All installations must be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. The installaton of the Diesel engines driving the refrigeration compressors is incomplete. The installation shall be completed in accordance with the Coast Guard approved drawing, and verified by a Coast Guard Marine Inspector prior to using the system.

All U.S.-flag vessels which engage in domestic voyages by sea (coastwise and intercoastal voyages) shall be subject to the applicable provisions of this part. The statutory authority to prescribe rules and regulations requiring certain vessels to have and display load line marks indicating the maximum amidship draft to which such vessels may be safely loaded and certification thereof by the assigning authority is in 46 U.S.C. 5101-5116. Each vessel subject to load line requirements shall carry on board a valid certificate attesting to compliance with such requirements. (See §§ 42.07-35 and 42.07-40 for additional data furnished to the vessel.) The vessel does not have a valid Load Line certificate. The vessel shall have on board a valid Load Line Certificate issued by a recognized classification society prior to operating beyond any boundary line as defined in 46 CFR Cahapter I, Subchapter A, Part 7.

The operator did not provide periodic exposure monitoring during the annual COI inspection. The monitoring must be repeated each July or August if benzene containing cargoes are carried during those months; monitoring must be conducted under those weather conditions that will maximize benzene exposure, such as low wind, stable air, and high temperature. If benzene containing cargoes are not carried during those months, monitoring must be conducted at the time of carriage nearest those months; monitoring must be conducted under those weather conditions that will maximize benzene exposure, such as low wind, stable air, and high temperature. The crew logged gasoline transfers in July and August of 2020 in the Oil Record Book. Provide records for the periodic monitoring or complete the periodic monitoring at the next time of carriage for cargos containing benzene. Vessel is restricted from carrying benzene cargo in excess of 0.5% benzene by volume. 46 CFR 30.25-3 and 46 CFR 197.540.

The employer must provide, and the employees must submit to, the medical surveillance examinations for employees, as required by this section. The employer shall ensure that no one performs a benzene operation exceeding the level criteria of paragraph (b)(2) of this section without having undergone an initial medical examination and periodic medical examinations yearly thereafter. The employer shall maintain an accurate record of all monitoring conducted in compliances with §197.540 for three years. All records required to be maintained by this section must be made available upon request to the Coast Guard. The employer did not provide medical records for employees involved in cargo operations on the tank barge. Provide proof of employee records enrolled in the Periodic medical surveillance exam. 46 CFR 30.25-3 and 46 CFR 197.570

Coamings of any fiddley, funnel, or machinery space ventilators in an exposed position on the freeboard or superstructure deck shall be as high above the deck as is reasonable and practicable. Fiddley openings shall be fitted with strong covers of steel or other equivalent material permanently attached in their proper positions and capable of being secured weathertight. Other openings in such casings shall be fitted with equivalent covers, permanently attached in their proper positions. Two fire dampers to the machinery space located on the aft bulkhead were unable to be closed weathertight when tested by the crew. Renew as original. 46 CFR 42.15-35

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have on board an emergency means to enable the person in charge of a transfer operation to a facility, to another vessel, or within the vessel to stop the flow of oil or hazardous material. The means to stop the flow must be operable from the cargo deck, cargo control room, or the usual operating station of the person in charge of the transfer operation. The crew operationally tested the emergency shut down for the port and starboard cargo pumps and the test could not be completed when using the pull cable from cargo tank #3 which is identified as an operating station for transfer. The pull cable only worked when pulled immediately outside of the machinery space. Provide an emergency shut down that is operable from the cargo deck, cargo control room, or the usual operating station of the person in charge of the transfer operation. 33 CFR 155.780

In any case where a venting system is required for a particular grade of liquid, the venting system permitted for a higher grade of liquid may be used instead. Grade A, B, or C liquids. Cargo tanks in which Grade A, B, or C liquids are to be transported shall be fitted with either individual pressure-vacuum relief valves which shall extend to a reasonable height above the weather deck or shall be fitted with a venting system consisting of branch vent lines connected to a vent header which shall extend to a reasonable height above the weather deck and be fitted with a pressure-vacuum relief valve. Grade D or E liquids. Cargo tanks in which Grade D or E liquids only are to be transported shall be fitted with gooseneck vents and flame screens. The inspector identified flame screens missing or damaged on the following cargo tanks: 1S, 2S, 2P, 4P, 4S, and 5P. The vessel outfitted with P/V relief valves for Grade A, B, or C liquids. The operator is not permitted to load using flame screens in an open ventilation arrangement for grade D or E cargo until the flame screens are replaced in the cargo tanks missing flame screens. 46 CFR 32.55-25

The magnet for the tankerman to check the cargo level on the closed gauging system is missing on cargo tanks 1P and 1S. Replace and renew as original.
