All engines must have at least two means for stopping the engine. Emergency stop housing on port and stbd engine is loose. Reattach housing. 46 CFR 182.200 (b)

Portable fire extinguishers must be located so that they are clearly visible and installation must be to the satisfaction of the OCMI. The fire extinguishers were stored in an unlabeled closed. Add a sign to identify location of fire extinguishers. 46 CFR 181.520

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. There were exposed wires in the lazarette and in pilot house. 46 CFR 183.200

Vapor barriers must be provided in machinery spaces. MI identified that the stbd engine room had insufficient vapor tight integrity. Conduct repairs to the satisfaction of the MI.

A hatch cover must be attached to prevent its loss.Shackle broken that attached the hatch cover to the vessel. Reattached and repair to satisfaction of MI. 46 CFR 179.360 (b) (2)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. While testing remote fuel shutoff of starboard engine, the valve did not reset properly and fuel in the line was drained from testing fireboy system, so it was only intaking air and not fuel. Therefore, it would not start. Operator couldn't resolve issue, so he called a technician nearby to assist and then fixed the issue.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. Vessel was found with 02 child lifejackets that need new lights.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel was missing portable lights for machinery spaces.

The stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. Current Stability Letter was not posted. Old one was. Post current Stability Letter as per above requirement.

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. Current COI was being kept out of sight. Post COI as per above requirement.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. 03 Cable penetrations, one on stbd side two on port need filled on the deck of the pilot house behind the console.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Multiple plastic trashcans found. Remove or replace with non-combustible type.

Each EPIRB, other than an EPIRB in an inflatable life raft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. Vessel was not conducting monthly EPIRB tests and logging it. Conduct an EPIRB test and log monthly.

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel drill logs showed no general description of fire drill scenarios. Vessel must conduct a fire drill and log the drill to the satisfaction of the Marine Inspector.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel Drill logs showed no general description of Man overboard scenario. Vessel must conduct a man overboard drill and log the required general description to the satisfaction of the Marine Inspector.

Ensure that one or more placards meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. Garbage placard had outdated requirements. Replace to satisfaction of MI.

The owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (4) Inspection of all railings and bulwarks and their attachment to the hull structure. Vessel’s starboard loading door was found with a crack in the laminate that was over 18” long. Submit repair proposal and conduct approved repairs to the satisfaction of the MI.

Vessel must undergo an annual inspection within the 3 months before or after each anniversary date. If the annual inspection reveals deficiencies in your vessel's maintenance, you must make any or all repairs or improvements within the time period specified by the OCMI. Vessel operator has failed to rectify deficiency in allotted time frame from annual inspection on 24APR2024. Vessel is restricted to limited coast-wise route until deficiency is cleared or vessel's COI will be amended to reflect new route change if not completed by 10OCT2024. 46 CFR 176.500 (b)(1)(iii)

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Vessel's port/starboard Fireboy systems did not automatically shut off engines during tests. Vessel to repair system to work as required and conduct engine shutdown tests to the satisfaction of the marine inspector. 46 CFR 181.420(a)(3)

The equipment required for lifefloats and buoyant apparatus must meet the following specifications. The painter must have a breaking strength of at least 1500 pounds. Vessel’s port life float painter has deteriorated beyond limits. Vessel to replace painter to the satisfaction of the marine inspector. 46 CFR 180.175(f)(3)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that the pump is permanently mounted. Vessels starboard lazerrete bilge pump is not permanently mounted. Vessel to permanently mount bilge pump to the satisfaction of the marine inspector. 46 CFR 182.520

At least one ring life buoy must be fitted with a lifeline and have a breaking strength of 1124 pounds. Vessel's life buoy lifeline has deteriorated beyond limits. Vessel to replace lifeline to the satisfaction of the marine inspector. 46 CFR 180.70(c)(5)

The ring buoy grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. Vessel's life buoy grab line has deteriorated beyond limits. Vessel to replace grab line to the satisfaction of the marine inspector. 46 CFR 160.050-3(c)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. 03 exposed wires found in port engine room. Vessel to cap exposed wires to the satisfaction of the marine inspection. 46 CFR 183.200(b)

All cable and wires must: Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Vessel has wire bundle exceeding 24 inches in starboard accommodation void. Vessel to install new clamps meeting the 24-inch space requirement to the satisfaction of the marine inspector. 46 CFR 183.340 (b)(4)

Batteries must be mounted in trays lined with, or constructed of, a material that is resistant to damage by the electrolyte. Vessel was found with multiple batteries in both stbd and port engine rooms without being mounted in trays. Vessel must mount all batteries in trays to the satisfaction of the Marine Inspector. 46 CFR 183.350(e)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Each first-aid kit must meet the requirements in ISO 18813:2006(E). Vessel has multiple expired medications. Vessel must meet the requirements of ISO 18813.2006(E) to the satisfaction of the Marine Inspector. 46 CFR 184.710

