Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Placard for life jacket model # S225RT is missing, post placards.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly inspections of fire extinguishers were not completed and logged.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacle is constructed of plastic.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Replace stbd side broken light fixture in the lazarette space.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life preservers were not stowed separate from adult life jackets. Ensure child life jackets are stowed separately from adult life jackets.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wire found in port outboard side of Generator. Repair or remove wires.

A submersible electric bilge pump may be used as a power bilge pump required by table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) carrying not more than 49 passengers other than a ferry, provided that: (7) a positive shut off valve is installed at th e hull penetration. The following sea cocks were observed to be seized: Port: Forepeak, Generator Space, Fuel Tank Void, Engine Compartment, Lazarette. STBD: Forepeak, Laz. Additionally the STBD Engine compartment is unable to completely close due to proximity to bilge piping and requires modification to properly close. 46 CFR 182.520 (e)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protected devices must be in satisfactory condition. Existing penetration in STBD forepeak is not permanently capped/terminated. 46 CFR 176.830 (a)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. STBD Engine fuel shutoff valve shielding is wasted and not providing proper shielding. 46 CFR 182.455 (b)(4)

A vessel with a power driven main steering gear must be provided with the following: an independent rudder angle indicator at the operating station. MI observed in port side steering space, an emergency steering station with no rudder angle indicator. Vessel must install means of providing rudder angle indcator at that operating station. 46 CFR 182.610

In systems and appliances where flexible hoses are permitted to be clamped, double hose clamping is required where practible. MI observed bilge dewatering system with flex hoses in both port and starboard steering flat without double clamping. Hoses must be double clamped. 46 CFR 182.410(d)(1)

The construction and arrangement vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull,protection against fire, means of escape in case of sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. On 03MAY2022, vessel reported loss of power/loss of propulsion. Prior to passenger operations, vessel shall submit repair plan for all impacted equipment, provide technician report attesting to operation of impacted machinery, and provide causal analysis report. 46 CFR 177.2100, 701c

Fire Insulation is missing on overhead hatches in port and starboard enginge rooms; insulation is not fastened to overhead in port generator room. 46CFR177.410(c)(4)

In accordance with AAWPP vessels stability is reduced to 131 of which 128 may be passengers when operating on a rivers route.

Stbd generator does not shut down when Fireboy system automatically operates. Prove proper operation of stbd generator Fireboy system.

Audible alarm on stbd engine Fireboy system is inoperable. Prove proper operation of audible alarm.

Electrical cables are bundled with fuel hoses in port and stbd generator room. Re-route cables separetely from hoses.

Replace light on magnetic compass.

Provide one red hand flare.

Replace port side raw water intake hose.

Prove proper operation of vessel's steering system and conduct man overboard drill.

Provide training and certification for CPR and Basic First Aid for 50% of the ships crew.

Cleanout console underneath the pilot house controls and remove all trash and all other nonessential equipment.

Provide shields in way of the port & stbd generators and port & stbd main engine fuel shut off valves at the tank IAW 46CFR182.455(a)(4).

Ensure 2nd deck stern rail meets the height requirement in 46CFR179.900. (39.5")

Ensure rail port fwd main deck meets the height requirement in 46CFR177.900.

Install the tank (nozzle) in way of the port and stbd fixed fire extinguishers in the machinery spaces IAW the manufactures instructions (nozzles must face the opposite bulkhead).

Provide corrosion resistant clip in way of ring buoy floating waterlight lanyard.

Mark instructions for opening the emergency escape hatch on the main deck from the inside.

Submit plans for the fire and smoke detection system IAW 46CFR177.202(b)(6)(iii) for review and approval by the local OCMI.

In accordance with 46CFR177.202(b)(5) submit a plan showing the lifesaving equipment locations and installation for review and approval by the local OCMI.

In accordance with 46CFR177.202(b)(3)(iii) submit plans of the ventilation systems onboard the vessel (natural and mechanical) for review and approval by the local OCMI.

In accordance with 46CFR177.202(b)(6)(v) submit plans showing the portable fire extinguishers types, sizes, & locations for review and approval by the local OCMI.

In accordance with 46CFR181.420(a)(4) have the manufacturer's instructions for the pre engineered fixed fire extinguishing system available for review during the COI.

In accordance with 46CFR177.202(b)(10) provide proof of the marine sanitations device is of the approved type.

Submit as built amendments of the following plans for local approval: 1) AC Distribution - Helm. 2) DC Helm Station Encl 6. 3) AC Helm Station Encl 7.

Provide a framed chart of the smoke detector locations IAW 46CFR181.400(c)(2) - 46CFR76.33-20.

The wire for the smoke detector heads from the main deck DC panel room to the detector heads is solid conductor. Replace replace the solid conductor wire with stranded wire IAW 46CFR183.340(b)(1).

Make modifications to the electrical as needed to match the approved plans.

Prove proper operation of the engine control system IAW 46CFR184.620(c). A loss of power to the propulsion control system must not result in an increase in shaft speed.

Provide independent means of for shutting down the propulsion engines IAW 46CFR620(b).

Install fuel shut off valves at the propulsion engines and generators IAW 46CFR182.455(b)(4).

Submit electrical load analysis for review to the local [name withheld] 46CFR177.202(b)(4)(ix).

Mark emergency escape hatch on upper deck "Emergency Escape Keep Clear" IAW 46 CFR 185.606.

Address/repair leaks in main deck windows and overhead.

Provide FCC Safety Certificate onboard vessel.

Mark fire axe with vessel's name.

Mark stairtower leading to upper deck with max passenger capacity in accordance with 46 CFR 185.604(g).

Provide Certificate of Documentation onboard vessel.
