The construction and the means for securing the weathertightness of cargo and other hatchways in positions 1 and 2 shall be at least equivalent to the requirements of §§ 42.15–25 and 42.15–30. The cargo tank top sight hatch on CC #3 port side was found with a wasted gasket. 46 CFR 42.15-20(a)

At positions 1 and 2 the height above the deck of hatchway coamings fitted with weathertight hatch covers of steel or other equivalent material fitted with gaskets and clamping devices shall be as specified in § 42.15–25(a)(1). The aft rake hatch knife edge was found corroded and will not create a proper edge seal. 46 CFR 42.15-30

The transfer procedures required by § 155.720 must contain, either in the order listed or by use of a cross-reference index page: A description of each transfer system on the vessel including a line diagram of the vessel's transfer piping, including the location of each valve, pump, control device, vent, and overflow;Procedures for operating the emergency shutdown and communications means required by §§ 155.780 and 155.785, respectively; and Procedures for topping off tanks. The emergency cargo pump shutdown procedure found in the transfer procedures is not labeled at the remote shutdown location. Crew members could not demonstrate the shutdown procedure at the remote location IAW the transfer procedures. Provide labeling at the remote shutdown location to assist crew with the procedure. 33 CFR 155.720

Boilers, main and auxiliary machinery, and piping systems shall conform to the requirements of subchapter F (Marine Engineering) of this chapter, except as otherwise provided for in this subchapter. 46 CFR § 32.35-1 Exhaust pipe installations must conform to the requirements of ABYC P-1 and Chapter 6 of NFPA 302 (both incorporated by reference; see § 58.03-1) and the following additional requirements. The flame arrestors on the dry exhaust for the port and stbd cargo pumps were found corroded or missing. 46 CFR 58.10-10

A ship of 100 gross tons or more constructed before July 1, 1974, and a ship of 100 or more but less than 300 gross tons constructed after June 30, 1974 must: (1) Meet paragraph (a)(1) of this section. (a) A ship of 300 gross tons or more constructed after June 30, 1974 must have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe, that: (1) For a ship of 300 or more but less than 1600 gross tons has a capacity of at least one-half barrel; The containment for the CC #3 tank vent cannot be drained and will not meet the capacity if it is full of rain water. 33 CFR § 155.320

Unmanned barges must meet light and sound signal applicances in 33 CFR 82 and 33 CFR 83. Prove operation of the navigation lights. 33 CFR 83

The port side gooseneck for #3 void space was found with a corroded gasket. 46 CFR 42.15-30

Each vessel subject to load line requirements shall carry on board a valid certificate attesting to the compliance with such requirements. USCG MSU Saipan received a notice of automatic suspension of Class from the load line certificate issuing authority, RINA. The notification states that statutory certificates issued by RINA automatically became invalid as from the date of suspension of Class as from March 20, 2026. The vessel's load line certificate is no longer valid. The vessel is restricted from movement and cargo operations until a valid load line certificate is on board. 46 CFR § 42.09-1(b)

Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station, conspicuously marked, and located at the midpoint of such vessel, or 100 feet from the engine, whichever is the more practical. The remote quick acting manual shutdown shall be installed on the engine so as to provide a quick and effective means of stopping the engine (such as by cutting off the intake air). During the COI Inspection, a designated Person-In-Charge was not on board. Prior to cargo operations, prove proper operation of the internal combustion engine driven cargo pump, alarms, and shutdowns to the satisfaction of the attending Marine Inspector.

Cargo spaces used in determining volume (Vc) for calculating net tonnage must be permanently marked with the letters “CC” (cargo compartment) which are at least four inches in height and positioned so as to be visible at all times. "CC" was not found permanently marked on each cargo space hatch. 33 CFR 156.120(j) 46 CFR 69.67

No person shall conduct an oil or hazardous material transfer operation unless: Each hose or loading arm in use meets §§ 154.500 and 154.510 of this chapter, respectively. IAW 33 CFR 154.500(g), the hose burst pressure and the pressure used for the test required by 33 CFR 156.170 must not be marked on the hose and must be recorded elsewhere at the facility as described in paragraph (f) of this section. The test pressure was found stenciled on the vessel's oil transfer piping. Each hose must be marked with one of the following: (1)The name of each product for which the hose may be used; or (2) For oil products, the words “OIL SERVICE”; or (3) For hazardous materials, the words “HAZMAT SERVICE. The product name or "oil service" was not found marked on the transfer piping.

