Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship or the efficiency or completeness of its lifesaving appliances or other equipment, the master or owner of the ship shall report at the earliest opportunity to the Administration, the nominated surveyor or recognized organization responsible for issuing the relevant certificate, who shall cause investigations to be initiated to determine whether a survey, as required by regulations 7, 8, 9 or 10, is necessary. Vessel Captain reported reoccurring faulty UPS affecting vessel automation. Vessel not authorized to run a periodically unattended machinery space when UPS is offline. Company is to notify classification society (ABS) and provide a cause and corrective action report.

The company should identify potential emergency shipboard situations, and establish procedures to respond to them. General instructions for fighting fires was not available. Firefighting manual was provided but was incomplete. When asked about general company policies for shipboard boundary cooling, overhaul of spaces, procedures for entering a space that a fixed CO2 system was used, none could be provided. Procedures for a hose team to attack a fire such as direct and indirect attack procedures were not available. Conduct an SMC audit on fire fighting procedures and instructions to be used for the vessel to establish updated procedures to respond to a fire. Complete corrective actions in 90 days.74 SOLAS (24) IX/3.1 ISM Code 8.1 ISM Code 8.2

The company shall establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements the company should ensure that inspections are held at appropriate intervals. During random sampling of parts identified in NSE, 2 of 5 items of the sample contained the incorrect number identified. Conduct an SMC audit on spare parts inventory. Complete corrective actions in 90 days.

The company shall establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements the company should ensure that inspections are held at appropriate intervals. Special Tools required for PSTP's were not calibrated. After review of NSE, special tools requiring calibration were not identified and being tracked. Calibrate required equipment and update SMS to ensure inspections of special tools are held at the appropriate interval.

The conditions of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all aspects will remain fit to proceed to sea without danger to the ship or the persons on board. The life boat and rescue boat's sliding doors are unable to latch close resulting in visible gaps, and not allowing for a weather tight seal.74 SOLAS (24) I/11

A firefighter’s outfit shall consist of an axe to the satisfaction of the Administration. All firefighter’s outfits are not outfitted with and axe as required. 74 SOLAS (97) II-2/ 17.1.1.5

The international shore connection shall be kept aboard the ship together wiht a gasket. No gasket provided for port or starboard international shore connection. 74 SOLAS (97 Cons.) II-2/19.4 16a

Lifebuoys complying with requirements of paragraph 2.1.1 fo the Code shall be so distributed as to be readily available on both sides of the ship. Five (5) lifebuoys were found unservicable with dry-rotted plugs compromising watertightness. 74 SOLAS (2020 Cons.) III/7.1 16a

Fire resistance doors shall as far a practicable be equivalent to that of the division in which they are fitted. Doors required to be self-closing shall not be fitted with hold-back hooks. Class “A” doors in the galley and C deck baggage locker were fitted with hold backs. 74 SOLAS (97) II-2/47

The machinery and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Two main engine expansion joints, 1 flexible joint adjacent to the auxiliary boiler and 1 bolted joint at the exhaust header, are failing and require renewal. 74 SOLAS (97) II-1/26

Stairways and lift trunks which penetrate more than a single deck shall be surrounded by at least A-0 class divisions and be protected by self-closing doors at all levels. Main deck self-closing stairwell door would not full close. 74 SOLAS (97) II-2/46.1

Any required fixed fire detection shall be capable of immediate operation at all times. Smoke detector above lube oil purifier found covered in plastic. 74 SOLAS (97) II-2/13.1.1

Doors provided to ensure the watertight integrity of internal openings which are used while at sea are to be sliding watertight doors capable of being remotely closed from the bridge...Indicators are to be provided at the control position showing whether the doors are open or closed. Open indicator on bridge for shaft alley WTD was inoperable. 74 SOLAS (97) II-1/25-9.2

Vessel shall be equipped with a block diagram and operating instructions for remote steering. No diagram or remote operating procedures were available on the bridge.

09SEP22: The rating of the overload circuit protection shall be indicated. The 24VDC Distribution circuit breaker panel located in the Emergency generator room has 2 breakers with inappropriate ratings. Emergency SWBD control and alarm DC6 is labeled for a 15AT breaker and installed with a 50 amp. Emergency SWBD control and alarm DC7 is labeled for a 50AT breaker and installed with a 15 amp.

Bridge emergency power breaker panel EP102 and main deck lighting panel L102 are not properly labeled for circuits they serve. Each discharge operation shall be recorded in the Garbage Record Book. Between 12JUL21 – 19SEP21, six entries were made where discharges to reception facilities were recorded in the ‘into sea’ column which included plastics and e-wastes. Reception facility receipts corresponded to the quantities and dates where the six incorrect entries were made. Discussed discrepancies with [name withheld] who noted the error and will make entries in the proper column going forward. MARPOL Annex V/9

Before the ship leaves and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Both port and stbd lifeboat kort nozzle fin and attachment bearings are damaged and seized in place.

Fire protection system to ensure required performance to include structural fire protection. Aft inboard upper EDG insulation and sheet metal shall be repaired original.

Condition and equipment shall be maintained to surveyed condition. DG exhaust and bunker line lagging shall be repaired to original.

Before the ship leaves an at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Following an approved temporary repair to correct Item No. 1), Palfinger and ABS require a permanent repair of both port & starboard kort nozzle, kort nozzle attachment points, and installed fin.

Fire-fighting systems and appliances shall be kept in good and working order and readily available for immediate use. Emergency fire pump unable to be started for adequate pressure. 74 SOLAS (2014) II-2/14.2.1.2

Vessels in oceans service shall be fitted with hawsers in general agreement with Standards established by the American Bureau of Shipping. Bow spring mooring line discovered severely deteriorated. 46 CFR 96.07-5(a)

No electrical equipment shall be installed unless the equipment is appropriatey certified for safe usage. Installed paint locker light discovered not intrinsically safe. 74 SOLAS (04) II-1/45.10.3

All access openings in bulkheads shall be permanently and strongly attached to the bulkhead and secured with clamping devices. Latch mechanism on stbd side A-deck door discovered broken. ILLC Part 1, Annex I, Chapter II, Reg 12.1

For all hatchways in position 1 or 2 steel bars or other equivalent means shall be provided in order to efficiently and independently to secure each section of watchway covers. Sliding dogs discovered misaligned for portside Bay #43 and stbdside Bay #35 and bay #37. ILLC Part 1, Annex I, Chapter II, Reg 15.13

After any survey of the ship has been completed, no change shall be made to the structure. Hatch dog for stbd generator one and two emergency escapes discovered loose. ILLC Part 1, Article 15

Complete 100% satisfactory testing of Class approved Periodic Safety Test Procedures (PSTP) and Design Verification Test Procedures (DVTP) in presence of your ACS or Coast Guard Marine Inspectors. Until all issues related to deficiencies noted in the PSTP and/ or DVTP are rectified, the COI endorsement for periodically unattended machinery space operations (PUMS) is suspended. (See attached 835)
