"L. The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gauge." Gauge at fire pump had a constant erroneous reading. Replace gauge with operable gauge. 46 CFR 181.10-1(c) (OLD T)

"Switchboards shall be placed in dry, accessible and adequately ventilated locations preferably outside the engine compartment. The switchboard shall be adequately protected and provision shall be made for access thereto." Switchboard in main deck aft pantry had cover falling off. Reinstall panel cover. 46 CFR 183.05-15(a) (OLD T)

(a) The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel sustained damage to the bow and aft port corner. - Submit a repair plan for approval - Have an MI witness satisfactory repairs 46 CFR 116.100(a)

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in whioch it is installed and capable of operating at constant marine load under such requirementsa without exceeding its designed limitations. - Provide a technician report attesting to the proper operation of both main propulsion engines to ensure vesel is fit for its intended route and service. - Have an MI witness a successful sea trial

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. MI identified no fuel tank shut off valve at the fuel tank connection. Must have a fuel tank shut off valve at the tank connection accessible from the space within 14 days. 46 CFR 182.20-40(a)(3)

The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. MI identified missing insulation above the port engine. Address the missing insulation or submit risk mitigation plan for approval. 46 CFR 116.405(c)

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are inproper operating condition and fit for the service for which they are intended: engine control mechanisms. MI identified twisted wire exposed on port engine. Repair within 14 days. 46 CFR 176.25-10(3)

MI identified leak in port and stbd shaft seals and excess water in bilge. Repair within 14 days. 46 CFR 176.25-10(1)

Switchboards shall be fitted with driphoods and non-conducting hand rails. Non-conducting mats or grating shall be provided on deck in front and rear of switchboards. Non-conducting mats or grating for electrical panel located in engine room within 14 days. 46 CFR 183.10-15

The intent of this part is to insure that each vessel is provided with safe, readily available and sufficient firefighting equipment. MI identified insufficient pressure when testing main fire pump and inaccurate pressure gauge readings. 46 CFR 181.01-5(a)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. MI identified exposed rotating machinery on port engine. 46 CFR 177.960

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members [name withheld] system does not operate. Repair fixed PA system. 46 CFR 121.610 (b)

.. all connections shall be made to the battery terminals with permanent type connectors. Spring clips or other temporary type clamps are not permitted. Vessel has temporary type clamps on battery from charger. Vessel shall install permanent type connectors.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment...Vessel was not capable of dewatering void 4 & bilge pickup was not equipped w/ strainer. Complete repairs on bilge pump & install suitable strainer. 46 CFR 182.25-1 OLD-T

The general construction of the vessel shall be such as to minimize fire hazards. Vessel missing SFP above DJ booth, office overhead. Vessel shall replace SFP. 46 CFR 177.10-5 (a) OLD-T

Vessel fitted with loading doors shall assure that all loading doors are closed watertight. Vessels port galley WTD door is difficult to operate & gasket is damaged. Vessel shall service door and prove watertight integrity. 46 CFR 185.20-17 (a) OLD-T

..vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. 01 bar receptacles are removed from bulkhead, exposing wiring. Vessel shall properly install electrical receptacles. 46 CFR 183.01-5 OLD-T

All electrical installations shall be to satisfaction of the Officer in Charge, Marine Inspection. 02 deck STBD floodlight has broken mounting hardware. Repair broken hardware. 46 CFR 183.01-15 OLD-T

All electrical installations shall be to satisfaction of the Officer in Charge, Marine Inspection. 02 deck STBD embarkation light is mounted w/ temporary zip ties. Vessel shall permanently mount light fixture. 46 CFR 183.01-15 OLD-T

Provide full electrical load analysis of vessels systems.

Auxiliary machinery shall be suitable in type and design for the purpose intended. Fire pump is not operational. Complete repairs on fire pump. 46 CFR 181.15-1 (a) OLD-T

All electrical installations shall be to satisfaction of the Officer in Charge, Marine Inspection. Wire runs & engine harnesses throughout vessel are unsupported. Vessel shall support wire harnesses throughout vessel. 46 CFR 183.01-15 OLD-T

Engine control mechanisms. Such mechanisms shall be tested by operation and examined, visually. Vessel has active high J/W alarm on STBD generator. Vessel determine cause of alarm and conduct repair. 46 CFR 176.25-10 (a)(3) OLD-T

Marine inspector conducts inspections of the hull structure and its appurtenances. Vessel has cracked windows on main deck on STBD side. Vessel shall repair cracked windows. 46 CFR 176.25-5 OLD-T

Vessel's STBD fuel cutoff valve did not operate. Vessel corrected deficiency w/ marine inspectors onboard. 46 CFR 182.20-40(b)(3) OLD-T

Each life jacket kept in a stowage container must be readily available. Vessel's aft main deck life jacket locker door was obstructed and life jackets were not readily available. Vessel shall stow life jackets so they are readily available. 46 CFR 117.78 (2)

At each....subsequent inspection for certification...the marine inspector shall require that all observed unsafe practices and hazardous situation be corrected. The fuel governor on the port main engine was leaking. The fuel filter transfer pump on the port main engine was leaking. 46 CFR 176.25-50(a) "Old T"

At each intitial and subsequent inspection for certification...the maine inspector shall require that all observed unsafe practices and hazardous situations be corrected. The port service diesel generator was leaking oil from the rack handle causing a severe fire and electrical hazard. 46 CFR 176.25-50(a) "OLD T"

Vessels contracted before on or after July 1, 1961 which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H of this chapter. An A Class door on the main deck on the port side midship was missing a handle and did not fully close. 46 CFR 177.10-5(a) "OLD T"

All bulkheads and decks shall be classed....upon the type of space on each side of the bulkhead or above and below the deck. (1) bulkheads or decks of the "A" class shall be composed of steel or equivalent metal construction. structural fire protection insulation was missing between the underside of Deck 2 in the overhead drop ceiling. 46 CFR 177.10-5(a) "OLD T"

Crew is not familar with the procedures to follow in a situation where passengers and crew neet to abaondon the ship.

