Vsl had on board inflatable buoyant apparatus that hasn't been serviced. Vsl route restricted to within one mile from land. 46 CFR 117.205(a)

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. 46 CFR 119.455(b)(3)

Exercises must be conducted at least once each calendar year, with no more than 18 months between exercises. Exercised wasn't conducted in the last 18 months. 46 CFR 104.230(c)(1)

"To maintain a valid Certificate of Compliance, you must complete your annual inspection within the periods specified in 176.500(b)(1) and your Certificate of Compliance must be endorsed." Vessel lapsed its Annual Inspection date. Must complete an Annual Inspection.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Port generator raw water exhaust cooling pipe was removed at time of inspection for repairs.

Each vessel that is 400 Gross Tons or more as measured under the convention measurement system must have a Nontank Vessel Response Plan. Vsl not in compliance.

Provide repair plan for stbd generator waster coolant line.

Vessel replaced structural fire protection in overhead above starboard fuel tank.

Vessel liferafts required annual servicing where the annual servicing of the liferafts was to be conducted on 30SEP2020. Vessel has a 406 MHz EPIRB which allows them on the vessel on its certificated route to operate without primary lifesaving until vessel is to operate in cold water. Cold water dates take effect in SECNY AOR on 01NOV2020. Vessel has until 31OCT2020 to service and install liferafts aboard vessel. Vessel operator was educated that all lifesaving installed on board the vessel shall be maintained in accordance with COI. CG-835v issued to vessel denoting that the operating time frame for the vessel.

Vessel bilge pump in no. 2 void from bow on starboard side had hairline crack at coupling between pipe flange and dischage pipe. Bilge pump originally installed however produced adequate pressure to dewater in case of emergency.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment under all practicable conditions. Bilge pumping sytem in port forepeak was inoperable.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel and stowed in a position that is readily accessible to crew members for launching. Attending Maine Inspector (MI) discovered both inflatable buoyant apparatus had the hydrostatic release installed incorrectly and with mounting bars still in place despite placard on mount clearly directing removal after installation. Vessel is not operating in compliance with the COI. 46 CFR 117.130 46 CFR 115.100(b)

This vessel is a "K-boat", therefore not required a safety management system (SMS) per regulation. Sector NY has found the following deficiencies to be objective evidence that a serious substandard condition has not been pro-actively managed by the vessel's owner/operator, leading to a lack of safety culture onboard the vessel. 46CFR115.830

The owner or managing operator shall instruct each crew member upon first being employed and prior to getting u/w for the first time on a particular vessel and at least once every 3 months as to the duties that the crew member is expected to perform in an emergency including but not limited to those listed in 46CFR122.510. Attending MI found that the owner/operator was not able to provide proof of which vessels individuals have been trained on and that the company was not conducting training every 3 months. Vessel not operating in compliance with the COI. *Submit plan by 0800 22NOV2019 on how company will guarantee only qualified individuals are sailing onboard each vessel platform while generating qual matrix* 46 CFR 122.420 46 CFR 115.100(b)

46 CFR 115.802 At each initial and subsequent inspection for certification, the vessel shall be ready for inspections of the hull structure and its appurtenances, including the inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Observed cracked welds on tonnage bulkhead in port side lazerette.

46 CFR 115.830(b) At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems or block emergency escapes. Excess water and oil in bilge of port side engine room must be removed.

46 CFR 120.220 Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway and must be suitable for the voltage and current utilized. Multiple equipment cords are plugged into a power strip from underneath bridge navigation console and could be a fire potential.

46 CFR 121.210 Cooking and heating equipment must be suitable for marine use. Each heater element of an electric space heater must be of an enclosed type. The element case or jacket must be made of a corrosion resistant material and must be provided with a thermal cutout to prevent overheating. Remove portable heaters from port and stbd lazerette when not needed.

46 CFR 115.830 (a) At each inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Must remove combustible supports and provide permanent metal supports for tubing in port and stbd lazerette.

46 CFR 118.310(e) Spanner wrenches must be provided for each fire hydrant required by this regulation. Remove all gear adrift missing spanner wrenches at fire stations 2 and 4

46 CFR 56.60.1 Pipe hangers must be of an acceptable metallic material in accoradance with accommodation spaces and service spaces must be of noncombustible material Insulation in port lazerette needs to be repaired.

46 CFR 117.70(b) Each ring life buoy on a vessel must: Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Life buoy on stern is missing retroreflective tape.

46 CFR § 120.200 Electrical equipment on a vessel must be installed and mainted to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The battery terminals in the port engine room need to be cleaned.

46 CFR 120.220 Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Light switch cover in stbd lazerette is loose and needs to be tightened.

46 CFR § 120.200 Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Light fixture in stbd engine room needs to be installed correctly without the support of plastic zip ties.

46 CFR 117.68(b) A vessel on lakes, bays and sounds or rivers route must carry three red hand flares distress signals and three hand orange smoke distress signals. All required navigational flares are expired and need to be replaced

Renew structural fire protection in both engine rooms. Worklist Item will become deficiency when vessel departs drydock. Due Date: October 11, 2019

Replace all missing structural fire protection insulation in all spaces as discussed. 46CFR116.415

Replace victaulic fittings on port hull fuel fill and vents lines.

Complete satisfactory internal structural exam prior to carrying passengers.
