20-95 (OLD T)

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not have an interconnected fire detection system in compliance with § 181.450.

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather deck. Vessel hull had several holes found below layers of paint. Vessel to blast paint to make bare metal hull accessible during next credit drydock. 46 CFR 176.25-5 (OLD T)

Electrical equipment on a vessel must be installed and maintained to: Provide services necessary for safety under normal and emergency conditions. Both port and starboard electrical controller enclosures for the generator do not close properly due to excessive rust on hinges and missing latches. 46 CFR 183.200

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. All navigation lights were inoperable. 46 CFR 183.420

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Public address system in Room 300 and 02 Deck Port Crew Room was inoperable. 46 CFR 184.610

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Upper deck light on starboard side and light in Captains stateroom were missing the covers. 46 CFR 183.410

Port forward AC unit is missing the cover for the electrical components. 46 CFR 183.200(b)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03–2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Alcohol breathalyzer was not calibrated within 12 months. 46 CFR 185.212

Except as noted in paragraph (c) of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. FCC Station license was expired. 47 CFR 80.13

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. COD was not located on vessel at the time of inspection. COD was located and placed onto the vessel prior to completion of inspection. 46 CFR 67.313

Both the port and stbd generators overheated to the point of shutdown. The port generator raw water discharge pipe developed a one inch crack aprox 6" up from were the pipe is connected to the generator engine exhaust overboard pipe.

Provide fire detectors in all overnight cabins that meet 46 CFR 181.405(c).

Repair or replace frozen door knob in Chef's cabin door.

Flammables found throughout the vessel improperly stored. Properly store flammables in flam locker of remove from vessel.

Electrical fuse panels for hot water heaters on aft 3rd deck show sign of significant overheating. Provide 3rd party electrical survey from certified marine electrician or electrical engineer attesting to proper operation.

Provide current hydrostatic release for 406 EPIRB.

Properly install IBAs.

Remove or properly terminate dead end wire in HVAC systems 4th deck.

Repair or replace broken electrical outlet on base of mast, 4th deck.

Remove or replace first aid kit on the wall aft, deck 2.

Repair or replace broken light fixture in hall deck two, fwd.

Provide an appropriate power outlet to eliminate the need for extension cord for portable AC unit in fwd "View Suites" passenger cabins. *alterations to vessel electrical system requires approved plans.

Provide GCFI electrical outlet in cabin 203 at sink outlet.

Replace broken spanner wrench at deck two fire main.

Provide fire detection testing and service report for vessel's fire detection and suppression systems.

Replace missing switch cover on aft bulkhead of hotel manager's cabin.

Remove or properly enclose exposed wiring inside of top of hotel manager closet on right side.

Repair or replace missing outlet cover on deck 2 aft deck electrical outlet.

Prove proper operation of Engine Room ventilation dampers

Remove chains and storage capabilities in port engine room entrance that inhibits the fire suppression warning sign from being seen or change location of sign.

Properly secure from movement laundry machine on aft deck.

Remove 12v battery from aft starboard main deck.

Adjust cabin door into galley on starboard side to prevent from rubbing on fire suppression system box.

Replace missing outlet cover on outlet for wine cooler fwd starboard side.

Properly secure refrigerator in hotel manager cabin.

Repair broken ground wire on compressor for HVAC on port side deck 1 HVAC.

Repair broken water tank ventilation piping support on bow.

Prove proper operation of bow anchor windlass.

Replace broken HVAC coolant line in HVAC system in engineer's cabin that services STBD dinning room.

Repair or replace missing thermostat assembly in engineer's cabin.

Several chest freezers and refrigerators found added throughout the vessel (06 total). They all need to be secured in place to prevent movement in heavy weather.

Added Freezers and refrigerators need a power source that does not require an extension cord. *Modifications to vessel electrical systems require approved plans prior to modification.

Provide a load analysis to MSC for approval for all added freezers and refrigerators.

Prove proper operation of port generator.

Prove proper operation of STBD E/R ventilation.

Replace missing lagging for STBD generator exhaust piping.

Prove proper operation of all bilge manifold valves.

Provide new indicator lamps for MSD electrical panel.

Remove liquid level fuel gauges in both engine rooms or meet 46 CFR 182.20.25(a)(5). If removed provide means of determining fuel level.

While the vessel is engaged in towing the vessel must be controlled by a licensed Master or Mate of towing. Vessel has until April 1st 2022 to man vessel with licensed Master and Mate of towing.

Vessel must provide updated stability calculations to include towing to Coast Guard Marine Safety Center to update vessels stability letter by April 1st 2022

Vessel shall provide correct light and day shape configuration for the tow being conducted prior to towing.

Vessel shall start and maintain records for towline and terminal gear used for towing astern or alongside prior to towing

Determine source of water ingress in port rudder.

Drill stop and gouge out crack in starboard stem and weld.

Replace starboard engine room bilge inboard strainer.

Make cabin/hull mounts available for inspection.

Crop and renew to good aluminum wasted transom plating in starboard side swim step void space.

Crop and renew wasted gussets in swim step void space along aft transom plate at bottom plate intersection.

Provide U/T shots of wastage in forward water tanks internal sideshell.

Make sea valves available for inspection.

Port side engine room fixed halon system, pull knob for engine shut-down missing pull knob. Replace missing pull knob.

Starboard side engine room fixed halon system, pull knob for generator shut-down missing pull knob. Replace missing pull knob.

Update emergency instructions to include abandon ship procedures.

Vessel shall have two fixed power bilge pumps, port bilge pump was not available for testing.

Liferafts must be serviced no later than the month and year on its servicing sticker, all liferafts were beyond their serviceable dates.

Vessel must be outfitted with a Cat 1, 406MHz EPIRB, no EPIRB was provided. Vessel restricted to less than three nautical miles from shore.

The vessel's firemain was winterized/not available for inspection, prove proper operation.

Underway drills are required to be conducted and logged at least once every three months, vessel rep was unable to provide logs or records indicating such.

A vessel must not operate without a valid SPV decal affixed to the vessel, provided SPV decals were expired.

A visual and audible bilge high level alarm must be provided at the operating station, demonstrate starboard engine room and troubleshoot starboard lazz alarm.

Pilot house visual bilge high level alarms were improperly labeled.

Repair or replace the emergency light located on the aft bridge deck passageway.

Emergency fuel shutoff valves were not available for testing.

Each lighting fixture must be fixed, fix or repair fixture hanging from the wires in the port engine room.

Identified multiple DC and AC temp wiring installations in both engine rooms and the starboard lazz, properly remove and terminate all temp installations.

Fuel shutoff valves must be provided at the engine end, repair or replace all missing Racor valve handles.

Identified the following bilge manifold stops valves did not function properly, the port cross over and crew bilge suction valves, and the starboard cross over bilge suction valve.

Identified installations that are ran through both engine room hatches that prevented them from being properly secured, remove interferences.

All cable and wire must be installed with metal supports not more than 24 inches apart, a majority of wire runs in both engine rooms and lazz'es are not properly supported.

Spaces must not have excessive accumulation of trash, debris or other matter that might create a fire hazard, clog bilge pumping systems or block escapes, remove excessive accumulations in both engine rooms and lazz'es

Shutdown switches for both engine room fixed fire fighting systems are improperly located in the space being protected. Relocate to an acceptable location.

Identified weld fracture in the port main diesel engine exhausst piping.

Renew un-serviceable main diesel engine sea valve.

The STBD #1 void was inaccessible due to excessive water. Dewater and make void available for inspection.

Identifiied an incomplete bulkhead weld in the stbd stern void.
