Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant OCMI. Vessel reported pinhole leak on their secondary fire main. Vessel is to make permanent repairs to restore the piping system to its original strength, to the satisfaction of the attending MI.

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity:(i) The vital systems listed in § 119.710 of this chapter. MI received report the port generator experienced an unexpected shutdown while transiting from a fuel dock to its facility. Serpentine belt failure identified as cause of shutdown. Serpentine belt was replaced and Kitsap ferry services/technician utilized the generator's down time to replace the alternator as well. MI witness operational test of the generator under a full load, all appeared to be satisfactory and the vessel was clear to operate under normal conditions. 46 CFR 120.310(a)(1)

The owner, managing operator, or master shall report damage to a boiler, unfired pressure vessel, or machinery that renders further use of the item unsafe until repairs are made, to the OCMI at the port in which the casualty occurred or nearest the port of first arrival, as soon as practicable after the damage occurs. Vessel master reported vessel's starboard main engine as inoperable. Vessel to provide documentation identifying root cause, rectification, and proper operation prior to departure.

Repairs to the hull must not be made without the approval of the [name withheld] repair proposal to the OCMI.

REQUIRED DRYDOCK/INTERNAL STRUCTURE EXAM DELAYED DUE TO YARD AVAILABILITY AND VESSEL OVERHAUL PLAN. VESSEL SHALL NOT OPERATE WITH PASSENGERS UNTIL COMPLETION OF CREDIT DRYDOCK/ISE.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel representative self reported a loss of oil pressure in the port propulsion engine. 46 CFR 119.200 (a)

Vessel representative self reported a 1/8 inch diameter hole in the vessel's hull in the starboard engine room located approximately one foot above the waterline which is affecting the seaworthiness of the hull. Vessel is to present permanent repair proposal and complete permanent repairs.

Vessel's main engine exhaust bracket in starboard engine room has cracked weld. Exhaust bracket that supports exhaust from the overhead has failed on one side and is being supported from the other side. Vessel is to be hauled out within the next 90 days where arrangements for repair are to be made. Notify issuing marine inspector upon satisfactory repair. 46 CFR 115.830 (a)

Vessel reported that they had a Low Pressure Alarm on the Jacket Water. After initial exam, it was determined that the expansion tank gasket and over fill vent were the source of the coolant spill into the bilge. Vessel is to provide a root cause analysis to the system failure and prove proper operation of the system prior to the carriage of passengers. 46CFR 119.420a3

Oil discharged out of the exhaust cooling water because the reduction gear oil cooler let go. Required to fix and ensure proper operation of the main engine prior to the carriage of passengers. 46 CFR 115.804

uring an internal structural exam carried out on 30SEP2020 several shell stiffener connections to the collision bulkhead were found with through thickness fractures in the port and starboard hulls. Though these failures do not appear to pose an imminent threat to the vessel a close watch must be maintained until temporary repairs can be made. These fractured stiffeners must be inspected each run to ensure this portion of the hull remains stable. Inspections are to be logged. these fractures may be temporarily repaired by v and welding the fracture on both sides. Proper permanent repairs are to be carried out during the vessels dry-dock exam next month.

Provide satisfactory repair for port engine prior to movement of vessel. 46 CFR 119.200 (a)

Vessel's starboard side where the side shell meets the deck, vessel sustained approximately an 8 inch crack. Vessel to make temp repair. Must notify CG of permeant repair plans.. Vessel must make regular rounds of void space to verify damage remains stable. 46CFR115.700

Make permanent repairs to port side-shell located amidship, approximately four (04) feet above of the waterline. Ensure that the fit-up, back gouge, and final welds are available for inspection by a USCG Marine Inspector.

Vessel must have nautical publications and charts for new operation area.

Vessel operations and crew training shall be conducted in accordance with the approved high speed operations manual and training program. Manual dated 09NOV2006 is not approved for operations in another OCMI zone with a different operator.

Conduct a new to zone inspection, to include man overboard and fire drills.

Required vessel to cut out CVK and hull plate up to third longitudinal on each side of the CVK throughout the entire length of the air conditioning space on the port side (approximately 40'x6" insert).

Repair port aft corner crack.

Replace bottom plate in sewage void.

Remove oil and clean bilges in port and starboard engine rooms.

Repair holes topside in starboard void #3.

Repair frame cracks in starboard void #2

Repair crack in wet deck sideshell of void #4.

Repair watertight bulkhead in starboard void #4.

Repair crack in port generator exhaust at shell plate.

Audio gauge transom plate port side engine room.

Audio gauge port side shell plate and center vertical keel shell plate in void #4.

Replace plate on aft watertight bulkhead with pinholes in port void #4.

Open engine room wet deck inspection ports on both sides of vessel.

