All ships of 150 gross tonnage and upwards shall be fitted with: a spare magnetic compass as referred to in paragraph 2.1.1, or other means to perform the function referred to in paragraph 2.1.1 by means of replacement or duplicate equipment. PSCO identified vessel did not have a spare magnetic compass. 74SOLAS20 V/19.2.2.1

All ships of 500 gross tonnage and upwards shall have: rudder, propeller, thrust, pitch and operational mode indicators... all to be readable from the conning position. PSCO discovered that the port rudder angle indicator stops at 20 degrees when a port 35 degrees command is issued. 74SOLAS20 V/19.2.5.4

Starboard lifeboat did not lower when dead man brake was released. After 30 minutes, crew finally was able to lower starboard lifeboat. During recovery, electric retrieval system did not work. Crew manually raised starboard lifeboat. Port lifeboat was successfully lowered but electric retrieval system did not work from local control. Limit switch from port davit broke off of mount.
