All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering). There is a pin hole in the return fuel line that runs from the emergency generator day tank to the fuel tank in the # 3 void. The pin hole is in the section of the line that runs through the men's restroom. Make satisfactory temporary repair to the return fuel line. Permanent repair must be completed at the next drydock to the satisfaction of the attending USCG Marine Inspector.

Each self-propelled vessel, except as provided in paragraph (d) of this section, must have an electric or mechanical engine order telegraph system from the navigating bridge to the engineroom. MI observed the EOT not working as designed.

Pipe joining fittings meeting the standards specified in this part and certified in accordance with subpart 50.25 of this subchapter are acceptable for use in piping systems. MI observed the port side steering gear room pipe union to be damaged and not as designed.

Each rescue boat must be capable of being launched with the vessel making headway of 5 knots in calm water. A painter may be used to meet this requirement. MI was notified that the locking pin for the davit broke. Vessel is able to get underway in the basin to conduct repairs. 46 CFR 199.160(a) 60C

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. MI was notified that the port shaft seal had water intrusion into the hull. Repairs will need to be completed on the drip less shaft seal. Vessel will need authorization from the USCG to get the vessel underway for power trials or for the vessel to attend emergency drydock.

During the fire drill, it was found that both nozzles were leaking water from every seal/connection of the nozzles. Crew stated that all fire nozzles were leaking and needed rebuilding or replaced. Conduct repairs or replace all leaking fire nozzles and test to the satisfaction of attending CG Marine Inspector.

The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. During re-inspection found the gunnel on port side mid ship had large inset causing the deck to push up and caused several gussets on the bulworks to trip and fracture several welds on top of bulworks. Remove insulation from engine room and MSD space around damage area for CG Marine Inspectors to check for damage and conduct repairs to the satisfaction of attending CG Marine Inspector at next drydock in May 2026.

Port Shaft was found with excess vibrations causing the coupling bolts to back out multipile times over the past month. Conduct repairs, satisfactory sea trial and provide tech report to the satisfaction of attending CG Marine Inspector prior to carrying passengers.

The rescue boat launching and recovery arrangement does not comply with 46 CFR 199.160(e) and 199.160(g) because the installed 1,500-lb davit cannot recover the rescue boat with its full complement of persons and equipment; NCDOT must either upgrade the rescue boat/davit system to full regulatory compliance or obtain an approved exemption from CGD East in accordance with 46 CFR § 199.20(d) prior to return to service following the next scheduled drydock.

Branch circuits supplying power to lights for survival craft launching operations must supply no other equipment. MI observed that both the port and STBD liferaft launching appliance lights did not operate as designed.

Lifebuoys. Each vessel must carry lifebuoys approved under approval series 160.150 as follows. MI observed the STBD aft lifebouy was damaged with a hole rubbed through the protective plastic.

Where a carbon dioxide extinguishing system is installed, the provisions of this subpart, with the exception of 46 CFR 76.15-90, shall apply to all installations contracted for on or after November 19, 1952. The flexible hose form the CO2 bottle that is furthest inboard to the main distribution pipe is cracked. Replace cracked flexible hose to the satisfaction of the attending Marine Inspector.

At least on-half of the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 46 CFR 161.010. The battery to the light attached to the forward starboard lifebuoy is expired. Replace battery.

Float light attached to life ring on port quarter was not functioning properly

Periodic tests and inspections shall be made of main and auxiliary machinery... fit for the service for which they are intended. The port main engine oil level increased overnight; possible coolant or fuel dilution suspected.

M/V Frisco 946908 has lost both rudders on Sunday 09 June 2024. After a quick evaluation it was determined both rudders are not working. The steering system is working inside the boat but the boat does not respond to the steering commands.

In the inspection of machinery of vessels, the standards established by the American Bureau of Shipping... shall be accepted as standard by the inspectors. Both shaft cooling alarms found inoperable.

All watertight doors in subdivision bulkheads shall be kept closed during navigation except when necessarily opened for working of the vessel, and in such cases they shall always be ready to be immediately closed. Watertight door on the main deck leading to the engine room failed to immediately close and become watertight.

Loss of propulsion port engine.

Exposed blade on fan for heating/cooling unit in pilot house. Poses hazard to the crew. Repair in accordance with 46CFR77.05.

