Vessel made alterations to fuel transferring system by removing hydraulic actuated valve and causing personnel to enter confined space for every transfer operations also creating a down-flooding point. Vessel is to provide an alternate mean of operation not involving personnel entering the confined space to transfer fuel.

The following ISM observations were identified during the inspection- A) M/E exhaust leakage was identified during OCI (MSC) inspection in November 2017. Crew began wearing dust masks & CO alarming meters. SAMMs VRR showed request for required meter shore-side calibration open 04FEB2016, calibration not completed. SMS also requires notification of hazardous conditions to be submitted to company. No notification submitted. B) All BW valves converted in 2016 during previous dry-dock. Numerous valves installed incorrectly or malfunctioning since install. No VRR submitted for correction until 2018. C) WTD 3rd party report from Apr 2017 indicated numerous failures of WTDs. 09 months have elapsed since initial report. D) Vessel crew is hindered by inabilities to access system for SMS repair work (SWFL/SAMM). Minimal crew have access, which increases time for others to input repair items.
