The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel was unable to successfully complete a VHF Radio DSC Test Call IAW MSIB 20-20. Complete a VHF Radio DSC Test Call IAW MSIB 20-20 to the satisfaction of the attending CG Marine Inspector. 46CFR176.840

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible. Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. The navigation chart is not up to date, due to no printed date of last chart correction. Provide an up to date chart for the area of operations. 46CFR184.420

Each lifeline on a ring life buoy must:(1) Be buoyant;(2) Be of at least 18.3 meters (60 feet) in length;(3) Be non-kinking;(4) Have a diameter of at least 7.9 millimeters (5/16 inch);(5) Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and(6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Ring life buoy lifeline does not meet above requirements. Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Lanyard does not meet above requirements. On a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy. No clip installed. Install ring life buoy life line, light lanyard, and light lanyard clip IAW above requirements. 46CFR180.70

Vessel was identified as having potential damage from Hurricane IAN. Prior to conducting repairs, contact the local USCG Marine Inspections office and submit a repair proposal for approval. Vessel is not permitted to return to commercial service until cleared by a USCG Marine Inspector.

A visual and audible alarm must be provided at the operating station. Port stern compartment high water alarm is inoperable. Check space for water ingress before and after each trip and log in ship's log until repaired. 46 CFR 182.530

The lights prescribe by these rules shall be exhibited from sunset to sunrise, in restricted visibility. The combination stern/anchor light is inoperable. Restricted to day light operations until repaired. 46 CFR 83.20(c)

As appropriate for the intended voyage a vessel must carry adequate and up to date: U.S. Coast Pilot [name withheld] for navigation area. 46 CFR 184.420

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Remount EPIRB so that it is in a float free position. 46 CFR 180.64

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Light for port high water alarm is inoperable. 46 CFR 182.530 (a)

At least one ring life buoy must be fitted with a lifeline. Each lifeline on a ring life buoy must: (1) Be buoyant; (2) Be of at least 18.3 meters (60 feet) in length; (3) Be non-kinking; (4) Have a diameter of at least 7.9 millimeters (5/16 inch) (5) Have a breaking strength of a at least 5 kilonewtons (1,124 pounds); and (6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Replace life line on ring buoy with specified type and length greater than 60ft. 46 CFR 180.70 (c)

For the purpose of flexibility only, flexibility hose installed in lengths of not more than 760 millimeters (30 inches) and subject to pressures of not more than 35 kPa (5 PSIG), may meet the following requirements: Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. Double clamp fuel lines at connections. 46 CFR 182.720(e)(3)(v)

Except on a vessel limited to daytime operations, the compass must be illuminated. Provide light for magnetic compass. 46 CFR 184.402(c)

Life ring is faded and cracked; replace in kind.

Label name of vessel on both sides of bow in 4 inch letters.

Repair or replace fuel shutoff valve on both fuel tanks.

Replace light list and coast Pilot with 2016 edition.

Repair or replace hatch covers

replace three clasps on starboard access gate.

Repair or replace canvas on most forward panel on starboard side.