Service all intake valves.

Clean sewage residue from bilges.

Determine original thickness of transom.

Repair sewage leak.

Vessel is in layup status. Prior to operation with passengers th vessel must undergo reinspection to ensure all lifesaving in place, conduct drills, be manned, and demonstrate operation iaw high speed op manual.

Port fore peak at the third frame, (aft leading forward) has a cracked weld between the 4th and 5th longitudinal stringer on the inboard side.

EPIRB was not being tested and logged monthly.

Main engines and reduction gears have two nonmetallic expansion joints each for a total of four (04) per engineroom. All eight (08) are showing signs of wear. Renew all eight (08) and document.

Port engineroom aft side and forward side of main engine exhaust muffler lagging is missing and/or not properly made up. Renew or repair lagging.

Port engineroom aft side of main engine exhaust muffler has signs of a raw water leak, clean area and make ready for operational underway test.

Port side engineroom mid compartment inboard side 120v outlet is broken and has exposed metalic contacts. Renew outlet.

Port side engineroom aft fuel oil remote shut off valve does not close completely due to interference with racor filter drip pan. Prove proper operation.

Port engineroom communication cable to the reduction gear outside sheathing is cut with exposed metal. Renew cable.

In vicinity of rubrail from foward blkhd aft in #4 compartment starboard side. Aluminum around through bolts wasted.

Four inch inset along Port side in #3 compartment port side. Crop out and renew.

Port side bow crop and renew damage and starboard foward quarter cracked weld grind and renew.

Two inch gate valve #3 compartment stbd and four inch lower gate valve in engine room will not fully cycle. (butterfly valves)

#4 Void port inboard brass pipe (water for bar) has break in line w/rubber hose.

Port engine room replace two inboard sea valves (2 inch and 4 inch).

Clean and remove oil from both eingine room bilges.

Rubrail on port side rub rail ivo #2 compartment cracked. Grind out and repair cracks.

Pitting on out board second frame #3 compartment.

Repair port engine room visual alarm indicator.

Repair port generator overspeed sensor.

Repair port generator oil pressure gauge.

Complete satisfactory underway sea trials to prove proper operation of the Port engine due to failure.

Steering system is in a continuous alarm state, troubleshooting results have been inconclusive stated by vessel rep. Manual indicates that the reported condition is a summary alarm, requiring further investigating by a qualified tecnichnician. Manual also indicated that if another fault condition occurs the steering system will fail. Prior to sailing, repair and test to the satisfaction of attendi

The back-up steering system on the port side is not operating as designed. When using the joystick the captain must take extra caution to not move handle as designed, otherwise the port steering system will trip off line. Identify problem, repair and test to the satisfaction of CG Inspector prior to sailing.

The montoring system has two (2) communication net work channels/bus, one of the network channels is inoperable. The monitoring system indicates all engine, cltuch, and steering alarm statuses, if the second channel fails there will be no reported conditions of the plant. Repair and test to the satisfaction of class.

STBD side bilge pump failed to get suction, prior to sailing test to the satisfaction of CG Inspector.

Vessel has microprocessor propulsion control systems 46 CFR 121.620(d) ; provide PSTP in accordance with 46 CFR 61.40.-6 & 61.40-10.

Properly terminate loose exposed wiring IWO port engine control box.

Re-stencil Ring buoy on the fantail with the vessel name and homeport.

Prior to sailing with passengers, submit repair report for vessel's port engine turbocharger to the satisfaction of the attending coast guard marine inspector.

Valve on port jet hydraulic block had o-ring failure causing loss of hydraulic oil and pressure.

HSC Operator's Manual is not approved as per NVIC 05-01. Complete approval of working plan and provide copy onboard vessel.

Determine reason for the CO2 discharge and provide report to the U.S. Coast Guard. CO2 discharged while vessel was underway with 123 passengers on board.

Upon discharge of CO2 all ventilation shut down resulting in the engine to overheat and the exhaust manifold gasket to fail. Verify coolant leak is from exhaust manifold and make proper repairs.

Provide legible survival craft placard.

Install hydro-static relaease on 50 man raft with a valid expiration date.

Replace inop bulbs on the main deck emergency lighting.

Prove proper operation of bilge system for stbd side fwd peak. Until repairs have been made vsl to use electric submersible.

Make proper repair to stbd forepeak bolt hole.

C/R damage IWO stbd bow stem.

Remove all seachest gratings and make all seachests available for inspection.

Make the following valves ready for inspection: port and stbd 2" generator, firepump 2 1/2" and lower seachest 4".

Clean and inspect welds inboard stbd sponson as marked.

Replace stbd lifering upper deck.

Cap 2" sewage discharge pipe.

Replace EPIRB battery.
