Machinery must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The design of the vessel’s steering system requires hydraulic steering hoses to rub against one another during normal operations. Multiple hoses were worn to the metal core, and one ruptured, causing the loss of all hydraulic fluid and subsequent loss of steering.

Resolution: Vessel re-routed and replaced all high-pressure steering hoses in rudder compartment. New routing of hoses negate the need for chaffing gear as hoses no longer rub against one another.
