The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel shall proceed to drydock for permanent repairs. Owner shall provide transit plan for approval prior to departure. Make repairs to the satisfaction of the attending Coast Guard marine inspector.

Starboard generator had coolant leak. All installations shall be of marine type engine suitable for the intended service. Make repairs to the satisfaction of the attending Coast Guard marine inspector

The reverse power relay failed during and test. Each generator arranged for parallel operation must have reverse-power or reverse-current trips. Make repairs to the satisfaction of the attending Coast Guard marine inspector.

Port and starboard generators over speed failed during annual test. The control system shall be designed for automatic shutdown of the engine with the actuation of audible and visual alarms for over speed. Make reapirs to the satisfaction of the attending Coast Guard marine inspector.

Port generator failed low lube oil pressure during annual test. The control system shall be designed for automatic shutdown of the engine with the actuation of audible and visual alarms for over speed. Make repairs to the satisfaction of the attending Coast Guard marine

During SIP Audit/ Inspection - witnessed crewmember perform ICR inspection P 05 G. Noted control panel cover screws on top of panel. When panel was opened, found wires disconnected from one of the breakers. Crewmember was unaware of loose wires before preforming inspection. SIP rep stated the chief had Carrier working on system. System was not tagged out, required rep to have system tagged out.

While witnessing crewmember perform ICR P-05 paragraph G. it was noted that wires were disconnected from one of the breakers inside the A/C control panel. Company is NOT following Safety Program procedures for log out tag out. Review procedure and provide training to ALL personnel.

During SIP Audit/ Inspection - witnessed crewmember perform ICR inspection J-02 B. 1. Noted that the warmers on wheels and are unsecured and stationed near their deep fat fryers presenting a hazard. The electrical cables from the warmers are touching the handrails to the ladder coming into the space creating a potential electrical hazard if there were a short. Move warmers and plugs to a less hazardous location and properly secure warmers.

Marine Inspector recognized the conditions of the concrete box patch in thruster tunnel to have changed. After further investigation, it was revealed that the temporary concrete patch was removed due to an ineffective installation and the vessel was intentionally beached on a sandy bottom and the crew installed a new box patch. The event was not logged, the repair was not indicated in the SIP correction report. SIP 2-99: Correction Reports become part of the company’s reporting mechanism and can be instrumental in identifying and correcting recurring problems with the vessel systems and subsystems. 46CFR71.55-1(a): No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery or equipment shall be made without the knowledge of the Officer in Charge, Marine Inspections. Multiple failures in the records keeping and deficiency reporting of structural watertight integrity company shall conduct training for the company SIP agent who is responsible for the implementation for the Vessel Action Plan. daily inspection records of the affected space shall be maintained that specifies water level and daily pump out. If any changes to the space occurs master shall contact the Officer in Charge Marine Inspections

Vessel Rep could only provide three months of logbook entries. The master or person in charge of a vessel that is not required by 46 U.S.C. 11301 to have an official logbook, shall maintain, on board, an unofficial logbook or record in any form desired for the purposes of making entries therein as required by law or regulations in this subchapter. logs must be kept available for review by a marine inspector for a period of 1 year after the date to which the records refer. Maintain logbook records for a total of 1 year for inspection for review by a marine inspector to the satisfaction of the attending Coast Guard marine inspector.

Vessel galley has a griddle and deep fryer fixed fire fire fighting system. Once the the fixed fire fighting system is activated the ventilation shall automatically stop the ventilation. During fire drill it was discovered that the ventilation shutdown was not working for all fans. Exhaust for galleys shall meet the applicable bulkhead requirements. In addition an automatic damper shall be installed in exhaust ducks over frying vats ect. Make repairs to the fixed fire fighting automatic ventilation shutdown in the galley. To the satisfaction of the attending Coast Guard Marine Inspector. Do not use deep fat fryer and griddle.

