In meeting these requirements, the company should ensure that: inspections are held at appropriate intervals; any non-conformity is reported, with it’s possible cause, if known; appropriate corrective action is taken; and records of these activities are maintained. Objective evidence discovered during an expanded ISM exam revealed that the vessel failed to properly inspect and maintain critical fire safety appliances including fixed fire detection, fire doors, and fire extinguishers. Additionally, these long-standing non-conformities have not been properly reported or addressed in accordance with the vessel’s safety management system. An external third party audit is required. 74SOLAS(2) IX/3.1; ISM Code 10.2

Any required fixed fire detection and fire alarm system with manually operated call points shall be capable of immediate operation at all times. PSCO observed four fire detectors in the engine room with rags secured around them and one that was dismantled rendering them unavailable for immediate operation. Four additional detectors have been inoperable since October 2022. 74SOLAS(97) II-2/13.1.1

The fire resistance of doors, shall, as far as practicable, be equivalent to that of the division in which they are fitted. PSCO observed numerous fire doors throughout engine room and accommodation space with holes, cracks, and missing closure devices.74 SOLAS (97) II-2/47.1

Fire extinguishing appliances shall be kept in good order and be available for immediate use at all times. PSCO observed multiple extinguishers throughout the engine room and accommodation spaces with worn hoses becoming detached from the extinguisher. 74SOLAS(97)II-2/20.1

The purpose of this chapter is to require the fullest practicable degree of fire protection, fire detection and fire extinction in ships. PSCO observed oil soaked rags stuffed behind high pressure fuel pumps on main engine. 74SOLAS(97)II-2/2.1

All valves and shell fittings required by the regulation shall be of steel, bronze or other suitable material. Valves of ordinary cast iron or similar material are not acceptable. All pipes to which types of regulation refers shall be of steel or other equivalent material to the satisfaction of the administration. PSCO observed soft patching on pipes inboard of valves. SOV-2 and ADV-8 in the engine room.

A ship must have a placard of at least 5 by 8 inches, made of a durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station, stating the following: Discharge of oil Prohibited. The Federal Water Pollution Control Act prohibits the discharg eof oil into or upon the navigable waters of the United States (...). Vessel did not have any placard regarding the discharge of oil being prohibited.

74 SOLAS (14) 1/11(c): M/V ICE GRACE suffered a loss of propulsion on the main diesel engine on 01MAR16. Recommend class attend and submit report outlining cause of the loss of propulsion and corrective measures to be taken for repair.

The condition of the ship and its equipment shall be maintained to ensure that the ship in all respects will remain fit to proceed to sea. The vessel suffered a loss of propulsion when attempting to shift from ahead propulsion to astern propulsion. 74 SOLAS (14 cons.) I/11; Prior to Movement

0830 - Pipes wires (insulate) - Steam leak observed at bunker tank return line in engine room at forward bulkhead. Steam line needs to be repaired and maintained to prevent injury to crewmembers. SOLAS 04 Cons. Ch 2-1 Reg 26.

0950 - Electric equipment in general. Terminations and joints in all conductors shall be so made as to retain the original electrical, mechanical, flame retarding, and where necessary fire-resisting properties of the cable. Electrical wiring for installed engineering alarm, main engine fuel oil filter and dead end wire at port side forward overboard valve improper. Solas 74 Ch 2-1 Reg 27

0720 - Fire fighting equipment. Fire extinguishing appliances shall be kept in good order and available for immediate use at all times during the voyage. Fire station #19 was leaking water at the valve. Solas 74 Ch 2-2 Reg 15

0938- Hull damage impairing seaworthiness - Approximately 14x2" crack in forward peak tank below the water line and just aft the bulbous bow. The administration shall satisfy in self that the general structural strength of the hull is sufficient for the draught corresponding to assigned freeboard.- ICLL 2005 Cons annex1 ch.1 reg.1 - 17(a)

1199- Other Cargo - Vessel needs to have oil transfer procedures for all oil transfer operations. Vessel did not have transfer procedures for lube oil transfer available. - 33 CFR 156.120 (t)(2) - 70(c)

MSD plant (Sewage) has the two following repairs that have not been approved by class: 1) Soft-Patch in pipeline leaping into tank, Tanktop 2) Tank top has been cut and Riveted and possibly comprimises the inner lining. (SOLAS 2004 Ch. I Reg. 2)

#3 SSDG has excessive F/O leak near the F/O Strainers. This oil leak creates a fire hazard in the engine room. (SOLAS 2004 Ch. I Reg.2)

THE VESSELS B/A CHARTS 1082, 899 AND 1063 WERE MISSING VARIOUS CORRECTIONS THE VESSEL WAS INSTRUCTED TO CORRECT THE CHARTS PRIOR TO DEPARTING.

A VIO OF 33 CFR 164.30 IN THAT THE VSL DID NOT HAVE O/B U.S. CHART 12304 ORIT'S ACCEPTABLE FOREIGN EQUIVALENT WHICH WAS REQ. FOR THE VSL'S ROUTE FROM CAPE HENLOPEN TO ARNOLD'S POINT.
