PSCO observed a lack of safety culture identified by watertight door operation knowledge gap, location of EEBD knowledge gap, international shore connection operation knowledge gap, and an active Hydrogen Sulfide leak identified by marine inspector via 4gas meter alarm and pungent rotten egg odor. Each passenger stateroom and crew cabin shall be equipped with entry doors that include peep holes or other means of visual identification. PSCO observed inoperable peep holes in cabins 10538, 10682, 8650. 46 USC 3507 (a)(1)(B) 50ac

In addition to the emergency lighting, the means of escape, including stairways and exits, shall be marked by lighting of photoluminescent strip indicators placed not more than 0.3 m (300mm) above the deck at all points of the escape route including angles and intersections. The marking must enable passengers to identify all the routes of escape and readily identify escape exits. PSCO observed inoperable low location lighting beneath the B-15 fire door across from cabin 9036. 74 SOLAS (97) II-2/28.1.10 50ac

Companies, masters, and crew members each have responsibility to ensure that each crew member can make a knowledgeable and informed contribution to the safe operation of the ship. PSCO observed crew member unknowledgeable on the operation of watertight doors. STCW 2017 A-1/14.1 10c

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed frayed electrical gland in engine room adjacent to potable water compressor 7. 74 SOLAS (04)II-1/40.1.3 10c

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II- I/42 or II- I/43. PSCO observed four emergency embarkation lights on the starboard side of the vessel inoperable. 74 SOLAS (97) III/11.4 10c

Fire hoses shall be of non-perishable material approved by the Administration and shall be sufficient in length to project a jet of water to any of the spaces in which they may be required to be used. Each hose shall be provided with a nozzle and the necessary couplings. In interior locations in passenger ships carrying more than 36 passengers fire hoses shall be connected to the hydrants at all times. PSCO observed missing spanner wrench at fire station FST01-5-28 and FST01-5-25. 74 SOLAS (97) II-2/4.7.1 10c

Ships of 500 gross tonnage and upwards shall be provided with at least one international shore connection complying with the provisions of paragraph 3. PSCO observed a lack of crew knowledge on the utilization of the international shore connection. 74 SOLAS (97) II-2/19.1 10c

The company should establish procedures to ensure that new personnel are given proper familiarization with their duties. Instructions which are essential to be provided prior to sailing should be identified, documented and given. PSCO observed a lack of knowledge on EEBD location in the engine room. 74 SOLAS (20) IX/3.1 ISM Code A.6.3 10c

All waste-receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed combustible waste receptacle in galley. 74 SOLAS (97) II-2/18.5 10c

Fire doors in main vertical zone bulkheads and stairway enclosures shall be capable of remote and automatic release from a continuously manned central control station, either simultaneously or in groups, and also individually from a position at both sides of the door. PSCO observed during fire-drill 14 remote released fire doors remained in open condition. 74 SOLAS 97 II-2/30.4.3

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead. PSCO observed weather tight door AC-5-13 with a crack at the bottom of the doors frame. ICLL 66/88 Reg. 12(1)

Fire doors in main vertical zone bulkheads and stairway enclosures shall be self-closing and shall have an approximately uniform rate of closure no more than 40s and no less than 10s. PSCO observed fire door 11-7-057 mens sauna did not close upon manual release but indicated closed on bridge. 74 SOLAS 97 II-2/30.4.1

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. Electrical breaker panel 61204/02 in deck 12 galley has uncovered breaker slots and pipe above is leaking water on to panel. 74 SOLAS (04) II-1/40.1

A line-throwing appliance complying with the requirements of section 7.1 of the Code Shall be provided. Pyrotechnics for all line-throwing appliances were expired. 74 SOLAS (97) III/18

0999 - Other (safety in general) - The machinery associated piping system and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so instaled and protected as to reduce to a minimum any danger to persons onboard. PSCO had H2S alarm in compartment 13 starboard side deck 1. The ventilation for the marinfloc bilge water treatment system is ventilating into the space. - 74 SOLAS (04) II-1/26.1

0750 - Fire door - The contruction of all doors and door frames in "A" class division shall provide resistance to fire as well as to the passage of smoke and flame. Roller shutter on deck 13, fire zone 4 grill was unable to close - 74 SOLAS (97) II-2/30.2
