Whenever a vessel is deprived of the service of a member of its complement, and the master or person in charge is unable to find appropriate credentialed personnel to man the vessel […] a report of sailing short must be filed with the OCMI having cognizance for inspection in the area in which the vessel is operating, or the OCMI within whose jurisdiction the voyage is completed. Vessel master/rep made sailing short notification for the position of AB from Sector New York to Sector Jacksonville. Notify local OCMI once position has been filled.

The condition of the ship and its equipment shall be maintained to conform with the provision of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Vessel made alterations to fuel transferring system by removing hydraulic actuated valve and causing personnel to enter confined space for every transfer operations also creating a down-flooding point. Vessel is to provide an alternate mean of operation not involving personnel entering the confined space to transfer fuel.

SOLAS 74 (2004) I/11: The condition of the ship and its equipment shall be maintained to conform with the provisions of the present condition to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The port side anchor chain wash remote valve was found broken and inoperable.

SOLAS 74 (2004) 11-2/13.1: The purpose of this regulation is to provide means of escape to that persons on board can safely and swiftly escape to the lifeboat and life raft embarkation deck. The port and starboard helicopter hangar doors were seized closed.

Found multiple through deck penetrations previously used for cable runs in cargo room 1D converted to a gymnasium, not properly sealed to prevent the spreading of smoke and fire.

MARPOL I/7.1: The Government of a Party to the present Convention may, at the request of the Administration, cause a ship to be surveyed and, if satisfied that the provisions of this Annex are complied with, shall issue or authorize the issue of an International Oil Pollution Prevention Certificate to the ship and, where appropriate, endorse or authorize the endorsement of that certificate on the ship in accordance with this Annex. Update IOPP Form A to reflect correct OWS installed, OWS throughput and account for correct tanks being sounded.

SOLAS 74 (1997) II-2/6.1: All fire extinguishers shall be of approved types and design. Found numerous portable fire extinguishers throughout the ship with missing nozzle clips for proper storage on extinguisher bracket.

SOLAS 74 (2004 Amend.) II-1/32.2: Each oil-fired boiler which is intended to operate without manual supervision shall have safety arrangements which shut off the fuel supply and give an alarm in the case of low water level, air supply failure or flame failure. Prove operation of the # 2 auxiliary boiler loss of combustion air alarm and shutdown, safety arrangement is currently not functional.

Make repairs to the misaligned rubber couplings on the # 1 and # 2 main ship service generators to alleviate the current strain on the installed couplings.

Make repairs to the excessive leak on the emergency diesel generators jacket water heaters circulation pump.

SOLAS 74 (2004 Amend.) II-1/45.1.3: All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched. Secure dead ended wires associated with the #1 hot water recirculation pump located in the engine room pump located in the engine room boiler space.

SOLAS 74 (2004 Amend.) II-1/25-9.1: The number of openings in watertight subdivisions is to be kept to a minimum compatible with the design and proper working of the ship. Where penetrations of watertight bulkheads and internal decks are necessary for access, piping, ventilation, electrical cables, etc., arrangements are to be made to maintain the watertight integrity. Make adjustments to the sliding watertight door leading into propulsion shaft alley to ensure full closure as designed.

SOLAS 74 (2004 Amend.) II-1/25-9.1: The number of openings in watertight subdivisions is to be kept to a minimum compatible with the design and proper working of the ship. Where penetrations of watertight bulkheads and internal decks are necessary for access, piping, ventilation, electrical cables, etc., arrangements are to be maintain the watertight integrity. Conduct preventive maintenance to draft reading tube in shaft alley to prevent potential ingress of water.

SOLAS 74 (2004 Amend.) III/6.4.2: A general emergency alarm system complying with the requirements of regulation 50 shall be provided and shall be used for summoning passengers and crew to muster stations and to initiate the actions included in the muster list. The system shall be supplemented by either a public address system or other suitable means of communication. Repair general alarm light in the shaft alley to also rotate when illuminated.

SOLAS 74 (2004 Amend.) II-1/45.1.3: All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Hold #4 panel, EP5A, Emergency lighting panel, EL 115, FDR EL100-11 and Engine room seawater panel, EP-12B need covers installed where breakers are missing to prevent electrical injury.

46 CFR 61.40-10: Vessel's Periodic Testing of Vital System Automation is lacking the procedures to adequately perform the Periodic Safety Tests Procedures (PSTP) required by 46 CFR 61.40-6 and 46 CFR 61.40-10. The current version of the vessel’s PSTP’s fail to address several of the systems required by the regulations.

SOLAS 74 (2014 Amend.) V/19.2.5.2: All ships of 500 gross tonnage and upwards shall, in addition to meeting the requirements of paragraph 2.3 with the exception of paragraphs 2.3.3 and 2.3.5, and the requirements of paragraph 2.4, have: a gyro compass heading repeater, or other means, to supply heading information visually at the emergency steering position if provided. Steering gear local gyro compass repeater is not visible from the local starboard steering pump control station.

