Vessel plans on installing a roller furler for their jib sail in the next three to four months. Once furler is installed, vessel can not use the jib with passengers on board, until the jib furler and associated rigging is inspected and operationally tested in the presence of the attending CG marine inspector. 46 CFR 176.700(a)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel must undergo sea trials after the mast and rigging is installed, tuned, and a satisfactory letter of attestation is received from a certificated rigger, to the satisfaction of the attending CG marine inspector. 46 CFR 176.840

No major repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge and approval of the Officer in Charge, Marine Inspection. The vessel owner is requesting to make a change to the vessel's forward cross beam. The modification includes changing the angle of the beam from aft-facing, to facing straight up, along with replacing the installed chain plates. Submit alteration proposal to the attending marine inspector and modify identified areas in accordance with approved verbal repair proposal. 46 CFR 176.20-1(a) Old T

Except as otherwise provided in this section, rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crews. The vessel's forward bow from port to starboard does not have deck rails or approved equivalent protection. Submit a written policy addressing how the vessel will implement physical or operational measures to a level that is considered an equivalent protection to railings to be approved by the OCMI. 46 CFR 177.35-1(a) "Old T"

Each lifeline on a ring life buoy must have a breaking strength of at least 1,124 pounds. The lifeline on the ring life buoy is deteriorating. Replaced with the approved lifeline to the satisfaction of the attending marine inspector. 46 CFR 180.70(c)(5)

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring life buoy. The waterlight lanyard is deteriorating. Replaced with the approved lanyard to the satisfaction of the attending marine inspector. 46 CFR 180.70(d)(2)

The Emergency Position Indicating Radiobeacons (EPIRB) must installed to automatically float free and activate. The EPIRB is stored in a casing that is non-float-free. Reinstall the EPIRB to automatically float free to the satisfaction of the attending marine inspector. 46 CFR 180.64

An Employee Assistance Program (EAP) training program must be conducted for the employer's crewmembers and supervisory personnel. Documentation of EAP training is missing. Provide a written record of EAP training to the satisfaction of the attending marine inspector. 46 CFR 16.401(b)

An adult life jacket must be provided for each person carried on board a vessel. Three life jackets were in unserviceable condition. 46 CFR 180.71(a)

A vessel must comply with the structural design requirements of one of the standards listed. In the starboard hull amidships, 1 side longitudinal has fractured off the hull, and 1 side longitudinal has delaminated plywood. Submit a repair proposal to the satisfaction of Marine Inspector. 46 CFR 177.300

Fuel tanks shall be adequately supported and braced to prevent movement. The supports and braces shall be insulated from contact with the tank surfaces with a non-abrasive and non-absorbent material. Gasoline fuel tanks on deck are missing straps to secure to deck. 46 CFR 182.15-25 OLD T

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Stbd bilge high water level alarm light is not operational. One additional crew member required to act as roving watch. Consequently, total passenger count is reduced by one to account for additional crew, until deficiency is rectified to the satisfaction of the attending marine inspector. 46 CFR 182.530

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Repair the fractured weld where the stanchion is welded to the mounting pad that fasten the deck rail to the vessels structure on the stbd second handrail stanchion from the bow within seven days (7) to the satisfaction of the attending MI. 46 CFR 176.830(a)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Provide an approved First aid kit to the satisfaction of the attending MI. 46 CFR 184.710

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables. Missing Coast Guard Light List. Provide Coast Guard Light List to the satisfaction of the attending MI. 46 CFR 184.420

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Missing the above requirement. In lieu of the valve direction, substitute the following: "TO STOP FUEL SUPPLY DISCONNECT FUEL HOSE FROM TANK." 46 CFR 185.608

Placards containing instructions for the donning and use of life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel has three types of life jackets, but only one placard type. Post additional placard for the other two types of life jackets. 46 CFR 185.516(a)

Where flexibility is necessary or desired, a reasonable length of flexible hose may be used provided it is fitted with suitable connection fittings and has high resistance to saltwater, petroleum oils and vibration. The port portable fuel tank hose is not labeled to determine if it meets the regulation. Replace with approved gasoline fuel hose that meets the J1527 standard. 46 CFR 182.15-40(a)(2) Old T

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel PMP inventory of rigging components replacement intervals was not updated when rigging was renewed at the June 2020 dry dock. Provide third party rigging report from the June 2020, dry dock and an updated inventory of rigging components. 46 CFR 176.840

An EAP training program must be conducted for the employer's crew members and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crew members and the employer's supervisory personnel. Crew members and supervisory personnel did not receive the above required training. Provide a training roster of attendance signed by crew members and DER to attest the training was provided to the satisfaction of the attending MI. 46 CFR 16.401(b)

A hatch exposed to the weather must be watertight, except that the following hatches may be weather tight: (1) A hatch on a water tight trunk that extends at least 305 millimeters (12 inches) above the weather deck; (2) A hatch in a cabin top; and (3) A hatch on a vessel that operates only on protected waters. Vessel's operates on exposed waters and the two forward hull hatches are not watertight. Make watertight and prove they are watertight in the presence of the attending MI. 46 CFR 171.124(a) Old T

Non-subdivided vessels operating on a limited coastwise route, in warm water, less than 3 NM from shore must be equipped with life floats with total capacity of 50 percent of the total persons the vessel is certificated to carry or a float free, Cat 1 406 MHz EPIRB. The vessel is not outfitted with life floats and the provided EPIRB battery is expired. Until life floats or EPIRB is provided, the vessel is restricted to operate in warm waters within 1 miles from shore on a Lakes, Bays and Sounds Route. 46 CFR 180.205(e)

