Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electric shock. Light switch cover was found loose with exposed wires in collision bulkhead space. 46 CFR 183.200(b)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel's first aid kit expired on 31OCT25. 46 CFR 184.710

Provision shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected. No means were provided to close STBD side ventilation to the engine room. 46 CFR 181.20-35(a)

Equipment mounted on a hinged door of an enclosure shall be constructed or shielded in such a manner that no live parts of the door mounted equipment will be exposed to accidental contact by a person with the door open and the circuit energized. Electrical panel #1 found with open space for circuit breaker. Provide filler plate or breaker to close open space to prevent accidental shock. 46 CFR 183.10-10(b)

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Multiple A/C electrical receptacle face plates were found broken/damaged in engine room creating an electrical hazard. 46 CFR 177.35-15(a)

Covered metal trash containers shall be provided in lounge areas and the spaces shall be maintained to minimize fire and safety hazards and to preserve sanitary conditions. General use plastic trash cans were found in lounge areas (Passenger berthing area), remove or provide metal trash cans. 46 CFR 177.30-7(d)

The two required fixed power bilge pumps shall be self-priming and shall each be driven by different sources of power. If one pump is driven off the main engine in a single propulsion engine installation, the other shall be independently driven. The pumps shall be permanently connected to the bilge main and may also be connected to the fire main. Discharge hose from electric powered pump/fire pump is arranged in a way that may restrict the outflow and has only 1 hose clamp. 46 CFR 182.25-10(c)

All wiring and cables shall be suitable for marine service and for the particular installation used. In addition, Flexible or portable cords such as SO, STO, and similar types cannot be used for permanently installed wiring. Portable cords were found extending the electrical circuit up to the flying bridge to supply A/C power to 03 deep freezers. Vessel may continue to utilize portable cords (14AWG) as a temporary means to supply power to deep freezers while the vessel is under command until permanent installation is installed. Prior to any electrical modifications; Provide single line electrical drawing of proposed electrical installation to facilitate A/C receptacle power supply to bridge, drawing shall also include list of materials, length of circuits, #AWG, and over-current protection rated for the entirety of the circuit.

Each distribution panel or switchboard must be totally enclosed and of the dead front type. Engineroom distribution panel dead front had a missing breaker cover.

The Marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Vessel to renew/replace plastic plugs in both SSDG racor bowls as well install metal flame shield on the STBD SSDG racor. (old t 176.25-50)

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Vessel did not have battery box tops secured properly.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. MI identified (02) portable extinguishers to be in need of servicing. (01) was in the galley area.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Vessel did not have the upper deck passenger limit posted/labeled.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The vessel experienced a loss of propulsion on the port main diesel engine. Vessel must prove proper operation of the port main diesel engine prior to movement.

All mattresses must comply with either: The U.S. Department of Commerce Standard for mattress flammability 16 CFR part 1632 or IMO Resolution A.688(17). None of the vessels mattresses on board meet either standard. Provide documentation that the mattresses meet either standard or replace the mattresses with compliant mattresses.

Three of the vessels bunk pads were determined to be post 1996 and non compliant. Provide documentation that the mattress meet either standard or replace the mattresses with compliant materials. Over night passengers restricted to 34.

Vessels fuel tank vent pipes must be fitted with flame screens. Flame screens were missing.

Vessel did not have a garbage management plan.

Vessels smoke alarm in berthing and accomodation space was not operational.

Vessels lifebuoy lightwas missing batteries and not operational.

Lifebuoy with self igniting light needs to replace retro material.

Lifebuoy missing name of vessel.

Vessel relocated portable extinguisher and left bracket in stairwell and a hazard for passengers.

Vessels distribution panel blanks covered with duct tape.

Vessels Emergency light not operational.

Vessel's medicine /first aid chest had expired medicine.

Vessel did not have drug testing consortium information on board vessel and no EAP posted.

Vessel could not provide proof of enrollment.

Vessel had dead-end wires located behind the navigation equipment IVO captains accomodations.

Vessel could not provide certificate of servicing.

Vessel could not provide proof of service.

Vessel missing garbage placard and waste management plan.

Through hull ball valve in Engine Room coolant line had ball rotate past closure.

Watertight transverse bulkhead from lazarette to Engine Room had 2 non watertight penetrations.

All flares onboard were expired. VSL required to have 6 day and 6 night prior to carrying passengers for hire.

Provide servicing Certificate for fixed fire fighting system in the machinery space.

Provide servicing certificate for one additional .

Install missing light fixtures in accommadation spaces raks #33 and #18.

Install sea rails on galley stove.

PROPERLY REPAIR 12 INCH FRACTURE IN FOREPEAK AREA AT WATERLINE.

CONDUCT SATISFACTORY FIRE, ABANDON SHIP AND MAN OVERBOARD DRILLS IN PRESENCE OF MARINE INSPECTOR.

CONDUCT THE FOLLOWING DRILLS IN THE PRESENCE OF A MARINE INSPECTOR: FIRE, ABANDON SHIP AND MAN OVERBOARD.

PROVIDE AND MOUNT ADDITIONAL SIGNS/PLACARDS SHOWING ADDITIONAL PFD STOWAGE AREAS.

REPLACE TWO RING BUOYS WHICH WERE REMOVED FROM SERVICE. PROPERLY MARK AND APPLY RETRO REFLECTIVE TAPE IN ACCORDANCE WITH NVIC 1-87.

CONDUCT THE FOLLOWING DRILLS IN THE PRESENCE OF MARINE INSPECTOR: FIRE, ABANDON SHIP AND MAN OVERBOARD.

REMOVE FROM VESSEL THE ADDED ON DECKHOUSE EXTENSION, DECK WINCH AND BOOM ORPROVIDE STABILITY ANALYSIS FOR SAME TO USCG HEADQUARTERS (MSC) FOR APPROVALPRIOR THE CARRIAGE OF PASSENGERS.

A BII EXTINGUISHER IS NEEDED IN THE BERTHING AREAS

BUNKLIGHT IS MISSING FROM BUNK #32

PROVIDE RUBBER LINED FIRE HOSE AS PER 46CFR181.15-10(D)(1).

PERMANTLY SECURE MSD DISCHARGE VALVE, STENCIL "DO NOT DISCHARGE IN U.S. WATERS".

PROVIDE PORTABLE PUMP OR OTHER EQUIPMENT FOR FOREPEAK AS PER 46CFR182,25-1 (A) (1).

VESSEL MAY NOT CARRY PASSENGERS UNTIL REQUIREMTNS OF MSO PORTLAND LETTER DATED 26JUN86 HAVE BEEN COMPLETED TO THE SATISFACTION OF THE COGNIZANT OFFICER IN CHARGE, MARINE INSPECTION.

PROVIDE CORRECTED STABILITY LETTER TO CORRESPOND TO MANNING REQUIREMENTS OFTHE CERTIFICATE OF INSPECTION.

VESSEL MAY CARRY NO MORE THAN 74 PASSENGERS UNTIL AN ADDITIONAL ADULT PFD IS PROVIDED.