SOLAS 74(1997) III/19.2: Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Lifeboats no. 1, 2, 3, and 4 were found with excessive oil and fuel in the engine compartment bilge.

SOLAS 74 (2004)II-I/20-4.1: All transverse or longitudinal blkhds which are taken into account as effective to confine the seawater accumulated on the ro-ro deck shall be in place and secured before the ship leaves the berth and remain in place and secured until the ship leaves the berth and remain in place and secured until the ship is at its next berth. The stbd side ventilation's access covers in cargo hold 1A were not secured and missing fasteners.

SOLAS 74(2004)II-I/45.1.3: All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Found dead end wires in overhead in passage way by the crew laundry room.

SOLAS 74(1997)II-2/12.3: Sprinklers shall be placed in an overhead position and spaced in a suitable pattern to maintain an average application rate of not less than 5L/m2/min over the nominal area covered by the sprinklers. Found damaged and missing sprinkler heads stbd side 02 deck, in way of JP-5 equipment room, and in way of free fall lifeboat.

SOLAS 74 (2004 Amend.)II-2/7.1.1: The purpose of this regulation is to detect a fire in the space of origin and to provide for alarm for safe escape and fire-fighting activity. For this purpose, the following functional requirements shall be met: fixed fire detection and fire alarm system installations shall be suitable for the nature of teh space, fire growth potential and potential generation of smoke and gases. Secure overhead smoke detector in the cargo hold just outside of the steering gear compartment.

Fixed fire detection and fire alarm system installations shall be suitable for the nature of the space, fire growth potential and potential generation of smoke and gases. The company should establish procedures to ensure that thw ship is maintained in comformity with the provisions of the relevant rules and regulations and withany additional requirements whichmay be established by the company.

Conduct ISM-SMC Intermediate Verification in the presence of USCG Marine Inspectors. The following ISM observations were identified during the inspection- A) M/E exhaust leakage was identified during OCI (MSC) inspection in November 2017. Crew began wearing dust masks & CO alarming meters. SAMMs VRR showed request for required meter shore-side calibration open 04FEB2016, calibration not completed. SMS also requires notification of hazardous conditions to be submitted to company. No notification submitted. B) All BW valves converted in 2016 during previous dry-dock. Numerous valves installed incorrectly or malfunctioning since install. No VRR submitted for correction until 2018. C) WTD 3rd party report from Apr 2017 indicated numerous failures of WTDs. 09 months have elapsed since initial report. D) Vessel crew is hindered by inabilities to access system for SMS repair work (SWFL/SAMM). Minimal crew have access, which increases time for others to input repair items. Possible STCW work/rest conflicts may occur without additional crew w/access to SMS repair system. ISM Code Part A Regs 10.2 & 6.2.2

Ballast tank levels in Deep 7-160-2, DB 8- 227(19) and BW-1B16 (converted gray water) can not be maintained. Possible ballast valve leak by occuring. Identify failures and repair.

Controls for WTD's throughout vessel have been identified with PLC failures. Indicator panel on bridge fails to indicate proper open/closed status. WTD to shaft tunnel is missing manual pump handle. WTD D-227(55) alarm does not work. 74 SOLAS (UnAmend) II-1/13(i)(i)

M/E exhaust leaking CO into engine room. Crew wearing uncalibrated CO meters for safety

Identified crew member in AB postion with medical certificate stating "not valid for service as deck officer/rating." Member switched to OS position.

A CO2 nozzle is missing from the high- pressure CO2 line in purifier room overhead

The 12" salt water cooling line located on the STBD side,lower level of the Engine Room has multiple leaks.

Oil is leaking from the overhead onto the generator side of the #3 MDG, presenting a fire hazard.

There is excessive oil leaking from the explosion cover for the #6 cylinder on #1 MDG.

Jacket water was leaking from the inboard side of the #6 cylinder on the #1 MDG.

OWS appears to be a mechanical issue and believe the issue is regarding the bearing. ABS has issued a OSR to be rectified 21NOV17. Meantime, vessel has sludge retention via holding tank. When surveyor attends he will ensure record book is update and discharge valve is locked to prevent inadvertent discharge to sea.

ECDIS Failure due to electronic malfunction on the main and secondary display. Functionality remains due to back up arrangements and paper charts. ECDIS failure will be rectified through a technician prior to 21NOV17 date. ABS surveyor will review tech report and ensure satisfactory update of ECDIS prior to departure.

Lifeboats #1 & #2, water ingress of deck/seat foam core. Delamination of exterior shell of #2. Crack & delamination of #1 on port quarter. Repair lifeboats in accordance with Lifeboat Mfg. specs iaw 46 CFR 160.135-7

Vessel currently undering going automation upgrades on vital valves to engineering plant. The overhaul requires full manning. Vessel is not approved for Periodically Unattended Machinery Space (PUMS) operations until Classification Society (ABS) re-issues Class Certificate with ACCU designation and confirms proper operation of the automated control, monitoring, and management system. 46 CFR 62.50
