Each fire hose nozzle must be of corrosion-resistant material and be capable of being changed between a solid stream and a spray pattern. Upper deck fire hose nozzle could not be changed from spray pattern to solid stream. 46 CFR 181.320(d)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel did not possess placards for all types of life jackets on board. Post placards containing donning instructions for all types of life jackets on board to satisfaction of attending marine inspector.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. Space containing children size life jackets was not labeled. Label space to satisfaction of attending marine inspector.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Replace all combustible waste receptacles with noncombustible waste receptacles to satisfaction of attending marine inspector.

The sidelights of vessels of 20 meters or more in length shall be fitted with matte black inboard screens and meet the requirements of 46 CFR 84.15. Vessel is greater than 20 meters and the sidelights are not fitted with matte black inboard screens. - 46 CFR 84.05(a)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Grab lines on ring buoys are deteriorated. Renew grab lines to the satisfaction of the attending marine inspector.

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. The expiration date of the battery may be marked on the battery or the owner or managing operator may have a record of the expiration date from the manufacturer of a battery marked with a serial number. Battery in ring buoy light is expired. Replace to the satisfaction of the attending marine inspector.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. STBD generator had excessive oil in is drip pan and windings that need to be cleaned. Repairs/Clearing and/or replacement is required. 46 CFR 176.830(b)

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Maintenance was being conducted but not documented in a log. This corresponds with Monthly maintenance under NFPA 10. 46CFR176.810(b)(2)(iv)

Each buoyant apparatus must have Type II retro-reflective material meeting subpart 164.018 of this chapter on each side and end. All buoyant apparatuses have worn/missing retro-reflective tape that needs to be installed/replaced. 46 CFR 160.010-4(p)

Installation of diesel engine exhaust pipe shall be in accordance with 182.15-20. Protective gratings shall be provided at such locations where persons or gear might come in contact with the exhaust pipe. Both MDE's were missing lagging at the beginning connection point of the exhaust manifold and at all connection bolts. Guards/Lagging are required to prevent crew injuries. 46 CFR 182.20-20 46 CFR 182.15-20(g)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Receptacle in the center of the engine room between the MDEs was missing its cover, allowing access to electrical hazards. 46 CFR 183.01-15(a)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Both MDE's have belt driven pump (bilge pump and fire pump) without guards. Exposing crew to possible injury from rotating machinery. Guards must be provided. 46 CFR 176.830(a)

Mechanically propelled "S" vessels which are ferry vessels or which carry more than 49 passengers and mechanically propelled "L" vessels shall comply with the provisions of Sub-chapter F of this chapter. Nonmetallic flexible hose may be used in lengths not exceeding 76 cm (30 inches) where flexibility is required. Forepeak bilge connection in the engine room from the bulkhead to a stop valve has a straight nonmetallic flexible hose where flexibility is not required. All other bilge connections in the engine room are metallic. Remove nonmetallic flexible hose and install metallic pipe connection or provide MI with justification for its presence. 46 CFR 182.40-5(b) 46 CFR 56.60-25(b)(2)

A propulsion engine control system, including pilothouse control, must be designed so that a loss of power to the control system does not result in an increase in shaft speed or propeller pitch. Submit Qualitative Failure Analysis (QFA) and Design Verification Test Procedures (DVTP) of the vessel's propulsion engine control system for loss of power.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel must submit plans for re-power of both main engines. Once plans are submitted to Marine Safety Center and approved, marine inspector will attend to test approved plans for main engines.

Propulsion machinery shall be capable of operating at constant marine load without exceeding it's designed limitations. Port side propulsion system is inoperable.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Port engine is missing its guard for the fire pump belt.

Penetrations and openings in watertight bulkheads must have means to make them watertight. In the machinery space, sealant used for each watertight bulkhead penetration (on both bulkheads) is worn, making the penetrations not watertight.

An adult lifejacket must be provided for each person carried on board a vessel. 02 lifejackets were condemned during inspection and another 02 lifejackets were missing from the vessel.
