The fire pump outlet shall be fitted with a pressure gauge. The gauge did not work on the fire pump. 46 CFR 181.10-1(c)/16c

L. All vessels shall be fitted with three ring life buoys of not less than 24 inches in diameter. 01 life ring was unserviceable. 46 CFR 180.70(a)(3)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. The certificate of Inspection shall, in general, be framed under glass or other transparent material and posted in a conspicuous place. COI was not posted. 46 CFR 176.01-40

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List. Vessel did not have current Light List and Coast Pilot on board. 46CFR184.420

An emergency instruction placard containing the following information will satisfy the requirements of § 185.510. Fire. (i) Cut off air supply to fire—close items such as hatches, ports, doors, ventilators, and louvers, and shut off ventilation system. (ii) Cut off electrical system supplying affected compartment if possible. (iii) If safe, immediately use portable fire extinguishers at base of flames for flammable liquid or grease fires or water for fires in ordinary combustible materials. Do not use water on electrical fires. (iv) If fire is in machinery spaces, shut off fuel supply and ventilation and activate fixed extinguishing system if installed. (v) Maneuver vessel to minimize effect of wind on fire. (vi) If unable to control fire, immediately notify the Coast Guard and other craft in the vicinity by radiotelephone. (vii) Move passengers away from fire, have them put on life jackets, and if necessary, prepare to abandon the vessel. Vessel did not have detailed instructions for machinery space fire response. 46CFR185.512(3)

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line must be marked to indicate the space served. Instructions were not available at the operating station or locally at the CO2 bottles. No schematic diagram was present locally at the CO2 bottles. 46CFR185.612(a)

Piping, valves, and fittings must be securely supported, and where necessary, protected against damage. The CO2 system distribution piping coming from the bottle's was not secured in its clamps. 46CFR181.410(d)(3)

All vessels must have navigational lights that are in compliance with the applicable section of the International and Inland Navigational Rules. All around white light was found inoperable. Vessel restricted to daylight operations, and no operations during restricted visibility. 46 CFR 183.420

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Submit plans to MSC for approval. 46CFR181.405(c)

On vessels described by 46 CFR 175.110(c), flammable items not otherwise covered by the regulations of this subchapter, such as rechargeable batteries, including lithium ion batteries utilized for commercial purposes, must be handled, stored, and operated in a way that mitigates the risk of hazardous conditions. Provide plan to attending MI's. 46CFR185.364

For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include but need not be limited to - (1) Training in the use and location of firefighting equipment and firefighting knowledge, including: (i) Location of firefighting appliances and emergency escape routes; (ii) Types and sources of ignition; (iii) Flammable materials, fire hazards and spread of fire; (iv) The need for constant vigilance; (v) Actions to be taken on board; (vi) Fire and smoke detection and automatic systems on board; and (vii) Classification of fire and applicable extinguishing agents. (2) The drills required by § 185.524, including fire location and fire type; and (3) Emergency egress training for each member of the crew, to occur for all members of the crew - (i) At least monthly while such members are employed on board the vessels; and (ii) Each time a crew member joins the crew of such vessel. Provide training program to attending MI's. 46CFR185.420

Light list and Coast pilot were not up to date. Vessel owner showed proof of purchase and tracking information and given 07 days to rectify.

Machinery, equipment, materials and arrangement not covered by the standards in this subchapter may be inspected in accordance with standards acceptable to the OCMI as good marine practice. Make ballast installation into more permanent of a configuration. Submit & communicate ballast configuration design to local OCMI prior to changing the configuration. To be completed prior to February 2022.

46 CFR 178.230 Vessel underwent a recent repowering and received informal approval from MSC to install led ballast to offset the weight change. O/O still does not have official MSC approval letter for the ballast. Provide USCG MI the official MSC approval letter attesting to led ballast install.

Due to recent engine modifications and weight changes, vessel's stability letter is nullified. Vessel must reach out to MSC to determine if new test needs to be completed to issue new letter prior to operating with passengers.

An exposed hazard such as rotating machinery must be properly protected by a cover. Engines had exposed hazards on belts and shafts. Required to add guards.

Generators must be designed for an ambient temperature of 50C or be de-rated to 80% if designed for 40C. Stbd generator was rated for 40C. Required vessel to contact generator manufacturer to get temperature testing range.

Vessel returned to homeport prior to attending marine inspector witnessing operational test of the vessel's two new main engines. Conduct operational test to the satisfaction of attending CG Marine Inspector.

Alcohol testing devices need to be carried on board each vessel. Found expired alcohol testing devices on board. Vessel is restricted to operate within two hours of their open collection site. Outfit vessel with valid alcohol testing devices to the satisfaction of the attending Coast Guard Marine Inspector.

The fire pump failed to produce the proper PSI. Repair /Replace the fire pump to produce the Proper PSI. Due date is 30 April 2018. Old T 46 CFR 181.10-1 (c).
