Certain rules in the subchapter require specific approval of equipment or arrangements by the Commandant, OCMI, or CG Marine Safety Center. These approval determinations will be made as a part of the plan review process. The Port side PAM installation was listed on the approved drawing as being placed at frame 60 and 7'10 1/2" from the side shell. The PAM is actually installed at 10' from the side shell, further in board than submitted. Vessel is restricted from departing until both the stability of the vessel and the deck strength assessments are updated and approved by MSC. 46 CFR 170.093 Code 17c

A vessel exposed to salt water more than three months in any 12 month period must undergo a drydock and internal structural examination at least once every two years. Vessel has failed to schedule drydock and internal structural examination within two years of last drydock as listed on the COI. Vessel shall complete credit drydock prior to carrying passengers.

Overcurrent protective devices must be installed so that, a short-circuit on a circuit that is not vital to the propulsion, control, or safety of the vessel does not trip equipment that is vital. The battery charger for the #2 generator’s starting battery system, also provides 24-volt electrical power to the electrical panel for following vital systems: 1) the 24-volt control circuit for starting and emergency shut-down of the #1 and #3 generators; 2) the 24-volt control circuit for the starting and emergency shut-down of both main propulsion engines; and 3) the 24-volt control circuit for the CO2 fixed fire-fighting system. A recent marine casualty involving the short-circuit of the #2 generator’s charging system resulted in a temporary electrical power surge to the power supply for the 24-volt vital system’s power panel, which tripped the main electrical power supply to the vessel. In the event of a shut-down of all engines, electrical power and propulsion would not be able to be restored, in a timely manner.

Repair oil leak on thermal fluid heater media pump, prove proper operation.

Properly repair the fitting on the power take off hydraulic pump.

Install marine grade cable on the emergency generator starting batteries.

Prove proper operation of the OWS system. (Was not inop, crew was not prepared to test.)

Provide completion of servicing and instillation of the engine space and bow thruster room CO2 fixed systems.

Apply retro reflective material to the rescue boat IAW 46 CFR 199.176.

Resubmit Fire plan to MSO SF Bay for review and approval.

Engine room CO2 siren did not sound during initial and subsequent test. Prove proper operation of siren.

Swing magnetic compass and verify proper heading consistant with gyro compass.

PROVIDE ADEQUATE EMERGENCY LIGHTING IN WAY OF EMERGENCY STEERING STATION.

SUBMIT PLANS FOR INSTALLATION OF PERMANENT (NON-WATERTIGHT) SPLASH SHIELD BENEATH MAIN SWITCHBOARD.

POVIDE ABS OR CG APPROVED PLANS FOR: MSD TNK; HEATING COIL MOD'S; & BILGE KEELS.

PROVIDE 10 ADDITIONAL EXPOSURE SUITS (W/CURRENT LIGHTS). RESTRICTED TO NMT28 PERSONS ON BOARD.

RENEW CHEM. LIGHTS FOR PFD'S (W/EXP DATE OF JAN95).

CORRECT MAGNETIC COMPASS ERROR AND PROVIDE NEW DEVIATION CARD.

PROVIDE 2 FULLY EQUIPED EMERGENCY GEAR LOCKERS IN SEPERATE LOCATIONS.

UPDATE FIRE CONTROL PLAN.

PROVIDE LOW LUBE OIL PRESSURE ALARM FOR BOW THRUSTER ENGINE.

CLEAN OUT OIL IN MACHINERY SPACE BILGES.

PROVIDE DOOR STOP FOR WTD BETWEEN ER AND STEERING FLAT.

PROPERLY ADJUST MDE'S OVERSPEEDS AND AIR DAMPERS, RPM IS LOWER THAN THAT INTECH MANUAL.

PROVE PROPER SETTING OF SAFETY VALVES FOR PRESSURE VESSELS.

MOVE SAFETY VALVES FOR PRESSURE VESSELS #163168&163173 TO LOCATIONS DIRECTLY ADJACENT TO PRESSURE VESSELS.

PROVE PROPER OPERATION OF OWS.

SECURE CARGO TANK VENT VALVES IN THE OPEN POSITION AND MAKE PERMINANT MODIFICATION TO VENTING CARGO IAW 46CFR32.55-20(C) AND APPROVED PLAND FOR P/V VALVES. VSL IS AUTHORIZED TO CARRY GRADES D AND BELOW UNTIL MODIFICATIONS ARE COMPLETED.

PROPERLY MARK EMERGENCY ALARM BELL IN STEERING FLAT.

PROVIDE 4 IMMERSION SUITS (INCLUDING 1 JUMBO), VESSEL IS RESTRICTED TO 12 PERSONS IN ADDITION TO CREW & SUITS MUST FIT ALL PERSONS ONBOARD.

REPLACE BATTERY & PROVE OPERATION OF MAIN DECK E-LIGHTING.

REPAIR SOUND POWERED PHONE (BRIDGE TO STEERING GEAR ROOM).

PROVIDE EXPLOSION PROOF JUNCTION BOX AT A.C. SUPPLY TO TRANSREC.

CONDUCT FIRE AND BOAT DRILLS WITH ASSIGNED VESSEL CREW PRIOR TO SAILING.
