A vessel must be provided with bilge pumps in accordance with Table 182.520(a). A submersible electric bilge pump may be used provided that: The pump is listed by an independent laboratory as meeting the requirements in UL 1113. Vessel bilge pumps operable but not meeting the requirements of UL 1113. Pumps require renewal for UL listing (Rule 2000). 46CFR182.520

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Blower for gasoline fuel tank space did not shut down upon testing of fire fighting system. Blower operated to completed air changes prior to starting engine. Captain will operate blower to make air changes then secure blower once underway. Operator given 30 days to wire blower into pre-engineered fixed firefighting extinguishing system. 46 CFR 181.420(a)(3)

Exhaust blower switches must be located outside of any space required to be ventilated by this section, and must be of the type interlocked with the starting switch and the ignition switch so that the blowers are started before the engine starter motor circuit or the engine ignition is energized. Vessel did not have interlock for starting and ignition switch. Provide interlock with starting switch and the ignition switch for gasoline fuel tank space blowers. 46 CFR 182.460(e)

Each portable fire extinguisher on a vessel must be of an approved type. Bracket for fire forward extinguisher is broken. Replace with new approved bracket as referenced on label of portable fire extinguisher. 46 CFR 181.500(a)

Battery installations must be mounted too protect against shifting of the vessel and free from exposure to water splash or spray. Vessel's battery boxes are not secured. Provide suitable means of securing to prevent shifting. 46 CFR 183.350

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Training documentation not provided for crew. Provide training for crew and provide documentation that they completed the training. 46 CFR 16.401(b)

Each vessel must be equipment with at least two operable portable battery lights. Vessel did not have two portable lights. Provide portable lights. 46 CFR 183.430

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing MMSI and LAT/LONG for DSC operation. Provide operational DSC VHF radio. 47 CFR 80.905

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. 46 CFR 185.604(f)

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Missing double hose clamps on bilge pump system. Provide double hose clamps on both suction and discharge ends of flexible hose. 46 CFR 182.410 (d) (1)

An adult life jacket must be provided for each person on board. Vessel missing 02 adult life jackets. Provide 02 adult life jackets. Vessel authorized to carry 10 passengers until life jackets provided. 46 CFR 180.71

To meet Work Instruction 31 Oxygen must be onboard and ready for use at all time. Items missing from Oxygen container to allow for ready use. Provide hose, mask, and full O2 bottle for oxygen. Vessel crew may not enter water until O2 is filled equipment provided, and the system functions as designed. Work Instruction 31

As appropriate for the intended voyage vessel must carry adequate and up to date charts, US Coast Pilot [name withheld], and Tide Tables. Nav publication either missing or not updated. Provide updated nav publications. 46 CFR 184.420

A hatch cover must be attached by captive chain to prevent loss. Provide captive chain for hatch cover. 46 CFR 179.360

All-round light shall show an unbroken light 360 degrees over an arc of the horizon. All-round light obstructed in current location by being on to short of a pole. Provide new extended pole for All-round light to show full light. Vessel restricted to daytime ops until operational. 33 CFR 83.21

Vessel required to have interlock for blower switch. Vessel exhaust blower interlock not operational on vessel that has fuel tanks for gasoline. Port side Sea Fire pre-engineered fixed fire fighting system gauge was in the discharge position. Provide a serviced pre-engineered fixed fire fighting system that shows the agent is in the charged/green/ready state in accordance with the gauge. Pre-engineered systems must be one whole system. Systems manufactures cannot be mixed.

Alcohol testing device not on board vessel. Provide alcohol testing device.

Emergency instruction list not on vessel. Provide and mount emergency instruction list containing the actions to be taken during fire, heavy weather, or man overboard at operating station.

Starboard side fuel vent damaged. Discharge ends of fuel vent pipe must be fitted with removable corrosion resistant wire of at least 30 x 30 mesh or flame arrestors.

Navigation side lights not displaying appropriately. Provide side lights that show an unbroken light over an arc of the horizon of 112.5 degrees and so fixed as to show the light from right ahead to 22.5 degrees abaft the beam on its respective side. Prior to nighttime operations.
