All valves and closing appliances, or other mechanisms which may be required to be operated for damage control purposes in case of emergency shall be conspicuously marked with letters at least 1" high identifying the control and the direction of operation. Bilge/ballast system labels found to be faded or non-existant. Label system valves and tag out ballast system (not in operationg). 46 CFR 78.47-38(a)

Structural Fire Protection Plan was not updated when an A-60 boundary was added to the overhead of the bow thruster space. Vessel management is to submit an updated Structural Fire Protection Plan and cut sheets for the materials used in creating the bow thruster A-60 boundary to MSC for approval.

Emergency lights in passenger cabin stbd side do not work under emergency power. Prove proper operation of embergency lights.

During de-watering testing of voids it was noted that water was able to bypass the check valve in void #4. Repair or replace check valve in void #4.

Replace broken stuffing tube on upper deck stbd side embarkation light.

Vital systems that are automatically or remotely controlled must be provided with an alarm system for detection of a failure or unsafe condition. The vessels electronic propulsion controls for the main engines was found operating in an alarm condition. When C/E was questioned about the alarm and he stated that it was a voltage issue with the system. The C/E did not have the manual available onboard to read the alarm code. C/E also stated that the alarm has been intermittent for two weeks. Vessel was able to restart the engines and clear the alarm. Manufactures representative was contacted and he stated the caterpillar engine interface with the twin disc controls causes the alarm when there is an error in the caterpillar engine. Vessel to conduct sea trial to verify if the alarm returns. Vessel not to sail with propulsion controls in alarm condition.

Fire screen doors shall have a latch arrangement that is automatically engaged when by the operation of the door. 03 Fire screen doors will not latch due to the door hitting the frame prior to engaging the latch. The doors are located in the following locations. Galley to passenger cabin, car deck to upper deck stbd side, and upper deck to passenger cabin stbd side.

Provide a sign on the bow thruster room door stating that the space is protected by CO2

Provide emergency light in stbd aft stairwell leading to car deck. Light was removed at shipyard.

Mark all emergency lights missing the E marking

Re-secure main electrical cable from generators to electrical panel and add chafing gear to prevent further damage to wire casing.

Repairs were made to the sprinkler system piping stbd side aft on the overhead of the freight deck. Vessel reps unable to test piping at this time. Test sprinkler system piping in area of repairs prior to carriage of passengers.

Terminate lighting cable for light fixture in steering space in a junction box.

Terminate jacket water alarm cable for the emergency generator in a junction box.

02 emergency lights were found to be not working.

De-scale and de-water #4 void. Vessel crew is required to inspect #4 void at least twice a day for the presence of water. If any water is observed it shall be reported to Sector Long Island Sound.

Fixed fire extinguishing systems could not be tested and serviced on day of inspection. Sea Safety will conduct all testing and servicing of portable and fixed extinguishing systems in the presence of a marine inspector on 16May19.

Replace stuffing tubes in deck iwo hydraulic power panel and stbd side upper car deck

Prove propper operation of aft lower mast light.

Replace broken light covers on fixtures located stbd side main deck aft.

Update periodic safety test procedures to include over speed shutdowns for the #1 and #2 generators as well as both main engines.

Prove proper operation of e-gen low lube oil and high jacket water alarms on the bridge.

Galley sanitation found to be unsatisfactory. The removable grease vents over the deep fat fryer and griddle were found with excessive grease and dirt creating an increased asked risk of fire. The galley floor was found with excessive amounts of dirt and grime throughout especially in corners and under equipment. Temperature sensitive food was also found left out for extended periods of time creating a health risk to passengers. Vessel management to ensure galley is satisfactorily cleaned and food safety measures are followed. Vessel management to provide a written fleet wide corrective action plan to the Coast Guard to preventfuture incidents. Future occurrences could lead to food service suspension.

Update vessel periodic testing sheets to include boiler and CO2 testing.

The search light control arm leaks water. The water leaks into the pilot house through the control arm and power switch. This condition poses an electrical safety risk. Make top of pilot house and search light watertight.

Compliance with the standards established by the American Bureau of Shipping, will be considered satisfactory evidence of the structural efficiency of the vessel. While conducting first quarter re-inspection the fuel tank void was found flooded with approximatly 4 feet of water. Crew was directed to de-water the space and make ready for inspection. After the space and make ready for inspction. After the space was dewated, a close up internal inspection found a three inch transverse crack running under the fourth longitudinal frame. Make permanent repairs to the crack t the satisfaction of attending marine inspector.

Vessel shall be incompliance with their COI at all times. Vessel has removed 2-25 person IBA's for servicing. Vessels total persons permitted reduced to 906.

Double doors capable of independent operation and latching may have a clearance between the doors not exceed 1/8 inch. Self closing automatic fire door did not latch. Prove proper operation of fire door closing device.

Repair check valve in void #4. Check valve is stuck in the open position.

Repair emergency light port side aft on car deck.

Provide placard for life jacket locker, starboard side forward on the sun deck.

Repair latch on the deli vent damper, starboard side on the sun deck.

Label general alarm, port side aft car deck.

Pipe backed out causing a dishcharge of CO2 into the car deck. Vsl is to have servicing tech provide report of failure and proper operation of system and to check all fire fighting systems on board the vsl. The vsl is to maintain a live fire watch in the space with extinguishers and 2way communication while operating the vessel. Once the def is cleared and requirements are met the vsl may opererate without the live fire watch.

PARK CITY had a loss of steering or a loss of control to the steering from the bridge, but was able to control steering from the pilot wing and emergency steering in the engine room. Engineering noticed loose cables and sodered them and this appreared to fix the problem. While USCG marine inspector was onboard pump #2 was not communicating from bridfe control, MI noticed a fuse was blown, engineers replaced fuse and steering system was fully functional and def was cleared.
