The owner or managing operator shall have tests and inspections of the fixed gas fire extinguishing system in accordance with 46 CFR 115.810(b)(2). Observed service Pipping with visible deterioration and rust STBD Eng compartment.

Vessel is required to carry 50% of IBA when operating in a limited coastwise route in warm waters. One of the vessels IBA's failed to pass annual servicing inspection. Vessel is restricted to carry not more than a 198 passengers until the other IBA has been installed for service. Must notify the USCG marine inspection office once the IBA has been installed. 46 CFR 117.200 (c)(ii) Code 50c.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests of the operation of the main propulsion machinery both ahead and astern. The vessel's port ME is currently inoperable due to the port ER fire. The vessel must conduct adequate repairs to the satisfaction of the cognizant OCMI prior to carriage of passengers. 46 CFR 115.804(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall have the vessel ready for inspection of electrical equipment and systems including the inspection of all cable as far as practicable. The fire in port ER has caused damage to multiple electrical equipments and cable insulations. A thorough plan review and repair to the satisfaction of the cognizant OCMI must occur prior to clearance of this deficiency. 46 CFR 115.806(a)

The owner or managing operator shall have tests and inspections of the fixed gas fire extinguishing system in accordance with 46 CFR 115.810(b)(2). The vessel's Port ER does not have means of fixed fire extinguishing. 46 CFR 115.810(b)(2)

Port fwd bilge suction optest was Unsat. Make necessary repairs and conduct full system optest during topside inspection.

Replace corroded coupling on stbd day tank fuel line. 46 CFR 115.804

The owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection of piping, controls, and valves, and the inspection and testing of alarms and ventilation shutdowns, for each fixed gas fire extinguishing system and detecting system to determine that the system is in operating condition. Wasted fixed firefighting system pull cable conduit, pressure switches in stackhouses. Prior to carriage of passengers, provide service report for the satisfactory condition and performance of the system addressing the satisfactory repairs of the conditions listed above. 46 CFR 115.810

Code 11118: An adult life jacket must be provided for each person carried on board a vessel. In addition, a number of child-size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried that is smaller than the lower size limit of the adult life jackets provided. USCG Marine Inspector found 160 adult size and 1 child size life-jackets with flotation foam material inside severely deteriorated, also, some were found with discoloration and damaged retro-reflective material. 46 CFR 117.71 (a) and (b). Code 17C.

Code 02108: Electrical equipment on a vessel must be installed and maintained to: a. provide services necessary for safety under normal and emergency conditions; b. protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; c. minimize accidental personnel contact with energized parts. An electrical outlet was found laying unsecured on the deck, underneath lifejackets in the portside bench number 2, behind operating station, deck 2. 46CFR120.200 (a), (b) & (c). Code 16C.

Code 03106: Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. The following water intrusion conditions were found: 1. Multiple windows in both sides, and both main and upper deck were found not maintaining its weathertight condition, allowing water intrusion into main passenger accommodation areas. 2. The panels covering all-around superstructure in between upper and main deck was found to have a gap, allowing water to enter, and being trapped in between decks, also leaking inside the main deck lower passenger accommodation area. 46CFR1020 Code 17C.

Code 02110: The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. A spray deflector as detailed in DWG 961 12-6F Raft Details is missing in port and starboard sides, and forward, underneath the vessel superstructure that prevents sea water intrusion to the main cabin deck.46 CFR 115.700(d) Code 17C.

Exhaust gas must not leak from the piping or any connections. Inspectors found an exhaust leak coming from the port side generator piping within the void aft of the engine room.

A vessel must comply with the structural design requirements of one of the standards for aluminum hull vessels. Make permanent repairs to the following items identified by marine inspectors: 01 hole port side hull- aft outboard near stern. 02 holes port side approximately 6' to 8' forward of undersized insert. 01 hole located at keel bottom approximately 1' fwd of the port sea-strainer guard. 02 previous permanent repairs located on the port side outboard hull and the outboard starboard hull. These repairs appear to be between 10" x 10" and 12" x 12" but less than the minimum 18" x 18" insert. Remove port sea strainer inlet grating for inspection. Remove temporary epoxy repair on port side engine room deck forward of the main engine approximately 4" x 5" for re-inspection and make permanent repairs as needed.

