Each vessel that operates on High Seas must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. MI observed that the Hydrostatic Release Unit had expired. Replace. 705c: Vessel restricted to less than 3 nautical miles from shore. 46 CFR 180.64 705c

Each life jacket carried on a vessel on oceans or coastwise must have a life jacket light secured to the life jacket. 02 life jacket lights not operating. 46 CFR 180.75 705c: Vessel is restricted to 40 passengers during daylight hours.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI observed that the stern light was inoperative. 705c: Vessel is restricted to daylight operations and 2 nautical miles visibility. 46 CFR 183.420 705c

Fire extinguishing equipment installed on a vessel must maintained in accordance with a recognized industry standard. MI observed that the monthly inspection maintenance logs were missing for all fire extinguishing equipment. Provide record. 46 CFR 181.500 16c

Electrical equipment on a vessel must be installed to protect person onboard and the vessel from electrical hazards. MI observed dead end wires in starboard side forward storage space and beneath the operating station. Secure source of electrical power. 46 CFR 183.200 (b) 16c

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. MI observed vessel unable to provide proof of drills conducted. 46 CFR 185.520(f)

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. MI observed vessel unable to provide proof of drills conducted. 46 CFR 185.524(d)

The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. MI observed vessel unable to provide proof of EPIRB testing. 46 CFR 185.728 (c)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI observed dead end wires beneath steering console at operating station. 46 CFR 183.200(b)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. MI observed unserviceable retroreflective tape on both life floats. 46 CFR 176.808 (b)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. MI observed portable fire extinguisher at operating station unsecured to vessel and not clearly visible. 46 CFR 181.520

A vessel of more than 65 feet in length must carry at least one rescue boat unless the cognizant OCMI determines that the vessel is sufficiently arranged and equipped to allow the crew to recover a helpless person from the water. MI observed crewmember not secured to vessel while recovering person from the water during MOB drill. 46 CFR 180.210 (a)(1)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing machinery failure. MI observed hydraulic leak on steering ram and under the helm. 46 CFR 182.100

An interconnected detection and alarm system must be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required to be protected by the interconnected detection and alarm system. MI observed that system was not alarming when tested. 46 CFR 181.450(a)(2)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. MI observed that first aid kit was expired. 46 CFR 184.710

MI observed dead end wires behind depth sounder display, under pilothouse console, and aft end of passenger compartment on weather deck. 46 CFR 183.200(b)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. MI observed that tat the required medium frequency radio was not inspected according to most recent Communications Act Safety Radiotelephony Certificate. CODE 705: Vessel may not operate beyond 20 nautical miles from land until radio is inspected by an FCC-licensed technician. 47 CFR 80.903

Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. Master stated that EPIRB had not been tested. 46 CFR 185.728 (a)

A vessel on an oceans, and coastwise route must carry six hand red flare distress signals and six hand orange smoke distress signals. Vessel was missing one of required distress signals. 46 CFR 180.68 (a)

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996, must comply with this part. Identified two new battery chargers installed on the vessel. Owner shall submit electrical plans to MSC for the new installation. 46 CFR § 183.115

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The medium frequency radio was not connected to the emergency battery power source. Vessel is restricted to routes less than 20 miles from shore until rectified. 46 CFR § 184.25-1 and 46 CFR § 183.01-10

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Identified a heavily corroded positive 10 AWG conductor on the supply side of the anchor winch and an inoperable 120 amp breaker. 46 CFR § 183.05-35

All cable and wire must Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Identified new installation of conductors in the steering space without supports. 46 CFR § 183.340

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline on a ring life buoy must be of at least 18.3 meters (60 feet) in length. The ring buoy had 43 feet of length of life line attached to it. 46 CFR § 180.70

The equipment required for lifefloats and buoyant apparatus must meet the following specifications: Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. Identified 3 fractured and decayed paddles for the life float. 46 CFR § 180.175

A vessel that is exposed to salt water not more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every tow years. Upon administrative review, vessel found to be overdue for drydock and internal exams. Vessel shall undergo drydock and internal structural exams prior to the carriage of passengers. 46CFR176.600(c)(1)

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. The operator installed a new power bilge pump that was not a replacement in kind in each watertight compartment after March 11, 1996. The submersible electric bilge pump located in the #2 compartment (numbered 1-4, fore to aft) was not operational when tested by the crew. Demonstrate proper operation of the submersible electric bilge pump located in compartment #2 to the attending marine inspector. 46 CFR 182.520(e)

As per 46 CFR 182.115(d), on or before March 11, 1999, an existing vessel must comply with the bilge high level requirements in 182.530. The bilge high level alarm located in the #2 compartment (numbered 1-4, fore to aft) was not operational when tested by the crew. A visual and audible alarm was not provided when tested. Demonstrate proper operation of the bilge high level alarm located in compartment #2 to the attending marine inspector. 46 CFR 182.530

