...there shall be provided a number of approved life preservers suitable for children equal to at least 10 percent of the total number of persons carried. MI observed 20 children's life preservers that were unsuitable due to worn out retroreflective material. 46 CFR 180.25-5(a)

At each initial and subsequent inspection for certification and re-inspection the marine inspector shall examine and test the main and auxiliary steering apparatus to the extent necessary to determine that its condition is satisfactory and that it is fit for the service intended. MI observed a hydraulic leak originating from the steering gear in the engine room and lazarette. 46 CFR 176.25-35(a)

...Such hose shall overlap metallic pipe ends at least 1.5 times the pipe diameter and shall be secured at each end by two clamps of corrosion resistant material. MI observed a missing clamp on the coolant line of the stbd generator. 46 CFR 182.15-30(d)

MI observed blow-by on both generators and main engines, resulting in the accumulation of lube oil on machinery surfaces, creating a hazardous condition. 46 CFR 182.15-30(a)

Exhaust piping shall be led to the point of escape with a minimum number of bends or elbows. It shall be gas tight throughout its length and shall be properly supported by noncombustible hangers or blocks. MI observed an exhaust leak on the port main turbo.

The marine inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that they are in a satisfactory condition. MI observed the port and starboard shaft seals of the main engines are excessively leaking. - to be rectified at next dry dock. 46 CFR 176.10-5(b)

A door in a stairtower, stairway, and main vertical zone bulkhead must meet the following additional requirements: A door must be of the self-closing type capable of closing against a 3.5 list of the vessel. Door leading from main deck to galley must be adjusted to self-close. 46 CFR 116.435(c)(6)(i)

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in each of the following normally unmanned spaces. High level alarm indicator on bridge did not have an audible component and must be repaired. 46 CFR 119.530(a)

An adult life jacket must be provided for each person carried on board a vessel. Replace all non-serviceable life jackets. Renew reflective tape and required markings as needed. 46 CFR 117.71

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Automatic emergency lighting at galley and engine room escape not function and to be replaced. 56 CFR 120.432(a)

A self-propelled vessel must meet the applicable requirements for main and auxiliary steering apparatus. hydraulic oil leak from steering gear. Diagnose and adjust to prevent leaking. 46 CFR 119.600

The construction and arrangement of a vessel must allow the safe operation of the vessel... giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. a) Adjust shaft fitting on port side to prevent leaking. b) Renew oil hose on starboard engine to prevent oil leak. Remove all sitting oil from leaks and repairs. c) Replace air vent fan guard in galley. d) Clean exterior of black water discharge pipe to check e) Renew stanchions on 2nd deck railings where wasted. f) Renew/replace electrical covers that are damaged and missing in galley, engine room and for peak. g) Relocate electrical box and extension cords near galley scullery. 46 CFR 116.100(a)

Exhaust piping shall be gas tight throughout its length. Starboard engine exhaust has hole in piping. 46 CFR 182.20-20

Each life bouy must be of an approved type, constructed in accordance with subpart 180.050. Ring bouy on starboard side 2nd deck must be replaced. 46 CFR 180.30-1

a visual and audible alarm must be provided at the operating station to indicate a high water level. audible alarm was not functioning in all spaces except for steering. 46 CFR 182.530

all observed unsafe conditions must be corrected. MIs observed excessive corrosion in the M.E. battery bins and on the battery connection terminals. 46CFR 176.25-50

each ring life buoy must be of an approved type, constructed IAW 160.050. replace ring life buoy grab line. 46 CFR 180.30-1

where life preservers are stowed the containers in which they are stowed shall be marked "life preservers" with the number of life preservers contained. re-label the second deck life preserver storage to reflect accurate number stowed. 46 CFR 185.30-10

Repair port main engine raw water overboard pipe. Pipe had spot that was thin approx 1 foot before going overboard.

Clean areas IWO exhaust ports where they exit the hull and make ready for inspection.

Vessel failed to provide servicing report for fire-fighting systems. 46 CFR 176.25-25

Subpanel in galley found with without breaker and not blanked off. 46 CFR 183.01-15 (a)

Semi-submersible pump bilge pump in fwd part of engine room found improperly installed (installed temporary jumper cable to a 110 outlet). 46 CFR 183.01-15 (a)

Galley hood and ducting system found with excessive grease. Clean to satisfaction of attending inspector.

Update lifejack placard with correct number (482) in the forward storage locker on the main deck. 46 CFR 180.25-15

Interface from side shell to deck aft centerline and port quarter are wasted. Operator to provide permanent repair proposal.

Handle is missing from engine room emergency escape hatch operator to provide.

Operator to provide safety rail for for stove top, right side.

Rear upright wasted, fwd stbd bow cleat. Operator to repair/ renew.

Gauge at operating station is reading incorrect pressure. Operator to renew sending unit.

Portable fire extinguisher behind bar gauge base is bent causing reading to be questionable. Operator to provide service.

Retro-reflective material on two life buoys is severly degraded. Operator to renew.

Show proper operation of bilge system. Show the dewatering of all compartments.

Make repairs to leaking cross-over valve for the fire & bilge line.

Make repairs to port side galley stove vent box @ bulkhead.

Make vessel ready for sea. Properly stow vessel equipment / stores.

Degrease range hood and grey water tank/area around tank. Additionally degrease deck under range and wall IWO range.

Show proof of FSD operation (FSD leading to galley STBD side) with holdback (local, from control station and upon disruption of the power system).