A space containing diesel machinery must be fitted with adequate means, such as dripproof ventilator, ducts, or louvers, to provide sufficient air for proper operation of main engines and auxiliary engines. Starboard E/R air vent louver damaged.

A vessel must comply with the structural design requirements of one of the standards for aluminum hull vessels. a) Identified fuel leaking from the outside of the inboard port hull integral fuel tank. Remove fuel and provide marine chemist certificate prior to USCG fuel tank inspection. b) Identified discoloration mark on the outside of the inboard starboard hull integral fuel tank. This mark smelled like fuel but no visible fuel leak could be seen. Remove fuel and provide marine chemist certificate prior to USCG fuel tank inspection.

Observed pitting along the bottom plating of both port and starboard day fuel tanks and located two deep pits .5" in diameter where fuel was observed leaking through the plating. Crop and renew bottom plating 6" past the the forward and aft bulkheads of both port and starboard day fuel tanks and approximately 4' upwards from the keel on the inboard and outboard sides of both port and starboard hulls. After fuel tank repairs are completed, conduct satisfactory hydrostatic pressure tests in the presence of USCG marine inspectors IAW 46CFR119.435(b)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel sustained damage to starboard shaft and propeller when departing Ceiba, PR. Length of shaft from propeller to vessel entrance at stern tube is missing. Vessel to remain out of service until dry dock oversight and repairs can be completed by attending marine inspector. Repair proposal and vessel specification of shaft/propeller to be sent to USCG for review. 46 CFR 119.200(a)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. STBD Side engine is not fully operational provided a mechanical report attesting of the operation of the engine to the satisfaction of the attending marine inspector. 46 CFR 119.200 (a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances. - Port side fore peak transversal frame num. 1 had 1/2 inch crack. Repair during drydock inspection.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The vessel experienced a starboard engine failure. Proof of repair and proper operation must be done in the presence of an attending marine inspector prior to normal operation.

Vessels to which this subchapter applies must meet the applicable provision of subchapter S (Subdivision and Stability. Due to the installation of the baggage corral the number of passengers allowed to be carried is reduced form 257 to 249. This CG-835 is to be posted next to the Certificate of Inspection.

Bulkheads and decks must be classed as A-0 as per tables 116.415(b) & Table 116.415 (c). As per the vessel's approved Structural Fire Protection Plan, found missing structural fire protection insulation in the following areas: a. Under bridge electrical cable runs, are open open to the bridge with three (03) openings. b. underside of ladder from the wheelhouse deck to flying bridge deck, the insulation had fallen. c. Port & starboard engine rooms overhead and forward bulkhead. Multiple small areas of insulation had been removed. d. Port & starboard engine rooms electrical cable penetrations were open.

At each inspection, the bilge may be examined to see there is no excess accumulation of debris that might clog the bilge pumping system. Metal shavings and rust was found in port & starboard engine room bilges.

Electrical equipment on a vessel must be installed and maintained to provide services necessary under normal and emergency conditions. The heat detector in the starboard engine room aft, the securing device was wasted and the detector was hanging by its cable.

Replace two missing Type I, USCG approved child size life jackets.

Provide a Senior Deckhand Letter of Designation.

Post a VHF Radio Emergency Broadcasting Placard next to the VHF radio at the operating station.

Vessel must carry an USCG approved first aid kit. 46 CFR 184.710.

Arrangements must be made to the ring buoy water light line to be attached to the life ring with a corrosion resistant clip, and around to the body of two life rings. 46 CFR 180.30-10.

Provide updated publication on board such as: nautical charts, tide & current tables, light list, coast pilot and COLREGS.

Provide the Marine Safety Center all required information of removal of existing AC units and installation of the new ones, in order to update any changes to the stability letter issued by MSC.

Properly secure all electrical wiring and connections in all newly installed AC units.