Vessels operating on a limited coastwise route must carry a minimum of 12 distress signals, 6 of which must be hand red flare approved with 160.021. The vessel was only equipped with 4 hand red flares. Vessel is restricted to a lakes, bays, and sounds route until 2 additional hand red flares are provided. 46 CFR 180.68(a)

One type 1 adult life jacket must be provided for each person carried on board a vessel. The vessel is certificated to carry 22 persons, 19 of which may be passengers. Only 20 life jackets were provided. Vessel is limited to 17 passengers until two additional life jackets are provided. 46 CFR 180.71(a)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Vessel has area of delaminated fiberglass on the stbd hull. Grind and scarf 12:1 and re-laminate in accordance with NVIC 8-87. 46 CFR 177.300 (c)

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Vessel railings were removed. Reinstall for attending marine inspector. 46 CFR 177.900 (b)

Sail rigging and components are to be maintained in accordance with good marine practice. Observed boom held in place by bolt that is larger than the width of the connection points and reef horns were bent. Replace hardware to the satisfaction of attending marine inspector. 46 CFR 176.800

When required by the marine inspector, a portion of the inspection must be conducted while the vessel is underway. Conduct sea trial prior to credit drydock. 46 CFR 176.802 (c)

In the application of inspection standards due consideration must be given to the hazards involved in the operation permitted by a vessel's Certificate of Inspection. Nylon rope holding trampoline net is severely frayed in several sections. Replace rope prior to carry passengers.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel's life ring buoy grab line deteriorated and in need of replacement.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing LAT/LONG for DSC operation. 47 CFR 80.905

Due to supply chain delays, bow beam installation delayed 1 month and vessel may continue to operate on existing bow beam. 46 CFR 176.700 (a)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: vessels constructed of wood must, in addition to paragraph (a), provide bilge level alarms in all watertight compartments except small buoyancy chambers; Stbd high level audible alarm at operating station is in operable. Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction; entire weld for the base of the fwd port stanchion was found crack; repair weld.

The constriction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of the COI giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in closed spaces, and necessary facilities for passengers and crew; Laminate frames in fwd port side hatch was for cracked. repair frames

Each certificated small passenger vessels that operates in Sector Honolulu's inspection zone, that has a reduced manning endorsement connected to in-water activities, shall prove to the satisfaction of an attending Marine Inspector, that it meets or exceeds the provisions outlined in Sector Honolulu's Work Instruction 31. Provide required documentation and equipment listed in Work Instruction 31. Vessel must maintain the required manning levels specified on COI of one (1) master and one (1) deckhand, on board the vessel at all times when passengers have access to the vessel.

Deck rails on a vessel engaged in excursion trips,including but not limited to sightseeing trips, dinner and party cruises, must have the following installed between the top rail and deck: chain link fence or wire mesh that has openings not more the four inches in diameter, or bars, slats, rail courses or equivalent spaced at intervals not more than four inches. Vessel has rail courses spaced at 12 inches. Install chain link fence, mesh or courses to the satisfaction of the attending Marine Inspector. Submit plans to OCMI for approval prior to modifying vessel.

A vessel must comply with applicable requirements for any radio installations as set forth in 47CFR80. VHF-DSC shall be programmed with MMSI and integrated with GPS.

Code 01305: The operator could only provide one month, September 2017, as evidence of conducting a rigging inspection aloft to the marine inspector. PMP records presented to inspector indicate the operator is visually inspecting the standing and running rigging at deck level monthly. Inspector found sheered fasteners on the starboard cap shroud chain plate, elongated linkage at the longeron to bow beam connection, damaged retaining pins at the longeron to bow beam connection, fractured reefing hook at the gooseneck, and elongated linkage at the port and starboard bow beam connection to hull. Operator shall provide 3rd party rigging surveyor report, to include rigging inspection aloft, prior to carrying passengers. Operator shall follow a preventative maintenance plan to adequately assess the safe condition of the sail rigging system. Provide evidence of a preventative maintenance plan regime to USCG OCMI in 30 days. 46 CFR 176.800(a) and 46 CFR 176.840

Code 02199: Starboard capshroud chain plate fasteners found corroded and sheered off at the fastener bolt head when operator attempted to remove fastener. PMP states fasteners should be checked for "creep" corrosion. Licensed master provided one years worth of PMP stating that fasteners were visually inspected for corrosion with no notation of the corrosion visually observed. Remove all fasteners on the starboard chain plate for inspection. Remove A-frame fasteners at the bow beam for inspection. 46 CFR 176.802(a)(3)

Code 02199: Bow beam found fractured at weld joint for head stay linkage. Licensed master visually inspected fracture along with the vessel owner and did not indicate issues on the PMP or to Coast Guard inspector. Fractured bow beam and loss of head stay will lead to dismasting. Submit repair proposal to OCMI for approval prior to conducting repairs. 46 CFR 176.802(a)(3) and 46 CFR 176.700

Code 99101: Reefing hook found fractured in two areas. Vessel is restricted from operating on routes with strong winds in which the master deems a reef in the sail is needed or from reducing sail area by adding a reef in the sail. 46 CFR 176.840

Port and Starboard side railings are loose. Submit repair proposal for cracked base of railing stanchions. 46CFR177.900
