An AFFF drop line was found to be wasted/fractured at the connection point to the main foam distribution line IVO the No.3 CT Winch at Frame 34 on the tank deck. The damaged foam line will need to be cropped and renewed to the satisfaction of the attending ABS surveyor. Vessel is restricted from carrying cargo until the foam system has been permanently repaired.

With the two pumps running simultaneously delivering through the nozzles specified in paragraph (g) of the Regulation the quantity of water specified in sub-paragraph (I) of this paragraph through adjacent hydrants, the following minimum pressures shall be maintained at all hydrants: 40 psi. REPLACE EXISTING 2500 PSIG RELIEF VALVES INSTALLED ON THE RAM TENSIONOR ACCUMULATORS WITH RELIEF VALVES OF PROPER CAPACITY AND SET PRESSURE (2000 PSIG). ENSURE VALVES DO NOT LIFT AT URE GREATER THAN 2000 PSIG.

IAW USCG COMMANDANT (G-MVI-1) LETTER 16703/16711/ DATED 26FEB88, SUBMIT PLANS TO USCG MARINE SAFETY CENTER TO REMOVE VENTILATION PIPING WHICH PENETRATES AUXILIARY FUEL OIL STORAGE TANK, AND MAKE MODIFICATIONS AS NECESSARY.

REPLACE ALL 1 1/2" FIRE HOSE WITH APPROVED TYPE OR VERIFY COMPLIANCE WITH USCG REGULATIONS:

DEMONSTRATE PROPER OPERATION OF THE ELEVATOR AND ASSOCIATED SAFETY DEVICES (DOOR WILL NOT OPEN WHEN CLOSED AGAINST OBJECT).

CONDUCT SATISFACTORY HYDROSTATC TEST OF THE CARGO PIPING AND CARGO HOSE. TEST CARGO PUMP RELIEF VALVE AND GAUGE.

REPAIR OR RENEW CARGO VALVE LOWER SUCTION, FWD PUMPROOM.

PROVE INSTALLATION OF THE SIDE SHELL ADDITIONS FORWARD JUNGLE DECK MODIFICATIONS ARE IN ACCORDANCE WITH USCG APPROVED PLANS.

CONDUCT SATISFACTORY TEST IN ACCORDANCE WITH USCG APPROVED AUTOMATION PROCEDURE FOR MODE 3 AND MODE 4 OPERATIONS REQUIRING CLUTCH TO BE ENGAGED. RECORD RESULTS IN OFFICIAL LOG AND FORWARD COPY TO [name withheld].

DEMONSTRATE PROPER OPERATION OF 10 CM. RADAR ARRAY AND MOTOR.

CONDUCT SATISFACTORY HYDROSTATIC TEST OF THE (36) AIR RECEIVERS ASSOCIATED WITH THE RAM TENSIONER SYSTEM.

REPAIR OR RENEW THERE FACTORY BASE (CONE) IN #1 AND #2 CLAYTON AUXILIARY BOILER.

REPAIR OR RENEW THE (RTD) RESISTANCE TEMP. DEVICES FOR THE PORT MAIN ENGINE#5 MAIN BEARING AND PORT MAIN ENGINE RTD OUTER THRUST BEARING.

AMEND POSTED FIRE CONTROL PLANS TO INDICATE NEWLY INSTALLED FIRE PROTECTIONEQUIPMENT.

PROVE INSTALLATIONS OF THE CARGO LUBE OIL TANK STRUCTURAL MODIFICATION AND CARGO LUBE OIL FILL TRANSFER AND DISCHARGE PIPING MODIFICATIONS ARE IN ACCORDANCE WITH USCG APPROVED PLANS.

PROVE INSTALLATION OF THE INCINERATOR ROOM (1-84-2) FIXED HALON SYSTEM IS IN ACCORDANCE WITH USCG APPROVED PLANS.

OBTAIN COAST GUARD APPROVAL OF THE SEVEN EXISTING RAM TENSIONER ACCUMULATORS OR INSTALL ONE OF AN APPROVED TYPE.

PROVIDE JOINT DETAILS FOR CARGO PIPING SYSTEMS IN ACCORDANCE WITH 46 CFR 56.30-30(B) OR PROVIDE ALTERNATE DETAIL OFFERING EQUIVALENT FIRE RESISTANCE.

OBTAIN COAST GUARD APPROVAL OF THE SEVEN EXISTING RAM TENSIONER ACCUMULATORS OR INSTALL ONE OF AN APPROVED TYPE.