Update labels on the following breaker panels: engine room panel #1, galley panels #'s 1 and 2, bridge panel #'s 1 and 2.

Repair or replace oil pressure gauge on port main diesel engine.

Replace expired items in first aid kit.

Provide guards for light globes in fuel compartment (3) and soda locker (1).

Repair or replace cooling water hose on port generator.

Repair or replace main deck door starboard side to make weather tight.

On the main deck stbd side forward the 2nd window aft needs to be reseated due to a man working over the side bumping it.

Main deck aft port side there is a broken window. Window needs to be replaced.

In the pilot house the port side window is broken and needs replaced.

Install cable rail course on stbd aft corner where missing.

Replace 15lb CO2 portable fire extinguisher located in the bar. Fire extinguisher is unacceptable for location as per 46CFR118.500 & 118.115

Make repairs to #1 generator saltwater discharge piping.

Make proper repairs to the #1 Marine Diesel Engine saltwater piping at the pump.

Conduct training for the crew on micro commanders and engine control.

Properly repair fuel oil leak on #1 generator fuel oil pump

Provide emergency lighting for engine room escape scuttle.

Provide report from Marine Electrician concerning ground to main switch board. Provide ground indicator lights for 120 volt distribution panel and main switchboard.

Properly secure hot water tank in engine room

Provide 02 more adult PFD's and provide retro reflective tape on 07 other PFD's. Prior to carrying 600 passengers.

Repair and prove proper operation of raw water piping on starboard main engine.

Repack port shaft packing.

Repair broken 220vac outlet in engine room.

Prove proper operation of bilge de-watering system from all spaces served.

Open fresh water tank for inspection.

Found leak in bottom plate in way of integral sewage tank. Required gauging of plate and opening of tank for further inspection.

Provide strainer for bilge suction in the #2 void.

Provide strainer for galley void bilge suction.

Found evidnece of wasted plate under galley at hull bottom near keel. Required gauging of hull bottom in way of galley void.

Found numerous areas of overhead A-30 insullation in engine room missing, sagging and peeling.

Found generator power supply cables to be routed in too severe an angle. Causing chaffing.

Found significant wastage in bottom hull plate in way of propellors.

A requirement had been issued previously to monitor lazerette due to unexplained leakage. Water appered to coming from under ballast cement near rudder penetration. Found 7" fracture along edge of wear-doubler plate on hull bottom in way of rudder post penetration at port side.

On or before next scheduled dry-dock scheduled 03JUN04, remove poured material inboard and forward of starboard rudder post and make bottom plate availible for inspection.

Provide ballast plan indicating exact location and amount of poured cement.

Until item #1 Dated 03MAR04 has been completed, the master must confirm and log condition of lazzarette prior to getting underway with passengers on board. Any change in condition shall be imediatley reported to [name withheld], FL.

Provide evidence of FCC ship station inspection.

Provide records of previous quarterly training.

Provide PA speakers inside bathrooms and galley prior to carry passengers.

POSITION ALL AROUND WHITE ANCHOR LIGHT IN A VISIBLE POSITION.

HAVE EVACUATION AND FIRE CONTROL PLAN READILY AVAILABLE ON VESSEL. PLANS MUST BE IAW 46 CFR 116.520/530.

Overboard line in #3 void space needs to be capped.

Fire main pump guage needs to be replaced or renewed.

Repair or replace STBD vent for steering room.

COMPLETE FIRE AND MAN OVERBOARD DRILLS PRIOR TO CARRYING PASSENGERS.

PROVIDE 50 ADULT PERSONAL FLOTATION DEVICES.

COMPLETE SEVICE AND HYDROSTATIC TEST ON THE 01-DECK GALLEY B-II CO2 PORTABLE FIRE EXTINGUISHER.

REMOVE DEAD END WIRES FROM ENGINEROOM.

PROVIDE COPY OF MASTERS RADIO OPERATORS PERMIT.

PROVE PROPER OPERATION OF 2ND RADAR.

PROVE OPERATION OF FIRST DECK DINING AREA E-LIGHT.

RENEW RETRO TAPE AND REPLACE BECKET LINE ON PORT PILOT HOUSE RING BUOY.

INSTALL ONBOARD A DAVIT FOR LOWERING THE RESCUE BOAT.

DEMONSTRATE SATISFACTORY OPERATION OF THE VESSEL'S TOILETS.

PROVIDE REPLACEMENT LUBE OIL PRESSURE GAUGE AND TRIP FOR STARBOARD GENERA- TOR. TEST LUBE OIL PRESSURE TRIP AFTER INSTALLATION.

PROVIDE A SUITABLE RESCUE BOAT AS REQUIRED BY 46 CFR 180.10-35. CONTACT THE COGNIZANT [name withheld] THE VESSEL WILL BE OPERATING FOR SPECIFIC REQUIREMENTS. CONDUCT DEMONSTRATION OF BOAT DEPLOYMENT. NO PASSENGERS TO BE CARRIED UNTIL THIS REQUIREMENTS IS SATISFIED.

CHANGE VESSEL'S NAME ON HULL AND LIFESAVING EQUIPMENT TO AGREE WITH VESSEL'S DOCUMENT. NO PASSENGERS TO BE CARRIED UNTIL THIS REQUIREMENTS IS SATISFIED.

DEMONSTRATE SATISFACTORY OPERATION OF THE VESSEL'S POTABLE WATER SYSTEM.
