Each vessel without overnight accommodations certificated to operate on a coastwise route, must:(1) ... be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 67% of the total number of persons permitted on board. MI's observed IBA removed from vessel for maintenance. 46CFR117.204(c)

Vessels to which this subchapter applies must meet the applicable provisions in Subchapter S of this chapter. Prior to the carriag eof pax, provide updated stability letter to local OCMI. 46 CFR 116.100

THE GENERAL CONSTRUCTION OF THE VESSEL MUST BE SUCH AS TO MINIMIZE FIRE HAZARDS INSOFAR AS IT IS REASONABLE AND PRACTICABLE. FURNACE PIPING LEADING TO EXTERIOR VESSEL HAS HOLES IN ELBOW. REMOVE WRAPPED LAGGING FOR FURTHER INSPECTION, SEND PICTURES OF CONDITION OF PIPING, AND MAKE NECESSARY REPAIRS AS DETERMINED BY ATTENDING MARINE INSPECTOR. FURNACE MAY NOT BE USED UNTIL THIS OFFICE RECEIVES PICTURES OF THIS ENTIRE PIPE, AND A REPAIR PROPOSAL FOR ANY NECESSARY REPAIRS. *RECEIVED PICTURES OF EXHAUST PIPING WITH LAGGING REMOVED. SHOWS SIGNS OF CORROSION/PITTING. PIPING MUST BE REPLACED PRIOR TO NEXT CREDIT DDX. UNTIL THEN, THE FURNACE MUST BE TAGGED OUT. 46 CFR 177.405(a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without approval of the cognizant OCMI. Discovered patch on port side outboard jet drive located in lazarrete. Remove patch, investigate and provide repair proposal for approval from OCMI.

At each inspection for certification of a vessel the owner/operator shall have vessel ready for inspection of the hull structure. #2 starboard void from keel, one frame up had multiple pits that met the 25% threshold. Clean and preserve all pitted areas. Vessel will crop and renew first frame from keel both port and starboard sides fwd to aft at upcoming dry dock.

At each inspection for certification all observed unsafe practices, fire hazards must be corrected. Port side engine room had smoke caused by exhaust leak or oil soaked lagging. Investigate and make permanent repairs to eliminate smoke.

Each ungrounded system must be provided with ground detection system that provides continuous indication of circuit status to ground. 1 amp ground was observed on ships power. Investigate cause and isolate ground.

Piping used in a vital system must be composed of nonferrous material. Vessels bilge suction line had rigid plastic installed. Remove and replace with proper material. 12MAR2020: Suctions replaced with metal fittings; starboard tank room suction not in place, in order to allow room for welding/moving the fuel tank.

On a vessel of at least 26ft in length bilge level alarms are required. Bilge alarm in in starboard lazarette sticks in the up position. Repair bilge alarm.

Each connection to a conductor or terminal must be made within an enclosure. Starboard bilge pump and alarm wiring were not in an enclosure. Place connected wires in watertight enclosure.

The use of plastic tie wraps must be limited to to bundling or retention of multiple cable installations. Zip ties were used to hold both port and starboard J boxes in Port lazarette. Remove zip ties and permanently install j boxes.

Provide up to date crew list to reflect requirements for DAPI program. Vessel's captain was not on list.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel's stern light was inoperable. Repair stern light.

Machinery, equipment, materials, and arrangements not covered by standards may be inspected in accordance with standards acceptable to [name withheld] down straps were deteriorated. Replace or remove tie down straps.

Operation of transmitter if any ship station must be performed by a person holding a commercial radio operator license or permit of the class required. Vessel's Captain did not have FCC Marine Radio Operator Permit.

Approximatley 4 inches of water with hydraulic fluid in port and stardboard lazerettes due to no properly securing hatch scuttles. 46 CFR 176.800(a).

Engine shall be capable of maintianing constant marine load. Forward port MDE has oil leak in forward part of engine near timing gear. 46 CFR 182.182.(200)(a)

Vessel shall be operated in good marine practice. Several hydraulic soaked rags were discovewred on the wet deck tunnel between lazerette spaces. 46 CFR 176.800(a)

A hatch exposed to the weather shall be watertight. Both lazerette hatches were found not dogged down correctly leading to water intrusion. Provide training to crew on hatch operation and securing.

Punctured hold and bent frame in starboard generator void. Uppermost strake all the way forward on the starboard outboard side. C/R 18x18 and replace bent stiffener. Tie insert into existing welds, utilize radius corners and relief cuts as marked.

Damage IVO starboard forward cleat IWO rub rail. Repair will be as close to 18x18 as practicable. Weld will have 2 radius corners and 2 relief cuts where plate ties into existing weld.

A pre-engineered fixed firefighting system must be approved by the Commandant and be installed in accordance with the manufacturer's instructions. Attending Marine Inspectors found the pre-engineered fixed firefighting system for the boiler had not been serviced since 2017 and was past due. Not in compliance with COI. 46CFR181.420 46 CFR 176.100(b)

A pre-engineered fixed firefighting system must be approved by the Commandant and be installed in accordance with the manufacturer's instructions. Attending Marine Inspectors found both pre-engineered fixed firefighting systems were installed on generator enclosures that were not sealed as designed. Not in compliance with COI. 46CFR181.420 46 CFR 176.100(b)

On a vessel of at least 26', bilge high level alarms shall be provided in a machinery space bilge that is subject to flooding. Attending Marine Inspectors discovered the bilge high level alarms in both of the engine rooms were not mounted and would not provide the required audible and visual alarms at the operating station if water intrusion occurred. Not in compliance with COI. 46CFR182.530(a)(2) 46 CFR 176.100(b)

A hatch exposed to the weather must be watertight. Attending Marine Inspectors discovered that starboard engine room, port lazarette and starboard lazarette scuttles were not able to be dogged down. *7 days to rectify* 46CFR179.360(a)

Piping that carries vapor, gases or liquids with temperatures exceeding 150 degrees F must be suitably insulated to prevent injuries. Attending Marine Inspectors found the insulation on the exhaust was falling off and exposing hot surfaces. *7 days to rectify* 46CFR177.970

Each ring life buoy on a vessel must be approved in accordance with 46CFR160.050. Attending Marine Inspectors determined the grab lines on the ring buoy were deteriorated. *7 days to rectify* 46CFR180.70(b)(1)

Each connection to a conductor or terminal part must be made within an enclosure. Attending Marine Inspectors discovered starboard lazarette bilge pump and alarm wire connections were not made in an enclosure. *7 days to rectify* 46CFR183.340(g)

The vessel YORK is a "T-boat", therefore not required a safety management system (SMS) per regulation. Sector NY has found the following deficiencies to be objective evidence that a serious substandard condition has not been pro-actively managed by the vessel's owner/operator, leading to a lack of safety culture onboard the vessel. 46CFR176.25-50

Found hole in corner of port transom/lazaratte. Crop and renew identified area and repair to satisfaction of attending MI.

Fire pump tested and only putting out 25 PSI at the pump outlet. Prove fire pump discharges water at 60 PSI.

01- Identify root cause and make all repairs necessary to restore proper operation to stbd outboard engine and jet propulsion unit. Prior to carrying passengers.

Crop and renew damaged rub rail / deck edge and port bow area identified damaged as a result to collision with concrete pier to the satisfaction of the attending marine inspector. Due 04 Jul 2016
