At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. An accumulation of oil with a potential to create a fire hazard was noted in the bilge below the starboard engine of the machinery space.

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he or she deems necessary to be assured of the safety and seaworthiness of the vessel. The deck flammable storage locker was not secured as to prevent it from moving or falling.

Employers must ensure that they and their crewmembers meet the requirements of this part (46 CFR 16). While conducting a DAPI spot check, vessel representatives were unable to provide the most recent MIS Report and employee list.

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. At least one lifebuoy on each side of the vessel must be fitted with a buoyant lifeline that is at least as long as twice the height where it is stowed above the waterline with the vessel in its lightest seagoing condition, or 100 feet in length, whichever is the greater. Additionally, at least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light. Each buoyant lifeline associated with a lifebuoy was found to be less than 100 feet in length. The lanyards attaching self-igniting lights to two of the vessel’s lifebuoys were found to be deteriorated and the light on the port lifebuoy was inoperable.

Onboard training, musters, and drills must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Every crewmember must be given instructions that include, but are not limited to, the problems of hypothermia, first aid treatment for hypothermia, and other appropriate first aid procedures. The vessel’s first aid equipment, required for the training/instruction in subchapter W, was found to have expired medicine.

All lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Each liferaft must be stowed as follows: Each liferaft must have a painter system providing a connection point between the vessel and the liferaft. The painter line was found to be disconnected from the hydrostatic release unit assembly.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. Lights were found in need of battery renewal.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Vessel did not have sea painter for IBA properly attached to the hydrostatic release.

Clean and make pressure vessel data placard legible.

Demonstrate proper operation of relief valve on pressure vessel.

Make pressure vessel ready for internal inspection.

Reweld cracked weld on the Port rake Chord as marked.

Repair Hole in deck, stbd side Rake fwd.

Repair cracked (2) stanchions for handrail located on starboard quarter.

Plug holes in Engine Room forward bulkhead to creat vapor tight barrier.

Provide 30 inch long lines for float lights on ring buoys.

Remove or isolate exposed electrical dead-ends in engine room.

Install bonding cables on vapor piping that is isolated from the vessel hull as per 46 CFR 39.20.

Provide non-conducting deck covering in front of electrical distribution panel in machinery space.

Remove 5 wasted cargo tank ullage dome tops and renew.

Crop out and renew wasted cargo deck plating where noted.

Make all cargo tank internals available for inspection.

Clad weld isolated pitting on cargo deck plate where noted.

Provide Oil Record Book per 33 CFR 151.25 LEGACY COMPONENT DESC: Oil Record Book

Provide plans for hydraulic, electrical, and other systems involved w/ replacement of hydraulic system.

Provide acid resistant battery box for genset starting battery. Battery was merely strapped to genset.

Seal all holes in the forward bulkhead of the machinery space so that it is gastight.