The minimum number of portable fire extinguishers required on each vessel shall be determined by the OCMI in accordance with Table 181.30-1(a). The portable fire extinguisher required to be located at the wheelhouse/steering station was not on board. Provide (1) 10 B:C portable fire extinguisher of an approved type in the wheelhouse/steering station. 46 CFR 181.30-1(a)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of a large enough scale to make safe navigation possible. The operator did not have nautical charts on board. Provide adequate and up-to-date charts of a large enough scale to make safe navigation possible. 46 CFR 184.420

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in 4.03-2 of subchapter A of this chapter shall comply with the requirements of 4.06 in subchapter A of this chapter. The vessel is authorized to complete oceans voyages of up to 100 nm from land. The operator did not have alcohol test strips on board for the crew in the event of a marine casualty. The vessel is restricted to voyages no further than 2 hours from an alcohol testing facility until alcohol test strips are on board. 46 CFR 185.212

46 CFR 180.200(a)(2) Each survival craft required on the vessel must remain in good and serviceable condition. Life floats have paint and reflective tape degradation on outside of buoyant apparatus'. Make repairs to life float as necessary and replace reflective tape.

46 CFR 180.70(d) A vessel must carry one floating waterlight, unless it is limited to daytime operation. Floating waterlight does not ignite. Vessel is restricted to daytime operations until replaced.

Each portable fire extinguisher on a vessel must be of an approved type. Vessel's engine room 40-B:C indicator was displaying it as discharge and unavailable for use. Service or replace.

Sea chests, sea valves and sea strainers must be opened for examination. Both main engine raw water intake sea valves are of ball valve design and frozen in the open position. Repair or replace and prove proper operation.

Propellers, shafts, bearings shall be made available for inspection. Port strut cutlass bearing appears to not be seated properly and is creeping forward of the strut. Re-secure or replace port strut cutlass bearing. Prior to DD credit.

Piping for vital systems are those systems that are vital to a vessel's survivability and safety. STBD M/E and auxiliary generator's raw water intake piping in poor condition and showing severe wear. Replace prior to DD credit.

Deck rails must be designed and constructed to withstand a point load of 200LBS applied at any point in any direction. Port side, aft, top rail cracked. Repair within 7 days.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals and six hand orange smoke distress signals, or 12 hand red flare distress signals, approved in accordance with 46CFR160. 8 of the 12 red flare distress signals on board were expired. Replace. - 46CFR180.68(a)

A pre-engineered fixed gas fire extinguishing system must be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector. Fireboy extinguisher had maintenance pin left from last service, preventing manual actuation. Have certified technician remove pin. - 46CFR181.420(a)(2)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB. Hydrostatic release for Category 1 EPIRB is expired. Replace in accordance with manufacturer's instructions. Vessel is limited to 3 nautical miles. - 46CFR180.64

A submersible electric bilge pump may be used as a power bilge pump provided that the pump is permanently mounted. Pump in forepeak is not permanently mounted. Mount. - 46CFR182.520(e)(3)

Employers must maintain records of chemical tests, including pre-employment and random testing, and must make these records available to Coast Guard officials upon request. Owner had no record of pre-employment or random testing. Provide record of testing from ASAP to USCG. - 46CFR16.260(b)(1)

Marine employers may use sponsoring organizations, or may use contractors, to conduct the random chemical testing programs required by this part. Owner had no record of current year contract with ASAP. Provide record of current year contract to USCG. - 46CFR16.230(d)

An EAP training program must be conducted for the employer’s crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer’s supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Owner had no record of training for crewmembers or supervisor. Provide record of training to USCG. - 46CFR16.401(b)

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. Alcohol test strips provided by ASAP used for testing within 2 hours of a serious marine incident are expired. Replace. - 46CFR16.240

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Owner had no record of current year MIS report from consortium. Provide report to USCG. - 46CFR16.260(a)

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer’s policy regarding drug and alcohol use in the workplace. Owner had no record of signed substance abuse policy for each employee as required by program. Provide record of signed policies for each employee to USCG. - 46CFR16.401(a)

The certificate of inspection and stability letter addendum must be posted under glass or other suitable transparent such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. Certificates are either expired or missing. Replace with copies of valid certificates. - 46CFR176.302

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Letters and numbers must be at least 2 inches high. Markings for number and sizes of life jackets are faded. Renew markings. - 46CFR185.604

The owner or operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part. Crew training shall be logged for review by the Coast Guard upon request. The training entry shall include the following information: Date of the training and general description of the training topics. Training and drills were last logged on 03NOV17. Provide updated training record to USCG. - 46CFR185.420