Provide all required information for company Employee Assistance Program and make it available to all employees.

Provide a DOT approved alcohol testing devices.

Provide additional day shapes: one diamond and one ball. COLREGS

Proof proper operation of 3 battery operated lanterns.

Main deck forward access hatch must be weathertight and replace broken tag. 46 CFR 116.1160 (b)

Replace PFD donning placard to show the existing PDF model currently on board. 46 CFR 122.516.

Post approved egress emergency plan and make it accessible to passengers in all decks. 46 CFR 177.05-3.

Emergency instructions must be updated. 46 CFR 185.25-5.

Stbdside exhaust pipe lagging must be re-installed and secured.

Port & Stbd side engine room access hatches are not airtight; missing segments of rubber gaskets and door tags must operate properly.

Several deck hatches door rubber gaskets are not sealing properly, allowing water ingress to various compartments, specially the stbdside steering room. 46 CFR 116.1160

Portside rudder angle gear is disconnected from steering actuator box. 46 CFR 176.25-35. DUE 07APRIL2021.

Repair sharp edge located on a bench seat at the exterior, back of second deck.

Repair stbdside fire pump as it was found leaking. 46 CFR 176.25-50

Stbdside VHF antenna cable has exposed wiring on deck above the operating station.

Portside engine room 12V distribution panel is missing its cover door.

Bull horn on upper deck has exposed dead end wires and not to be connected.

The following areas have missing or compromised thermal/heat insulation: a. Port & Stbdside engine room access surrounding bulkheads and stairways leading to the second deck; b. underneath stairways leading to the 3rd deck; c. overhead of the existing cold room.

Provide copy of the approved structural fire protection plan. 46 CFR 177.05-3. DUE 07APRIL2021.

Replace border seals around new A/C units and insulation above overhead tiles.

Conduct testing of all heat detectors in presence of an USCG Marine Inspector.

Test emergency fuel shut-off valves in presence of an USCG Marine Inspector. 46 CFR 185.30-20.

Submit electrical load analysis to MSC for new A/C installations.

Stuffing boxes on both port & stbd engine rooms access must be sealed with approved fire retardant sealer (electrical stuffing boxes)

Portside fire pump pressure gauge was found inoperable. 46 CFR 176.25-50.

Ensure all PDF storage locations displays accurate count of PDFs within that storage. 46 CFR 185.30-10.

Provide copy of the approved structural fire protection plan of the engine rooms. 46 CFR 177.05-3. DUE 07APRIL2021.

Re-position EPIRB to a location more accessible to be retrieved in case of emergency and fin a float free position. 46 CFR 177.64.

Log monthly testing of the EPIRB. 46 CFR 185.25-20.

Suitable means such as an automatic damper must be provided for shutting off the passage of air through the ventilation duct in the event of a fire. Found the damper mechanism for the starboard engine room would not completely reset leaving one side closed.

In the presence of a Marine Inspector complete the testing of the Propulsion Control Qualitative Failure Analysis (QFA) and Design Verification and Periodic Test(DVTP) procedures per MSC Plan review letter E2-2100376 dated February 17, 2021. Provide all references listed in the letter prior to conducting the testing.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel was hauled out at North River Shipyard with the intent to conduct repairs without notifying the cognizant OCMI. Vessel must submit proposal for engine repairs prior to conducting work.

The OCMI may require an inspection and testing whenever a repair or alteration is underataken. Vessel must conduct sea trials once repair work is complete.

Repair fire alarm system.

- Replace CO2 fire extinguishers in accommodation spaces with foam or dry chemical extinguishers.

Replace blank on distribution panel, 2nd deck.

Replace 07 child life jackets

Repair structural fire protection in 2nd deck electrical space.

Repair/remove bench stand (tripping hazard), aft second deck.

Repair exposed wiring in void #2, port side.

Re-attach wiring mounts in void #2, starboard side.

Port side ventilation closure for the ER was inoperable. Repair prior to carrying PAX.

Hydrostatic releases on IBAs were not installed properly. Vsl restricted to no more than 1 NM from shore until corrected.