Vessel drill logs showed no general description of fire drill scenarios. Vessel must conduct a fire drill and log the drill to the satisfaction of the Marine Inspector. 46 CFR 185.524

Vessel Drill logs showed no general description of Man overboard scenario. Vessel must conduct a man overboard drill and log the required general description to the satisfaction of the Marine Inspector. 46 CFR 185.520(f)

For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include, but need not be limited to— (1) Training in the use and location of firefighting equipment and general firefighting knowledge, including: (i) Location of firefighting appliances and emergency escape routes; (ii) Types and sources of ignition; (iii) Flammable materials, fire hazards and spread of fire; (iv) The need for constant vigilance; (v) Actions to be taken on board; (vi) Fire and smoke detection and automatic systems on board; and (vii) Classification of fire and applicable extinguishing agents. Vessel crew training logs were found unsatisfactory displaying any description of the conducted training. Vessel must conduct crew training and log the required information needed to the satisfaction of the Marine Inspector. 46 CFR 185.420(b)(1)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Port Starboard light was found not operational. Vessel must repair to the satisfaction of the marine inspector prior to conducting night operations. 46 CFR 183.420

Each EPIRB, other than an EPIRB in an inflatable life raft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. Vessel was not conducting monthly EPIRB testings. Vessel conducted an EPIRB test the morning of the inspection, Marine Inspector discussed the requirement of monthly testing, corrected on the spot. 46 CFR 185.728(a)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Port and STBD shafts rotate and are exposed without a cover or guard to protect from the moving parts. Ensure that moving parts are protected appropriately. 46 CFR 177.960

Vessels operated beyond the 20 nautical mile limitation specified in paragraph (a)(1) of this section, but not more than 100 nautical miles from the nearest land, must be equipped with a MF–DSC frequency transmitter meeting the requirements of § 80.1101(c)(3) and capable of transmitting J3E emission and a receiver capable of reception of J3E emission within the band 1710 to 2850 kHz, in addition to the VHF–DSC radiotelephone installation required by paragraph (a)(1) of this section. Verify input of MMSI number into VHF radio and link GPS to VHF to allow DSC feature to be operational. 46 CFR 184.502 47 CFR 80.905 (a)(2)

Expired Lifesaving equipment: Each item of lifesaving equipment with an expiration date must be replaced during inspection, and replace is expiration date has passed. EPRIRB Hydrostatic release was found expired. Replace with new, reduce voyage to less than 3nm from shore until new hydrostatic release is installed. 46CFR185.728(a)

A hatch cover must be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Both PORT and STBD forward engine room hatch covers were found with out attachments. 46CFR179.360(b)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard or rail. STBD engine (FWD) pully drive was found exposed. 46CFR177.960

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Negative results not available for review. 46 CFR 16.260

The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. If the cognizant OCMI determines that there is no suitable mounting surface aboard the vessel, the emergency instructions need not be posted but must be carried aboard the vessel and be available to the crew for familiarization. Emergency instructions not mounted at operating station. 46 CFR 185.510

The EPIRB test required by paragraph (a) shall be logged or otherwise documented. Required monthly EPIRB test not logged. 46 CFR 185.728(c)

An inspection for renewal of a Certificate of Inspection (COI) is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. Vessel's COI expired on 15JUN20 and is overdue for renewal. Complete COI renewal inspection prior to carrying passengers. 46 CFR 176.404

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Placard did not reflect the type of life jackets on board the vessel. 46 CFR 185.516 (a)

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Broadcast instructions are not legible. 46 CFR 184.506

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Vessel's charts are not up-to-date. 46 CFR 184.420 (a)(1)

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. Vessel did not have an adequate garbage management plan posted during inspection. 33 CFR 151.57 (a)

When a marine employer determines that a casualty or incident is, or is likely to become, an SMI, the marine employer must ensure that the following alcohol and drug testing is conducted: (a) Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the SMI.Vessel's alcohol test strips are expired. 46 CFR 4.06-3

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520 (a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: a positive shutoff valve is installed at the hull penetration. Positive shutoff valves for the electric submersible bilge pumps throughout the vessel are unable to close. 46 CFR 182.520 (e)(7)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Reduction gear oil cooler return line is worn and needs to be replaced. 46 CFR 182.100

The master of the following vessels shall prepare a voyage plan: A vessel making an oceans or coastwise voyage. Master of vessel added voyage plan entry to logbook during inspection. 46 CFR 185.503 (a)(1)

Safety Radiotelephony Certificate is expired. 47 CFR 80.903
