Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Placard for life jacket model # S225RT is missing, post placards.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly inspections of fire extinguishers were not completed and logged.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacle is constructed of plastic.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Replace stbd side broken light fixture in the lazarette space.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life preservers were not stowed separate from adult life jackets. Ensure child life jackets are stowed separately from adult life jackets.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wire found in port outboard side of Generator. Repair or remove wires.

A submersible electric bilge pump may be used as a power bilge pump required by table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) carrying not more than 49 passengers other than a ferry, provided that: (7) a positive shut off valve is installed at th e hull penetration. The following sea cocks were observed to be seized: Port: Forepeak, Generator Space, Fuel Tank Void, Engine Compartment, Lazarette. STBD: Forepeak, Laz. Additionally the STBD Engine compartment is unable to completely close due to proximity to bilge piping and requires modification to properly close. 46 CFR 182.520 (e)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protected devices must be in satisfactory condition. Existing penetration in STBD forepeak is not permanently capped/terminated. 46 CFR 176.830 (a)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. STBD Engine fuel shutoff valve shielding is wasted and not providing proper shielding. 46 CFR 182.455 (b)(4)

A vessel with a power driven main steering gear must be provided with the following: an independent rudder angle indicator at the operating station. MI observed in port side steering space, an emergency steering station with no rudder angle indicator. Vessel must install means of providing rudder angle indcator at that operating station. 46 CFR 182.610

In systems and appliances where flexible hoses are permitted to be clamped, double hose clamping is required where practible. MI observed bilge dewatering system with flex hoses in both port and starboard steering flat without double clamping. Hoses must be double clamped. 46 CFR 182.410(d)(1)

The construction and arrangement vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull,protection against fire, means of escape in case of sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. On 03MAY2022, vessel reported loss of power/loss of propulsion. Prior to passenger operations, vessel shall submit repair plan for all impacted equipment, provide technician report attesting to operation of impacted machinery, and provide causal analysis report. 46 CFR 177.2100, 701c

Fire Insulation is missing on overhead hatches in port and starboard enginge rooms; insulation is not fastened to overhead in port generator room. 46CFR177.410(c)(4)