Replace chaffed wiring and secure wire runs for port and starboard spotlights.

Repair port aft void watertight hatch and gasket.

Replace or test port and starboard Halon system discharge hoses.

Secure sewage overboard.

ALL OF THE VESSEL'S INFLATABLE BUOYANT APPARATUS HAVE BEEN REMOVED FOR ANNUAL SERVICING. THE VESSEL IS LIMITED TO A LAKES, BAYS, AND SOUNDS ROUTE NOT MORE THAN ONE NAUTICAL MILE FROM SHORE UNTIL THE IBA'S HAVE BEEN RE-INSTALLED TO THE SATISFACTION OF AN ATTENDING MARINE INSPECTOR.

Install manual fuel shutoff valves for the port and stbd fuel lines near the fwd fuel tanks.

Prove porper operation of propulsion machinery.

There was a hydraulic leak in the stbd steering system. Repair.

Fire pump gauges were inoperable. Repair/replace.

Gasket on stbd fire system sea chest was leaking. Replace.

There was an exhaust leak in the port laz. Repair.

Replace p/s E/R bilge suction hose with approved piping/hoses.

Identified eight (08) cracked welds in the aft compartments. Repair.

The vessel must complete an annual exam prior to operating with passengers.

Replace the raw water cooling bellows on the stbd engine, discharge side.

Repair the port and stbd remote fuel shut off valves.

Replace the EPIRB battery, old battery is expired.

Properly label all emergency escapes.

Provide the vessel with a Bridge to Bridge certificate.

Prior to the carriage of passengers, prove that both PORT and STBD fire pump can produce 60 PSI at the pump dischage. Could only produce 30 psi on the STBD and 40 psi on the PORT.

Prior to the carriage of passengers, remove all soft patches on all vital systems as discussed. Found several areas that the pipe was either repaired or coupled with a rubber hose w/hose clamps. All piping is aluminum.

Prior to the carriage of passengers, repair/replace missing insulation in PORT engine room. A sizable piece of insulation was missing from BLKHD in the PORT engine room.

PRIOR TO CARRIAGE OF PASSENGERS, EFFECT REPAIRS TO THE TRANSOM OF THE STARBOARD HULL AND THE DECK RAIL/SWIM PLATFORM ABOVE THE STARBOARD HULL TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

Vessel lost (1) inflatable life raft on voyage from New York to Florida. Prior to carrying passengers, vessel must provide adequate lifesaving equipment IAW COI.

Firemain in stbd engine room wasted through IWO "T" on overhead. Make proper repairs to the satisfaction of the attending Marine Inspector prior to carrying passengers for hire.

Prove proper operation of stbd ship service generator prior to carrying passengers for hire.

Portside bilge system failed to operate. Prove proper operation of bilge system prior to carrying pasengers for hire.

All Halon to Stbd Engine Room discharged. Fire fighting capability must be restored and certified by competent servicing company. LEGACY COMPONENT DESC: Quantity of Halon

Fire damaged electrical components to be repaired. Thermo couples to stbd MDE turbo/exhaust to be replaced. Plastic covers to light fixtures melted by fire to be replaced.

Turbo charger (Inboard/A-bank) to be repaired along w/ exhaust elbow and heat shield.

Make repairs to damaged cooling system, incl. leaking hose at fwd of engine connecting water pump to engine block.

Remove all oil from engine room bulkheads, bilge, engine exterior, and overhead. Fire and pollution hazard.

Missing 2" cleanout pipe at terminating orifice over both MDE's.

Permanently secure cash register and loose gear under bridge

Identlify and repair leak in bridge spotlight.

REPLACE PORT A/C LOCAL SWITCH BOX COVER.

COMPLETE AS BUILT VERIFICATION FOR NEW INSTALLATION OF DRY EXHAUST SYSTEM, MODIFICATION TO FUEL TANK CUT OFF VALVES, AND MODIFICATION TO E/R BLOWERS.

Pickup welds where burned through in overhead in void #2 on stbd side.
