No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. The vessel's starboard engine flex coupling failed rendering the engine inoperable. Your vessel is prohibited from transiting any port within the Sector Honolulu COTP Zone without the assistance of a towing vessel of adequate horsepower and maneuverability. The towing vessel shall be under the direction and control of an individual holding a Merchant Marine Credential (MMC) authorizing them to engage in assistance towing under the provisions of 46 CFR Part 11.482. Your vessel is prohibited from departing any port within the Sector Honolulu COTP Zone until satisfactory repair of the vessel is completed, and the proper operation of the propulsion system is demonstrated to the satisfaction of the attending Marine Inspector. 46CFR91.45-1(a)

The liferafts must be supplemented with additional liferafts to bring the total capacity of the liferafts available on each side of the vessel to at least 100 percent of the total number of persons on board.

Each vessel subject to load line requirements shall carry a valid certificate. LL11D is not updated with to match hatch modifications.

Ring buoys on bridge should, when released, fall to the water directly into the water without striking any part of the vessel. Prove the Bridge buoys can be released and clear the vessel.

Four service lines, cleaning brush, and instructions are required for a line throwing appliance. Kit is incomplete IAW CFR, bring line throwing kit to compliance. 18FEB26- 04 New Impulse onboard missing the 160.040-4 02 Buoyant type and 199.170(c)(2) 1500 ft of line. Only 600 ft onboard.

Electric installations on vessels must ensure protection of crew from electrical hazards. Control switch in the bow thruster escape trunk was open exposing personnel to electrical hazard.

Marine Inspector shall test machinery to check safe operation. During PSTP testing of the port M/E the high RMP Low oil pressure should alarm at 40 PSI, didn’t alarm until 20 which is the setting for low RPM.

All machinery shall be provided with adequate guards or covers. The OWS pump motor missing junction cover and fan guard.

It is the duty of the Chief Engineer when assuming charge of machinery of a vessel to examine thoroughly if any items are bad they shall be reported to the Master [name withheld] on the #3 generator leaking oil.

Gages on pressure vessels shall be 1.2 but not more than 2 times the setting of the PRV. Valves set to 275 psi gages for the center and port air receivers are 0- 600 psi.

Oil strainers and other equipment subject to normal leakage must be fitted with drip pans. Fuel oil strainers in the engine room did have drip pans.

Where fire extinguishing equipment is installed but not required it must meet the requirements of this part. Galley window auto closure for control of the spread of a fire is not functioning.

A fixed CO2 or other approved system must be installed in all paint lockers. Large paint locker did not have CO2 system installed.

Instructions and illustrations must be posted with information on the donning of life jackets. Additional life jackets did not have donning instructions posted for multiple types onboard.

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. Vessel informed OCMI that there is a pinhole leak in the slop tank leading to water ingress into the engine room.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Marine Inspector observed the stairwell landing on the 01 Deck is required to meet structural fire protection standards, however a new electrical cable installed through the multi-cable transit was not properly fitted; compromising the fire protection integrity of the space. A plan on how to complete this repair is to be developed and submitted within 30 days. 46 CFR 92.07-10(d)(2)

The following emergency lights must be supplied from a distribution panel on the navigating bridge: Emergency lights on the navigating bridge. Marine Inspector observed one emergency light not working in the navigating bridge.46 CFR 112.43-7(a)(4)(ii)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Marine Inspectors observed that the port side rescue boat was able to launch and maneuver approximately 50 yards away from the vessel before the engine on the rescue boat became inoperable. Effect permanent repairs to the effected equipment to the satisfaction of the attending US Coast Guard Inspectors.

If two or more voice communication stations are near each other, there must be a means that indicate the station called. Marine Inspectors attempted to contact one sound powered phone station on the bridge from another sound powered phone station on the bridge, and could not clearly hear the audible indicator emitting from the sound powered phone being hailed. Effect permanent repairs to all of the effected sound powered phone stations through out the vessel exhibiting the same issue to the satisfaction of the attending US Coast Guard Inspector.

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries, and storerooms, other than small service lockers, shall be of "A" Class construction. Marine Inspectors observed a 3 FT. by 3 FT. panel of insulation had been removed from the bulkhead in order to conduct maintenance. Effect permanent repairs to the "A" Class boundary to the satisfaction of the US Coast Guard attending Marine Inspectors.

The spring in a relief valve for pressures greater than 250 psi, the spring shall not be reset for any pressure more than 5 percent above that for which the valve is marked. Marine Inspectors were unable to verify proper operation of 2 relief valves on air receivers. The valves were marked for 275psi and did not relieve when the gauge read over 285 psi. Operator replaced relief valves and corrected the issue on the spot.