While conducting a fire drill onboard for an engine room fire, the crew did not display strong competence.

two weathertight side loading doors do not seal properly. There is no closure device on the door to a storage closet on the port side of the vessel near the bow.

Multiple power strips were found on the vessel. Specifically in the DJ storage room on deck 2, there were two power strips to charge approx 20 radios. Must prove a need and amp/watts does not exceed the the assigned circuit breaker. Must be mounted and stored properly during use to prevent fire.

The owner must submit drawings, sketchs, or written specifications describing the details of any proposed alterations the cognizant [name withheld] new installations were found, to include the addition of a bilge alarm panel on the bridge, an air conditioning unit, shore tie connection and led lighting on deck 1 and 2.

Vessels over 65 feet in length shall be equipped with two fixed fire pumps. Vessels port MDE PTO bilge pump was not functioning and crew had no knowledge of operation.

Examine and test by operation all watertight closures in the hull, decks and bulkheads. The watertight door on the port side of the storage room near the entrance to the steering compartment did not completely seal when closed. There were hidden temporary repairs made in the coaming that the CG was not made aware of.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible…Vsl did not have adequate charts onboard.

Each flare carried to meet this section must be stowed in one of the following: (1) A portable watertight container carried at the operating station, and marked as required by § 122.614 of this subchapter; or (2) A pyrotechnic locker secured above the freeboard deck, away from heat, in the vicinity of the operating station. The hand red flare distress signals were in plastic bags on the bridge.

An ammeter that is readily visible shall be included in the battery charger circuit. There was not an ammeter connected to the battery charger underneath the operating station on the bridge.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. There were dead end wires in the cabinet underneath the operating station on the bridge.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The children and adult lifejackets stored in the aft locker located on Deck 3 were mixed.

Portable fire extinguishers shall be installed and located to the satisfaction of the Officer in Charge, Marine Inspection. The portable fire extinguisher located in the DJ storage room on Deck 2 was not properly secured in a mount.

At all inspections special attention shall be paid to ensure that no fire hazards exist and that guards or protective devices are provided in all hazardous places. A section of lagging was missing on the starboard main engine exhaust line.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Water was leaking from the exhaust water separator on the starboard ship service diesel generator.

Examine and test by operation all watertight closures in the…decks…The watertight hatch for the steering compartment had a wasted ring.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: (1) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Neither Master had their Marine Radio Operator permit available for inspection.

Vessels shall be equipped with a suitable number of portable battery lights. Vsl did not have portable lights onboard.

The fuel systems for the main propulsion engines, all auxiliaries and all other fuel systems installed shall be checked. The plugs on the bottom of fuel filter shields were plastic causing a fire hazard

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. There was no guard on the rotating belt on the front side of the starboard main engine.

Each…ring life buoy must be marked with Type II retroreflective material approved in accordance with § 164.018 in subchapter Q of this chapter, or other standard specified by the Commandant. The letters of the vessel’s name covered the retroreflective material on a ring lifebuoy on Deck 3.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. The cover for the lights in Void #2 was removed.

After loading and prior to departure, and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's…stability letter, Certificate of Inspection…as the case may be. Large speakers on Deck 1 and Deck 2 were not safely secured for sea. There were multiple areas, to include the storage room where access to the steering compartment is, the two alcohol storage areas adjacent to the engine room, by the bar, in the storage locker on the port side of the vsl, the DJ storage room, and in the cabinets on the bridge where MIs identified excess stowage. Additionally, there was combustible gear that was stored on top of the compressor for the walk-in refrigerator located near the main entrance the engine room. Boxes were taken out of the engine room by crew during the inspection.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years…If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. The ship did not have a Communications Act Safety Radiotelephony Certificate onboard at the time of inspection.

At each…subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service for which they are intended: (2) Overload or circuit protective devices. The dead front covers were bent and not properly secured on the port and starboard distribution panels in the engine room causing a personnel electrical hazard.

Except as allowed by paragraphs (b) and (c) of this section, before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made. Vessels safety orientation did not contain all information in 46 CFR 122.506.

All electrical installations shall be to the satisfaction of the OCMI including the IEE and NEC. Electrical panel in galley port side was not marked properly and spare number three was energized.

Vessels contracted before or after July 1, 1961, which carry more than 150 passengers shall meet the requirement of subpart 72.05 of Subchapter H (Passenger Vessels) of this chapter. An A-Class door on the 2nd deck aft failed to fully close.

At least one ring life buoy must be fitted with fitted with a lifeline. The lifeline must be buoyant, 60 feet in length & dark color. 46 CFR 117.70(c)

A vessel of more than 65 feet in length must carry a minimum of three life buoys of not less than 24 inches in diamter. 46 CFR 117.70 (a)(3)

Each life jacket kept in a storage container must be readily available. Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. 46 CFR 117.78 (a)
