When a vessel owned in the United States is operating in waters that are not subject to the jurisdiction of the United States, the testing requirements of 16.210 and 16.230 do not apply to a citizen of a foreign country engaged or employed as pilot in accordance with the laws or customs of that foreign country. Vessel did not have an active DAPI program that last the duration of the COI or records of chemical testing of employees as required by regulations.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. The master was unfamiliar with his duties and vessel was missing emergency instructions and station bill.

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Vessel had a Master onboard and there was no record of training looged in the logbook or onboard.

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required.The vessel's waterlight had the incorrect battery as described by the manufacture.

A vessel over 26 feet in length must have an audible and visual bilge alarm at the operating station. Lazerette bildge alarm's visual indicator was inoperable.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR.” Aft emergency exit in passenger space not labeled. 46 CFR 185.606

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). MMSI number not input into VHF installation. 47 CFR 80.905(a)(1)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First Aid kit on board has expired contents and does not meet the requirements of 160.041 or an equivalent. 46 CFR 184.710

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Shaft coupling on aft end of engine fully exposed when rotating. 46 CFR 177.960

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. No proof of DAPI enrollment since November 2020. 46 CFR 16.230

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible. Chart is from 2020 with no corrections. 46 CFR 184.420(a)(1)

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. Missing cap for closure of MSD tank does not comply with 33 CFR 159.46 CFR 184.704

A manned oceangoing ship (other than a fixed or floating drilling rig or other platform) of 40 feet or more in length that is documented under the laws of the United States: The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship. No garbage management plan on board. 33 CFR 151.51

The owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances. Stanchions in way of cargo loading doors are cracked on both sides of the vessel. 46 CFR 176.802(a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel to replace generator unit to original approved plans. Plans to be submitted to Sector Charleston Prevention for review and approval.

A hatch exposed to the weather must be watertight. The deck hatch to access the engine room leaked water when hose tested.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. The soft patch on the port side of the deck above the engine room leaked when hose tested.

A weathertight door must be provided for each opening located in a deck house or companionway. The aft cabin door leaked when hose tested.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Plastic tie wraps were used as support components for wires in engine room.

Vessel passenger count restricted to 47 passengers due to 2 lifejackets being deteriorated.

Provide list of employees currently enrolled in drug consortioum.

Vessel's VHF Radio does not have DSC capabilities.

Penetrations in bulkhead between engine room and lazarette need to be made watertight.

Label all PFD's with vessel name.

Install approved hose or pipe in vsl's bilge/fire main system.

Vessel's pre-engineered fire extinguishing system needs annual service and must be tied in with shut downs of main engines and generator. Prove system is adequate capacity for space and install audible and visual alarm at the operating station.

Properly repair ships service generator. Gen set powers vsl's fire and bilge pumps.

Properly install draft marks or full load reference marks at vessels next dry dock.

Conduct emergency drills in the presence of the CG. Test vsl's bilge and fire main system in the presence of the CG.

Install approved hose of pipe in vsl's installed bilge system.

Propellers were damaged while underway. Vessel did not go aground.

Company has not contracted company to handle drug and alcohol testing.

Vessel has converted to new T regulation and needs fixed fire system

PVC piping on fixed bilge system located in the forepeak space is to be replaced, in kind, with suitable metallic piping prior to the vessel going back into service and COI being reissued.

Missing bow rail stanchions are to be replaced prior to the vessel being placed back into service and a COI being reissued.

The port propeller was noted to excessive pitting and the propeller blade was razor thin in areas. Propeller is to be refurbished prior to the vessel being placed back in service and a COI being reissued.

Port and starboard propellers are to be blued in the presence of a CG marine inspector prior to the vesse returning to service and a COI being reissued.

The port and starboard strut support brackets are wasted and must be replaced prior to the vessel being placed back in service and a COI being reissued.

Hull is to be audio gauged A/M in the presence of a CG marine inspector prior to the vessel returning to service and a COI being reissued.

Air conditioning supply line is to be double clamped IWO hose to sea valve connection prior to the vessel returing to service and a COI being reissued.

Sea valve for the sink supply and sea valve and houseing for the generator cooling supply line are to be renewed prior to the vessel returning to service and a COI being reissued.

Crack on starboard rudder is to be ground and rewelded prior to the vessel returning to service and a COI being reissued.

Vessels COD is no longer valid - COI removed from vessel.

Provide proof of current documentation & place original COD onbd vsl.

Provide proof of company's enrollment in a drug consortium.

Procure & install a hatch gasket for the lazerette hatch.

Provide securing device for aft cabin door to secure it when door is not closed.

PROVIDE A VALID FCC SAFETY CERTIFICATE. UNTIL THIS ITEM IS CLEARED THE VESSEL IS RESTRICTED TO 1000 FEET FROM SHORE.

INSTALL BLANKS IN "KNOCK-OUT" HOLES IN THE PORT AND STBD 24 VOLT JUNCTION BOXES ON THE FORWARD ENGINE ROOM BULKHEAD.

REPAIR LEAK AT 90 DEGREE ELBOW JOINT IN THE SUCTION PIPING FOR THE MECHANICAL BILGE PUMP.

PROVIDE A BRIDGE LOG THAT INCLUDES STEERING GEAR DRILLS, EMERGENCY STEERINGDRILLS, MONTHLY FIRE AND MOB DRILLS AND ALL CASUALTIES.

PROVIDE A TRAINING LOG BOOK THAT INCLUDES DATES AND DESCRIPTION.

MAKE PERMANENT REPAIRS TO THE BILGE SUCTION LINE IN WAY OF THE #2 COMPARTMENT.

REFASTEN BILGE LINE AND STRAINER IN THE BOW COMPT.

RENEW P/S MDE SEA STRAINERS.
