Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Vessel had a patch on the fuel tank covering wastage and several areas of cracking on railing and bulwarks. Owner/operator is to make appropriate repairs to be inspected and tested once complete.

The vessel is sufficiently maneuverable, arranged, and equipped to allow the crew to recover a helpless person from the water. Mount missing for block and tackle used for MOB recovery. Install permanent mount.

Each portable fire extinguisher on a vessel must be of an approved type. 2A extinguisher in operation station mounting bracket was broken.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Portable lights were missing.

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight: E/R hatch drain was clogged and gasket damaged.

On a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. Life line need corrosion-resistant clip to be able to be removed since only 1 ring buoy is carried.

The whistle, bell and gong shall comply with the specifications in Annex III to these Rules (33 CFR part 86). The bell or gong or both may be replaced by other equipment having the same respective sound characteristics. Bell mount was broken.

Stability information (stability details indicated on the Certificate of Inspection, a stability letter, or a stability booklet), is required on certain vessels by paragraphs (b), (c), or (d) of this section. Enough stability information, including stability calculations and assumptions made to use them, must be provided to allow the master to be able to determine operating guidelines, loading restrictions, and ensure compliance with the applicable intact and damage stability regulations of this chapter. Cargo loading areas as directed by stability letter were faded and not clearly marked.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Crew had difficulty recovering training manikin from water, crew conducted training and once deficiency #1 is corrected and block and tackle properly stowed and continued training should be corrected. Crew member had to enter water to get life sling on. Recommend vessel fabricate rescue platform.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency. Crew member had not conducted drill on vessel previously and no record of drills were present.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16) (incorporated by reference; see 46 CFR 175.600). Three type I PFDs require new application of SOLAS retroflective tape.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel's lifejackets and lifebuoy were marked with the wrong vessel name of Haig Point.

Whenever damage to the hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may require the vessel to be drydocked or taken out of service to assess the extent of the damage. Vessel ran aground while underway. Prior to carrying passengers vessel must conduct a bottom survey and submit the results to the attending marine inspector.

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight Engine room, steering gear room, and fore peak hatches leak and are not watertight. Aft cabin door is not weather tight.

The number of penetrations in a watertight bulkhead or deck must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Penetrations in the space under port operation station are not watertight.

Except as allowed by paragraph (d) of this section, each life jacket must be approved in accordance with either §160.002, §160.005, or §160.055 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.155 or 160.176. 10 adult life jackets have retro that is no longer functional.

Except as otherwise provided in this section every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station. Except on a vessel limited to daytime operations, the compass must be illuminated. Compass Light is not wired to a power source.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. STBD MDE PTO electrical connections have to be put in an enclosure.

Flexible hose designed for use with metallic hose clamps may be installed with two clamps, at both ends of the hose. P&S shaft vent have 1 plastic hose clamp on each end.

Vessel starboard side shell plating wasted just above waterline amidships. Section between 1st transverse frame and 3rd transverse frame in main engine compartment showed external signs of wastage and holed 1-2 inches above waterline. Deep pitting observed interior along stiffeners just above stitch welding.

Seal on main deck hatch leading to main engine compartment worn.

Vessel missing Coast Pilot and Light List

Install missing cotter pins or double nut port and stbd rudder post.

Install postive shut off valve in vsl's aft lazerette bilge and thru hull.

Install new hatch gasket in aft lazerette and engineroom hatches.

Replace broken valve handle on starboard MDE sea chest.

Remove auto discharge nozzle from vessel's fixed fire system in engineroom and test manual pull for proper manual operation and train crew on main space fire procedures.

Port engine failed without warning while carrying passengers. No cause of failure has been determined at this time and owners intend to replace engine. Issued no-sail CG-835 to make vessel available for in service inspection prior to carrying passengers.

Vessel suffered marine casualty on 10/20/05 and did not report it to the OCMI. The casualty involved the port main engine failing with passengers on board. The cause in inknown and the company has taken the vessel out of service.

Cover or install circuit breaker in opening in box on aft engine room bulkhead.

The COD was invalid, had expired in June.

Need to stencil Emergency Exit on the forward door of the passenger cabin.

PASSENGER COUNT IS REDUCED TO 38 PROVIDED PORTABLE SEATS (TWO) ARE MECHANICALLY FASTENED PRIOR TO GETTING UNDERWAY. PORTABLE CARGO BOXES PLACED ON BOARD MUST NOT INTERFERE WITH PASSENGER EGRESS.
