Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure that all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquid insofar as practicable. Excessive fuel/oil was found in the bilge of the engine room. Remove fuel/oil from the bilge of the engine room.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch (MAWP) must be equipped with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacture, by more than 10 percent. Current pressure relief valve is inadequate in relieving capacity; reduced in size to accommodate piping. Renew pressure relief valve with one of the same nominal pipe size as designed; do not utilize a reducer fitting when mounting the relieve valve to the pressure vessel.

Alarms and monitoring. Each vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. Vessel does not have a means of conducting an operational test of the main engines lubricating oil pressure, cooling water temperature, auxiliary generator lube oil pressure, cooling water temperature, or low hydraulic steering fluid levels. Vessel shall provide reliable means to test these alarms IAW manufactures specifications and/or standard engineering practices for inspectors to witness.
