Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. COD on board vessel was expired. Provide valid COD to the satisfaction of attending Marine Inspector. 46 CFR 67.7

Fire pumps must be fitted with a pressure gage on the discharge side of the pumps. Vessel fire pump pressure gage inoperative. Replace fire pump pressure gage with operational gage. 46 CFR 95.10-5 (e)

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel of the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and the vessel complies in all respects with the regulations. Number three generator shutdown and tripped the circuit breakers for all three generators causing a loss of electrical power to the vessel. 46 CFR 91.30-1(a).

At each inspection for certification the inspector will determine that all fire-extinguishing equipment is in suitable condition and may require any tests necessary to determine the condition of the equipment. One pressure switch serving the thermal fluid heater space is missing the cap on top of the plunger, replace. 46 CFR 91.25-20

Efficient guard rails or bulwarks must be fitted on all exposed parts of the freeboard and superstructure decks. Stern portible railings were found to be made of non-metallic materials. Install metallic railings in accordance with load line assignmnet form.

Simple, complete operating instructions must be conspicuously located at or near any release station and in the extinguishing agent cylinder storage room. Change all fixed CO2 extinguishing system instructions to say "CLOSE ALL DOORS, HATCHES, AND VENTILATION CLOSURES TO THE PROTECTED SAPCE."

All pressure vessels other than unfired steam boilers shall be protected by pressure-relieving devices that will prevent the pressure from rising more than 10 percent above the maximum allowable working pressure. Pressure relieving device found to be isolated by shut-off valve.

Motor shaft for aft port side cargo crane winch motor found damaged and the crane is inoperable. Cargo gear survey must be conducted by an approved surveyor. Replace crane winch motor shaft and provide OCMI with valid cargo gear survey report. The Coast Guard considers cargo gear to include gear forming part of a vessel’s equipment which is used for the handling of cargo other than bulk liquids, but does not include gear which is used only for handling or holding hoses, handling ships’ stores, or handling the gangway or boom conveyor belt (see 29 CFR 1919.2(b)). While not specifically subject to inspection at a regular interval, other gear or lifting appliances that are not considered cargo gear by the above definition should be maintained in good working order. 46 CFR 91.25-25 (b)

Primary bilge pump in the engine room found inoperable due to a faulty gasket. Replace gasket and prove proper operation of the pump. 46 CFR 96.03-1 and 46 CFR 56.50-50

Load Line deck mark on port and starboard side is not painted in a contrasting color to the hull. Paint deck mark in a contrasting color and contact USCG or ABS surveyor for re-inspection. 46 CFR 42.13-40

Stability book states all freeing ports shall remain unobstructed. Multiple freeing ports on the port and starboard side main deck found with rubber covers over entire freeing port. Remove or resubmit stability information .

Structural fire protection must comply with 46 CFR Subchapter I. Multiple electrical penetrations in steel bulkheads on the bridge, galley pantry, corridor bulkheads, and stairtowers found unsealed and do not meet structural fire protection. Provide proof of structural fire protection IAW subchapter I.

Provide satisfactory fire damper test for fire dampers that may damage fusible links when tested. Remove heavy lifting wire strap attached to port aft engine room damper in dry dock. Damper is unable to fully close.

Each cargo vessel must carry at least one rescue boat approved under the Coast Guard approval series 160.156. The installation of a workboat in place of an approved rescue boat was noted. Provide an approved rescue boat or prove an equivalent effectiveness of the installed work boat by requesting a determination of an equivalent level of safety IAW 46 CFR 199.09.
