Vessel reported a lube oil leak originating from the port side shaft seal. SOLAS (20) CH.I/11(a). 17ac.

Automatic sprinkler, fire detection and fire alarm systems…The pump and piping system shall be capable of maintaining the necessary pressure at the level of the highest sprinkler to ensure a continuous output of water sufficient for the simultaneous coverage of a minimum area of 280 m2 at the application rate specified in paragraph 3. When testing of the automatic sprinkler system PSCOs found only 3 pumps were operable. Pump 4 is currently under a condition of class. Only 3 of the 5 pumps are operational bringing into question the reliability of the system. 74 SOLAS 97 II-2/12

Crew was unable to test OWS when asked by PSCOs due to a failed suction pump rendering the system inoperable. MARPOL 2017 Annex I / 14.2

The construction of all doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. PSCOs observed fire door 640 (starboard side main galley), when in the closed position, had an approximate 15 millimeter gap which would allow the passage of smoke and flame between the top and sides of the door and frame. 74 SOLAS 97 II-2/30.2