Properly mount electrical boxes for AC in MSD space. Mount in accordance with 46CFR77.05

Pull cable for valve release on fixed CO2 failed during testing, replace with new prior to carrying passengers iaw 46 CFR 76.15-10.

Starboard PTO driven fire pump is providing lower that 40 PSI while running at 125 PSI at suction, troubleshoot and provide course of action for remedy iaw 46 CFR 70.10-5.

Port rudder assembly is leaking excessively at the hull penetration. NCDOT QA tightened packing during inspection to remedy, crew to monitor and report if additional/continued leaking and notify CG Marine Inspector.

Remove dead end wiring located in EDG space iaw 46 CFR 77.05-1.

Replace fuel tank on rescue boat, current is in disrepair iaw 46 CFR 199.190(a).

Jockey bar teflon chaffing plate is wore and in need of replacement prior to contact with install bolt iaw 46 CFR 77.03-1.

Ensure drills are conducted prior to carriage of passengers, current captain and crew conducted sat, prior week there is no record in log book iaw 46 CFR 199.215.

Ensure rescue boat is outfitted with all required equipment including repair kit iaw 46 CFR 199.175.

Lifebuoy light is working intermittently, conduct additional inspection to verify working order or replace with new iaw 46 CFR 199.70(a)(3).

Bilge pump housing piping to gauge and aux hose broken off. Make permanent repairs to bilge pump and test suction in each compartment and verify port and stbd rudder tubes are properly adjusted to eliminate water colleting in bilges. All repairs are to the satisfaction of attending MI.

Emergency Generator Room CO2 bottle straps are loose. CO2 head loose. Tighten in accordance with 46CFR76.05-20.

Port shaft cooler alarm cable spliced. Install junction box in accordance with 46CFR111.60-19.

STBD MDE exhaust lagging worn/holed. Replace/repair in accordance with 46CFR58.05-1

NCDOT maintenance contacted MI directly in reference to sluggish rudder commands due to possible missing rudder. Capt noted issue at South Dock on 06 Dec 2021 @ approx. 2030. Vessel is to be hauled out for inspection and permanent repairs. All repairs are to be completed to satisfaction of attending [name withheld] 46CFR71.55-5.

On November 19th 2021 M/V FRISCO experienced steering issues. Identify cause and make repairs to the satisfaction of a Coast Guard Marine Inspector IAW 46CFR58.10-10.

Light on stern of rescue boat is inoperable. Replace/repair IAW 46CFR199.176

Light in control room is missing a cover. Replace IAW 46CFR72.03-5

Port MDE exhaust lagging is damaged. Replace IAW 46CFR58.60.13

Prove 50% of crew holds a valid first aid CPR card. Currently 6 crew onboard with only two valid First Aid CPR cards. 3 cards exprired, 1 card missing, 2 valid.

#1 MDE deflector on glass fuel filter bowls. Total number of deflectors throughout vessel were 10 and missing 2.

The owner, master, or PIC shall perform the tests and inspections of all hand portable fire extinguishers as described in 46 CFR 71.25-20. During the inspection, inspectors were unable to verify that satisfactory maintenance was performed on the portable extinguishers by a properly licensed servicing facility/individual.

Found unapproved modification to the port longitudinal frame and deck support stantion iwo main sea strainer, between transverse frames 53 & 54. Submit plans to MSC for approval Note: Modification exist class wide with slight variations and was made years ago to accommodate the large sea strainer.

A relief valve has been installed in an effort to limit FM pressure however, if the engine rpm is raised the PTO driven fire pumps are still capable of producing pressures in excess of 150 psi. Provide means to comply with regulation.

Found the stbd fire pump strainer foundation mount missing. Install strainer foundation mount.

Found the following water tight doors and/or hatches unable to make watertight: - main deck to engine room - main deck to bow thruster - main deck to fuel void

Unexplained ingress of water into the MSD space. Sea trials required with Marine Inspector present and deck plates removed for observation.

Found PTO driven fire pumps capable of producing pressures in excess of 125 psi with no relief valve installed. Install relief valve next drydock.

Starboard main engine breaker opened and engine stopped. Repair breaker to the satisfaction of CG marine inspector.

Repair and prove proper operation of the Port Side PTO fire pump.

Remove or place all dead end wires in a proper juction box under CCR control panel.

Provide the approved PSTP's for the CAT-18 and Twin Disc as required.