As per 33 CFR 155.715, the letter referenced in § 155.710(e)(1)(ii) must designate the holder as a person-in-charge of the transfer of fuel oil and state that the holder has received sufficient formal instruction from the operator or agent of the vessel to ensure his or her ability to safely and adequately carry out the duties and responsibilities of the PIC described in 33 CFR 156.120 and 156.150. The company did not provide a designation letter for the PIC currently serving onboard. 33 CFR 155.710(e)(1)(ii)

A line diagram of the vessel's transfer piping was not in the transfer procedures. The emergency shutdown procedure found in the transfer procedures does not match the actual procedure to shut down the cargo pump. The topping off procedure does not match the PIC's description of the topping off procedure used. 33 CFR 155.720

Side scuttles to spaces below the freeboard deck or to spaces within enclosed superstructures shall be fitted with efficient hinged inside deadlights arranged so that they can be effectively closed and secured watertight. The machinery space port hole located on the port side will not close due to a wasted gasket material and uneven closing devices. 46 CFR 42.15-65

The installation of all electrical engineering; with the details of design, construction, and installation, must meet the requirements of sub J. Welding cable found on the starting conductors for the cargo pump. The batteries are not secured for movement in all directions. 46 CFR 32.45-1 46 CFR 111.15-5 46 CFR 111.60-1

Missing retro-reflector tape on ring buoy. Install retro-reflector tape.

Damaged drain valves on FWD most pipe connection containment. Replace drain valve.

Repair damaged FWD deck hand rails.

Secure drain plugs for containment on cargo deck.

Unable to conduct operational test of the cargo pump's prime movers due to low charge on batteries.

Found two extinguished NAV lights on mast.

Found numerous severely wasted fasteners on manhole covers to voids (port and starboard voids).

VSL shall have a valid Cargo Ship Safety Construction Certificate on board.

All cargo tank hatches must be clearly marked in not less than three inch lettering " DANGER - KEEP OUT".

All ships of 400 gross tons and above shall be survey and issue a valid International Air Pollution Certificate.

Holed bellow on starboard prime mover exhaust. Temporary repair completed by shorside support in Guam pending new bellow to replace as inkind.

Installed prime mover for cargo pump must be suitable for intended services. Controler for starboard pump is not oeprational. Repair engine controls and show proper control oeprations to attending MI.

Repair damaged deck rail aft of the corgo pump room.

Existing pressure gauges have expired calibartion dates (01FEB2019). Provide up-to-date proof of complete calibration on all cargo pipe gauges.

Grab line must be in serviceable condition as required by 46 CFR 160.050-4. Replaced deteriorated grab line on (03) ring life buoy IAW manufacture instructions.

Hose must be hydrostatic test annually as required by 33 CFR 155.310. Conduct hydrostatic test to be witnessed by CG Marine Inspector.

Cargo piping must be hydrostatic test as required by 33CFR155.310. Prepare cargo pipes for hydrostatic test.

Replace all expired items in first aid kit

Port running light is out of position due to collision with TUG Mangilao. Repair damage support structure for port running light IAW 72 COLREGS.

Missing forward deck railing. Replace as original.

Remove oil boom stored on top of the cargo tank.

Mechanical means of closing each drain and scupper in the enclosed deck area as required by 33 CFR 155.310. Install captive chain to the drain plug.

Fire control plan is not current with equipment location on board. Update FCP.

Replace deteriorated gasket on STBD #2 cargo tank hatch cover.

Install valve opening and closing indicator.

Stern railing post are not permanently attached to deck. Make permanent repairs for railing post.

Junction box for emergency stop is not gastight, watertight or explosion proof. Junction box must meet requirements in 46CFR111.81-1

An additional permanent structure forward of cargo pump room was added after Class RINA and MSC approved plans and issued stability letter. Verify stability calculation and submit updated general arrangement drawings to MSC and Class for approval.

Discrepancy on actual cargo tanks capacity. Approved cargo tank plan has total cargo tank capacity of 6101 barrels and stability booklet with a total cargo tank capacity of 6377.87 barrels. Verify actual cargo tank capacity and submit updated tank capacity plan and stability calculations to Class RINA and CG MSC for approval.

Batteries are stored in cargo handling room. Relocate batteries in accordance with 46CFR111.105-29.

Per 46 CFR 32.35 main and auxiliary machinery for Tank Barges shall meet requirements of 46 CFR 58.05-1 and 46 CFR 58.10-5 (a), (c) & (d). Provide documentation proving that auxiliary diesel engines meet requirements of marine type and are designed and constructed in conformance with this subchapter.

Diesel fuel tank for prime mover must have shutoff valve locate at the tank. 46CFR56.50-60.

Cargo pump has installed relief valve that discharges directly into cargo pump room. Install return line IAW approved piping arrangement plan ref RINA letter SHMC/2014/00379/LHG which states that delivery side of cargo pumps shall be fitted with relief valves leading back to their suction. 46CFR56.50-20

Per 46 CFR 58.05-1 Material, design and construction must be review by classification society (C/S) or OCMI. Provide quadruplicate copies of drawings and particulars for auxiliary machinery to C/S or OCMI for review.

Per 46 CFR 110.25 - Electrical plan must be submitted for Coast Guard review. Provide three copies of electrical schematic plans and particulars to OCMI for review.

Per 46 CFR39.20-3: Show proper operation of the cargo tank high level indicating devices.