Galley ventilation above deep fat fryer and griddle had excessive grease buildup and lent that is at risk for fire. The general construction of the vessel shall be such as to minimize fire hazards. Clean ventilation above deep fat fryer and griddle to the satisfaction of the attending Coast Guard marine

Watertight door handle in the galley is coming into contact with an electrical panel cover which has partially broke free from bulkhead. If the electrical panel cover come completely out from bulkhead it will impede the WTD handle and prevent the door from closing in the event of a fire or flooding. The general construction of the vessel shall be such as to minimize fire hazards. Make repairs to the satisfaction of the attending Coast Guard marine inspector.

An extension cord supplying a freezer was handing over the top of the galley fryer vat creating a fire hazard. The general construction of the vessel shall be such as to minimize fire hazards. Remove extension cord over fryer vat to the satisfaction of the attending Coast Guard marine inspector.

Vessel fixed fire fighting halon agent was accidentally discharged by Silco fire and security during test of the system leaving the engine room without a fixed fighting system. The company is waiting for a replacement bottle. An approved extinguishing system must be installed as required by table 76.05-1 on all self propelled vessels. Vessel shall not operate engine room machinery until fixed fire fighting system is repaired. Rectify deficiencies prior to movement.

Solenoid valve for generator failed.

Vessel representative could not provide training form (B&B Riverboats Marine Employee Training System Employee Log) that is used to track employees training which is compulsory in the company action plan. The crew must be instructed in the use of the vessel's fire-extinguishing and lifesaving appliances and in survival at sea at the same interval as the drills. Individual units of instruction may cover different parts of the vessel's lifesaving and fire-extinguishing appliances, but all the vessel's lifesaving and fire-extinguishing appliances must be covered within any period of 2 months. n2-99 track and record training for individual employees, and the training required for the designated SIP support personnel to implement the CAP and the VAP. Provide marine employee training records in accordance with B&B Riverboats Company Action plan and vessel action plan to the satisfaction of the attending Coast Guard marine inspector.

Exit sign on first deck did not illuminate. Illumination exit signs are required and must be installed in accordance with Subchapter J. Repair exit sign to the satisfaction of the attending Coast Guard marine inspector.

Thirteen emergency light bulbs were extinguished or missing from the pilot house down to the first deck. Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide safe egress from the space. Repair/renew all extinguished or missing emergency lights to the satisfaction of the attending Coast Guard marine inspector.

MI identified two wasted life jackets in rescue boat. Life jackets must meet approval requirements of 46 CFR1 199.70 Replace wasted life jackets to the satisfaction of the attending Coast Guard inspector.

ICR A-02 of the companies SIP states that the notice to mariners are onboard for the intended operating area. The marine inspector noted no notice to mariner were onboard the vessel. Licensed deck officers are required to acquaint themselves with the latest information published by the Coast Guard and the National Geospatial-Intelligence Agency regarding aids to navigation. Neglect to do so is evidence of neglect of duty. It is desirable that all vessels have available in the pilothouse for convenient reference at all times a file of the applicable Notice to Mariners. Implement ICR A-02 of the companies SIP and provide notice to mariner for the intended operating area to the satisfaction of the Coast Guard marine inspector.

Owner reported a failure of the battery charger for the ships service generator that subsequently reduced power for vessel and loss of steering pump. Operating generators must provide a continuous and uninterrupted source of power for this ships service under normal operational conditions. Conduct repairs to the battery charger for the ships service generator to the satisfaction of the attending Coast Guard Marine Inspector.

During man overboard drill the Belle of Cincinnati's rescue boat engine stalled while underway and failed to start again. Each passenger vessel of less than 500gt must carry one rescue boat. Conduct repairs or replace rescue boat engine.

Rescue boat fire extinguisher was not in its bracket and had expired. Each passenger vessel of less than 500gt must carry at least one rescue boat that is equipped in table 199.175 of this part. Replace fire extinguisher and mount in fire extinguisher bracket.

During fire drill, two fire stations, one forward and second deck and the other located in the engine room, charged at the fire station while not in use. The outlet at each fire hydrant must be provided with a cock or valve fitted in such a position that the firehose may be removed while the fire main is under pressure. Conduct maintenance on the fire stations so that the fire station does not charge while not in use.

Affect repairs as necessary to ensure that oil is not discharged into the navigable waters of the U.S. via the vessel's port main diesel generator exhaust and/or effluent.
