"Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors." All four engine block heaters must be removed from vessel due to electrical shock hazard, poor workmanship and installation. MI was presented with a shock hazard while inspecting engine shock hazard.

MI's observed a cracked weld in the starboard #2 void above the waterline. Vessel needs repair and perform NDT in the presence of an MI.

"Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations." MI's observed water leaking around hull connection of the inboard starboard jet drive. epair and conduct functional test in presence of MI's

"A duct must be of rigid permanent construction, which does not allow any appreciable vapor flow except through normal openings, and made of the same material as the hull or of noncombustible material. The duct must lead as directly as possible from its intake opening to its terminus and be securely fastened and supported." MI's observed gasket material around opening leading to port engine compartment in poor condition and partially removed from closure to compartment. Restore to like new condition.

"Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The marking is not required on a life jacket carried to meet a temporary need for additional life jackets, if the life jacket has the name of another vessel or company marked on it. For an immersion suit, the name of the person to whom the immersion suit is assigned is an acceptable alternative to the name of the vessel." MI's observed starboard forward and port aft life rings missing letters for name of vessel.

MI's observed forward starboard ventilation damper not fully closing after conducting test of CO2 shutdown. Vessel's crew rectified deficiency while inspection was being conducted.

"At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. (b) At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes." MI's observed mold in overhead and ceiling tiles, oil in both engine compartment bilges and soaked into insulation. Must clean passenger spaces ,both engine rooms, lazarette and remove all debris, insulation, oil, hydraulic fluid from all surfaces.

Install placarding on E/R hatches to keep closed during operation.

Remove all plastic piping from bilge system.

Forward door in cabin area does not open. Fix.

Port Laz -Finish stitch welds. -Straighten bent longitudinal and weld. -Weld pipe bracket into place.

Port aft top deck -Multiple cracked stanchions. Repair. -Broken deck. Crop and renew. Fitup authorized.

Vessel did not have fire resistant sight glasses.

A fixed gas fire extinguishing system aboard a vessel must be approved by the Commandant and meet the system requirements of 46CFR181.410. Attending Marine Inspector (MI) found the port side engine room pressure switch inoperable and the starboard side engine room system required maintenance to operate. Vessel is not operating in compliance with the COI. 46 CFR 181.410(a) 46 CFR 176.100(b)

Each ring life buoy on a vessel must be in compliance with 46CFR160.050. Attending MI determined the grab lines on the after life ring buoy to be deteriorated. 46CFR180.70(b)(1)

Each hatch exposed to the weather must be watertight. Attending MI found the both lazarette hatch covers not watertight (bad gasket and seized dogging mechanism). 46CFR179.360(a)

A vessel is inspected for compliance with the standards required by this subchapter; arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Attending MI found there was insulation missing in the port engine room, aft and inboard which was part of the original installation and arrangement of the space. 46CFR176.800(a)

Fixed gas fire extinguishing systems shall be inspected and tested in accordance with 46CFR147.60, 147.65 and 176.810(b). Attending MI found the fixed gas fire fighting system alarm horns excessively deteriorated and evidenced a of lack of maintenance. 46CFR176.810(b)(2)

This vessel is a "T-boat", therefore not required a safety management system (SMS) per regulation. Sector NY has found the following deficiencies to be objective evidence that a serious substandard condition has not been pro-actively managed by the vessel's owner/operator, leading to a lack of safety culture onboard the vessel. 46CFR176.830

Provide technician report for fixed fire extinguishing servicing.

Observed corroded pad eyes on main deck compromising watertight integrity of voids underneath the main deck. Replace corroded pad eyes to maintain watertight integrity of main deck and spaces below.

Observed multiple areas on port and starboard sides where rub rails are missing. Replace all missing rub rails around the vessel.

Replace missing starboard aft cleat.

Repair leak in starboard shaft packing.

Replace gaskets on bridge doors.

Remove dead end wires located inside the bridge cabinet, starboard aft.

Reposition and secure loose hanging electrical wire in the bilge located in starboard lazarette.

Label life jacket stowage location with type and number of life jackets (port side main cabin, leading into engine room).

Provide proof LED navigational lights meet UL 1104.

Replace 2 corroded fire extinguisher brackets located at the port and starboard aft deck.

Each vessel that relies on electricity to power vital systems, interior lighting, communication systems, and vanigations systems must be arranged so that the loads can be energized from two sources of electricity. Vessel's #1 generator completely shut down and the vessels #2 generator was reading power loss errors with no working guages. Vessel must prove two sources of power for the systems listed above to an attending MI prior to carrying passengers.

Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passenger and crew. Equivalent protection may include, lifelines, wire rope, chains, and bulwarks, which provide strength and support equivalent to fixed rails. Bulwarks on the port quarter, passenger upper deck, are heavily damaged. Replace or make proper repairs to identified bulwarks.

Following a grounding, vessel must be assessed for a return to commercial service. The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel must be attended by CG Marine Inspector and deemed fit for service intended.

Stern light wiring is exposed to weather

wiring located above port ansd starboard engines are supported using zipties.

Wiring located above generator in engine room is covered with rubber and clamps.

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel is outside scope of annual inspection window and shall complete required annual inspection prior to carrying passengers.

Upon testing the fixed CO2 system, the automatic shutdowns of ventiliation and engines were inoperable.
