All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months. Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire Extinguishers. Certificates detailing service on all fire extinguishers need to be provided to inspectors for both portable and semiportable extinguishers. 46 CFR 142.240(a)(1)

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Port and Stbd engines leak excessive amounts of oil. Repair to the satisfaction of attending Marine Inspector. 46 CFR 143.205(b)

Piping and machinery components that exceed 220 °C (428 °F) must be insulated. Insulation on Port Side Engine was torn and worn. Replace to the satisfaction of the Attending Marine Inspector. 46 CFR 143.220(b)

5. Connections and terminations must be suitable for the installed conductors, and must retain the original electrical, mechanical, flame-retarding, and where necessary, fire-resisting properties of the conductor. All fire hoses must be inspected for excessive wear, and subjected to a test pressure equivalent to the maximum service pressure. All fire hoses which are defective and incapable of repair must be destroyed. Stbd fire hose was leaking. Replaced during inspection. 46 CFR 142.240(a)(7)

The name of the towing vessel must be marked or painted in clearly legible letters on each EPIRB, except on an EPIRB in an inflatable liferaft. The owner or managing operator must maintain valid proof of registration. EPIRB not registered. Name not on EPIRB. EPIRB has not been tested. Rectify to the satisfaction of the attending Marine Inspector. 46 CFR 141.380( c)(d)

Each towing vessel must carry at least one appropriately-sized lifejacket for each person on board. 03 lifejackets were no longer serviceable. Replace 03 lifejackets. 46 CFR 141.340

Stowage positions for lifejackets stowed in a berthing space or stateroom and all lifejacket containers must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the lifejackets stowed inside the container. Lifejacket were stowed in a container without required labeling. Label container appropriately. 46 CFR 141.340(h)

A portable watertight container carried at the operating station. Portable watertight containers for pyrotechnic distress signals must be of a bright color and must be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high with “DISTRESS SIGNALS” Container was not appropriately labeled. 46 CFR 141.375( e)(1)

Towing vessels must be equipped with the following equipment, as applicable to the area of operation a Fathometer (except Western Rivers). Fathometer was inoperable. Rectify to the satisfaction of the attending Marine Inspector. 46 CFR 140.725(a)

Towing vessels must be equipped with the following equipment, as applicable to the area of operation a Search light, controllable from the vessel's operating station and capable of illuminating objects at a distance of at least two times the length of the tow. Search light light bulb was inoperable. Rectified during inspection. 46 CFR 140.725(b)

The emergency duties and duty stations required by this section must be posted at each operating station and in a conspicuous location in a space commonly visited by crewmembers. 46 CFR 140.405( c) Unless otherwise stated, each crewmember must receive the instruction required by this section annually. 46 CFR 140.420( c) Captain and crew were unaware of where to find emergency procedures. Provide and post emergency procedures for crew and Captain awareness.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Alcohol strips were expired. Ensure vessel is equipped with valid alcohol strips for maximum persons onboard (6) at minimum. 46 CFR 4.06-15(a)(1)

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Illuminated magnetic compass. The compass must be readable from each operating station. Compass was not illuminated and GPS has not been recently calibrated. Provide proof of calibration and ensure illumination. 46 CFR 140.725( c)

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. Guards on both engines are forward of the rotating parts. Move guards to cover rotating parts. 46 CFR 144.820

Air filters on Port side engine were excessively dirty. Replace air filters on Port side engine. 46 CFR 143.205(a)

Each towing vessel must be fitted with a general alarm that: Is activated at each operating station and can notify persons onboard in the event of an emergency; General alarm could not be activated. Rectify to the satisfaction of the attending Marine Inspector. 46 CFR 143.235(b)(1)

Each towing vessel must have a fire-detection system installed to detect engine room fires. Engine room starboard heat detector was inoperable during testing. 46 CFR 142.330(a)

The results of the inspections required by 33 CFR 164.76 must be documented in the TVR, or official logbook. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. The vessel did not have any recordkeeping for towing gear on board. 46 CFR 140.820

A fixed fire pump must be capable of delivering water simultaneously from the two highest hydrants at a pressure of at least 50 psi and a flow rate of at least 80 gallons per minute. Installed fixed fire pump could not reach 50 psi. 46 CFR 142.325(a)(1)

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire. There are no dampers or ventilation covers to stop air flow to the engine room. 46 CFR 144.605

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Port and starboard generator windings are coated excess oil and dirt, and require cleaning. 46 CFR 143.400(a)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Life raft painter line on board was not secured to the weak link and would not release upon hydrostatic release activation. 46 CFR 199.190

Machinery must be designed and maintained to provide for safe operation of the towing vessel. Port and starboard generator fuel filters are not properly secured. 46 CFR 143.205(a)

AIS must be maintained in effective operating condition, which includes the accurate input and upkeep of all AIS data fields and system updates. The currently encoded antenna position dimensions and draft are inaccurate and must be encoded in meters, not feet.

