46 CFR 121.420(a)(1) As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible. The vessel lacked onboard navigational charts during the inspection. 16c

The inspections required by HG-500 through HG-540 must be performed by the “Authorized Inspector” as defined in HG-515 of section IV of the ASME Boiler and Pressure Vessel Code (incorporated by reference; see 46 CFR 53.01-1). The Authorized Inspector shall hold a valid commission issued by the National Board of Boiler and Pressure Vessel Inspectors. After installation, heating boilers must be inspected for compliance with this part by a marine inspector. Make boiler ready for inspection. 46 CFR 53.10-3(a), Subchapter F

Carbon dioxide warning signs. Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.” (2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. Engine room and CO2 Cylinder room entrances lacked required markings. Add markings to entrances.

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Control station lacked line diagram. Add line diagram to CO2 control station.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR.” Vessel must label engine room escape hatch.

All electrical installations shall be to the satisfaction of the Officer In Charge, Marine Inspections. Circuit breaker boxes in break room had broken doors, loose coverings, and labeling was not legible. Repair doors, ensure coverings are secured with all bolts, and relabel all circuits.

Vessel must have navigation lights that meet UL 1104. During inspection it was identified that existing lights did not meet the requirement. Vessel must provide approved lights.

Vessel must provide repair proposal for tonnage bulkhead cracked welds and cracks in the plate, and complete repairs prior to credit drydock.

Prove proper operation of power bilge pump in the engine room.

Vessel must get up-dated chart for area of operations.

Repair air intake piping connectors on bow thruster eng.

Starboard side main diesel engine is inoperative. Prove proper operation during sea trail in presence of USCG Marine Inspector, prior to carrying passengers.

Overboard bilge discharge valve frozen in closed position.

Auxillary boiler (heating) shutdown/low water level audible/visual alarm inoperable.

COMPLETE AS-BUILT VERIFICATION OF THE APPROVED PLANS FOR INSTALLATION OF MAIN DECK PASSENGER DOORS WITH COAST GUARD MARINE INSPECTORS.

Provide fire extinguisher inspection record.

Provide Retro-tape on rescue boat as per NVIC 1-87.

Seperate and label adult/child PFD.

Provide fire control plan IAW 46CFR116.530

Provide safety guard on bow thruster shaft in midship compartment.

Provide life saving equipment maintenance schedule and record.
