Ships shall be designed, constructed and maintained in compliance with the structural requirements of a classification society which is recognized by the Administration in accordance with the provisions of regulations XI-1/1. The crew of the vessel observed water coming from the top of the #3B port double bottom ballast tank during the vessel's last voyage from Honolulu to Oakland. On further investigation, a hole approximately 3 inches long was found in cargo hold number 3 on the portside near the bilge well. It is on the lower level of this two level tank.

An automatic radar plotting aid (ARPA) shall be fitted on ships constructed before September 1, 1984, that are not tankers, if of 15,000 gross tonnage and upwards, by September 1, 1988. [name withheld] reported all three radars are not receiving gyro input signals and the ARPA is not operating as originally designed and approved by the Administration. The Master's standing orders require the Officer in Charge of the Navigation Watch (OINW) to manually input the gyro heading in the ARPA for overall system functionality. Provide a technicians service report to attest to the repair and functionality of the vessel's ARPA as originally designed and approved by the Administration.