The owner, managing operator, or master of a towing vessel must ensure that the vessel’s lifesaving is in good working order. The starboard side lifebuoy had a wasted plug in the outer shell. In addition, the port and starboard lifebuoy’s retro reflective tape needs to be replaced.

The frame or support of any semiportable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. Permanently attach the frame for the semiportable fire extinguisher.

Electrical conductors passing through watertight bulkheads must be installed so that the bulkhead remains watertight. Several cable penetrations above the pilothouse were not watertight and are required to be sealed.

Towing vessels must meet the communication requirements of 33 CFR 26 and 33 CFR 164.72 as applicable. The starboard forward VHF antenna bracket is corroded and is required to be remounted.

Each remote fuel shutoff valve control must be marked in clearly legible letters, at least 1 inch high, indicating the purpose of the valve and the way to operate it. Found no labeling of remote valve controls.

Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. 01 lifejacket in the crew berthing had an expired lifejacket light.

Electrical systems and equipment must minimize fire and shock hazards. One (01) 120 VAC duplex outlet in the galley near the table was missing its receptacle cover plate.

Piping and machinery components that exceed 220 degrees Celsius, including fittings, flanges, valves, exhaust manifolds, and turbochargers must be insulated. Found the exhaust lagging loose for the port main engine, port/starboard generators, lock wire was missing.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. The outer cable insulation running into the bottom of the capstan electrical box was not held in place by the cable clamps.

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. The port and starboard generators are missing guards over the rotating pulley on the front of the engine.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitting with a floating electric water light approved under approval series 161.010 or 161.110. The installation of corrosion resistant clip for both the port and starboard water-lights did allow the water-lights to float.

The owner, managing operator, or master of a towing vessel must ensure that the vessel’s lifesaving and fire suppression and detection equipment is in good working order. The portable fire extinguisher near the starboard side engine room door had excessive corrosion around the neck to the cylinder and requires replacement.

A vessel of 20 meters or more in length shall be provided with a bell. Replace the bell clapper.

Pressure vessel must be equipped with one or more spring-loaded relief valves. Found the both the port and starboard start air receivers relief valves would not reset after being hand relieved.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: High bilge levels. Center bilge alarm on engine room was non operational.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. - Water found dripping on the port side gen set. - Inspectors found exposed wires above port and starboard MDE. -Inspectors found exposed wires under operating station. - Grounding cables on STBD side generator are disconnected from ground.

The employer shall provide an Employee Assistance Program (EAP) for all crew members. All safety- sensitive employees must be enrolled in an acceptable drug and alcohol testing program. All individuals serving in a licensed capacity on vessel need enrollment into company DAPI. Master is required to be added to the drug and alcohol program.

A towing vessel that is certified for rivers, lakes, bays, and sounds, less than 3 nautical miles from shore must be equipped with an approved 160-B semi-portable fire-extinguishing system or a fixed fire-extinguishing to protect the engine room. -Vessel was not equipped with a 160-B semi-portable fire extinguisher or a fixed fire extinguishing system.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light. -Port side water light attached to life buoy not operable.

The following list of items must be recorded in the TVR, official logbook: Personnel records, Safety orientation, Record of drills and instruction, Examinations and tests, Operative navigational safety equipment, Navigation assessment, Navigation safety training, Oil residue discharges and disposals, Record of inspection of towing gear, Fire-detection and fixed fire-extinguishing. - No presence of records on board vessel for Personnel records, safety orientation, navigation safety training, examinations and tests, pre-departure tests and inspections.

When the towing vessel is underway, the EPIRB must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the towing vessel sinks. -EPIRB hydrostatic release unit unit found expired.

(a) All towing vessels must be operated in a manner that minimizes the risk of down-flooding and progressive flooding. The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. -Install gasket on galley port hole cover. -Make watertight galley door.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 75% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.

Essential systems or equipment must be regularly tested and examined. If a component is found unsatisfactory, it must be repaired or replaced. The bilge manifold showed signs of severe wastage on piping which led to a steady water leak into the bilge.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weathertight function properly. The forepeak hatch gasket was severely wasted.

All machinery and equipment that is not in proper working order must be removed; made safe through marking, tagging, or covering; or otherwise made unusable. The PVC piping and pipe thread in the engine room overhead used for sewage is rusted and severely wasted. Piping penetrating the deck into the engine room from the washer was also found rusted and wasted.

No person shall conduct an oil or hazardous material transfer operation unless each drain and scupper is closed by mechanical means required by 155.310 for fuel oil containment. Both the port and starboard side fuel oil fill station enclosed coaming had openings at the bottom that would not allow for a plug or other device to contain fuel oil if discharged during transfer.
