Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards. Dead end wires found beneath center portion of helm console, in void space beneath wheelhouse on starboard side near battery. 46 CFR 183.200

Connections must be made to battery terminals with permanent type connectors. Battery beneath wheelhouse had wingnuts. 46 CFR 183.350

Waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Plastic trash cans found on 3rd deck bar and main deck near galley. 46 CFR 177.405

At each initial and subsequent inspection for certification, managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment including the following: inspection of each lifejacket, work vest, and marine buoyant device. Third deck life ring buoy line is fraying and requires replacement. 46 CFR 176.808

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. In blackwater void #2, starboard transverse frame located 2 frames forward of aft bulkhead there is hole at the top of the transverse frame near the deck connection. Opposing side of hole is inaccessible as it extends over the water. Determine extent of damage on opposing side and provide a repair proposal. 46 CFR 176.802

Vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H (passenger vessels) of this chapter: "A door in a bulkhead required to be A-0 Class must be of solid or hollow steel or equivalent material construction, and capable of meeting the requirements of an A-0 Class bulkhead." Two (02) replacement doors installed in the A-0 fire boundary aft of the galley are not fire rated and do not meet the requirements of an A-0 class bulkhead as specified in the vessel's fire control plan. Vessel is limited to a maximum of 150 passengers until structural fire protection is restored."Old T" 46 CFR 177.10-5 & 46 CFR 72.05-25(b)(2)

A hatch exposed to the weather must be watertight, except that a hatch on a vessel that operates only on protected waters may be weathertight. During 13May2025 annual inspection MI observed and documented starboard aft galley door corrosion compromised weathertight integrity. A corresponding worklist item was issued prior on 07Mar2025 in response to operator self reported plan to replace aft galley doors. Due date for said worklist item was extended and has lapsed. Demonstrate weathertight integrity of port and starboard aft galley doors to satisfaction of marine inspector. 46 CFR 116.1160(a)

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. The hydrostatic release on the starboard middle IBA is expired. Maximum persons onboard reduced by 100 to 420. 46 CFR 117.130(a)(2)

A weathertight door must be provided for each opening located in a deck house. Pilot house door is not weathertight. 46 CFR 116.1160(d)

Each vessel certificated to operate on a lakes bays and sounds route must, if operated in cold water, be protected with life floats of 100% of total number of persons permitted on board. The vessel has 4 out of 6 IBAs requiring servicing. Restricted to operations within 1NM from shore or 200 total persons onboard. 46 CFR 117.207(c)(1)

Each vessel must be provided with a visible and audible bilge level alarms at the operating station in each space provided in 46 CFR 119.530. Vessel's forpeak audible alarm for the bilge level is inop. 46 CFR 119.530(a)

At each subsequent inspection for certification of a vessel the owner shall prepare for inspection of operation of main propulsion machinery. During the underway portion of the inspection, MI discovered excessive leaking from vessel's shaft seals. 46 CFR 115.804(a)

Marine employers shall establish programs for chemical testing for dangerous drugs for crew members on inspected vessels. Vessel to provide 2023 MIS report and provide a list of all crew members covered in drug testing program. 46 CFR 16.230(a)

At each subsequent inspection for certification the owner shall have the vessel ready for inspection of each portable fire extinguisher. Vessel's portable fire extinguisher aft of control station is inoperable. 46 CFR 115.810(a)(1)

The general construction of the vessel shall be to minimize fire hazards in so far as reasonable and practicable. Vessel to re-install wasted/missing insulation stbd aft of machinery space. 46 CFR 177.10-5(a) (OLD T)

Waste paper baskets shall be constructed of approved incombustible materials with solid sides and botooms. Found multiple trash cans onboard constructed of combustible material. Vessel to replace trash cans or provide proof that trash cans are constructed of incombustible materials. 46 CFR 177.10-5(a) (OLD T) 46 CFR 72.05-55(b)

Each ring life buoy must be marked with type II retro material approved IAW 164.018. Life rings missing retroflective tape. 46CFR122.604(1)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to crew and passengers. Vessel had no placards posted on the upper passenger deck. 46CFR122.516(a)

Each hose must: Be lined commercial firehose that conforms to UL 19 “Standard for Safety for Lined Fire Hose and Hose Assemblies” (incorporated by reference, see § 114.600 of this chapter), or hose that is listed and labeled by an independent laboratory recognized by the Commandant as being equivalent in performance. Fire hose on forward portion of second deck is damaged and leaking. Hose requires replacement of equivalent commercial grade.

A vessel must be provided with satisfactory pumping arrangements for draining any watertight compartment under all practicable conditions. Bilge pump failed to actuate.

As appropriate for the intended voyage all vessels must carry adequate and up to date publications. Vessel's navigation rules are out of date. Replace with valid publication. 46 CFR 185.20-35

All wiring and cables shall be suitable for marine service and for the particular installation used. Vessel's surge protector on bridge is broken and needs replacement. Replace with new surge protector. 46 CFR 183.10-20(a)

At each inspection the marine inspector shall examine all accessible parts of the hull. There is a steel hull failure on the main deck forward underneath the stairwell. Vessel must submit repair proposal for CG approval. 46 CFR 176.25-5(A)(i)

At each inspection the marine inspector shall inspect all railings and bulwarks. Vessels gangway has cracked weld and needs repair. 46 CFR 176.25-5 (A)(4)

Overcurrent protection shall be provided for each conductor for the purpose of opening the circuit if the current reaches a dangerous level. Vessel's distribution panels on the port side of the galley and the upper deck are improperly sized. These panels are to remain de-energized until properly sized over current protection is installed. 46 CFR 183.10-35

Cocks are prohibited for use in fuel lines except those of the solid bottom type. Vessel's drain cocks for fuel filters are not approved and leak fuel. Provide proper drains for fuel filters. 46 CFR 182.30-40 (A)(4)

Each vessel must have adequate emergency lights fitted along the line of escape that are actuated automatically upon loss of power. Fix installed emergency lighting on vessel. 46 CFR 120.432 (A)

Propulsion machinery must be suitable in type and design for propulsion requirements. Vessel's starboard engine has a leaking shaft packing gland. Replace packing gland. 46 CFR 182.05-1(a)

Each vessel if operated in cold water shall be provided with life floats or iba's of an aggregate capacity that will accomodate 100% of the total persons on board. Vessel only carries 300 person IBA. Reduced passenger count to 300 total persons on board.

Battery terminal connections shall be made permanent. Battery terminals throughout vessel have loose connections. Tighten loose connections. 46 CFR 183.05-20 (c)

Each vessel operator must ensure that the approved Vessel Security Plan (VSP) and any approved revisions or amendments thereto, are onboard and made available to the Coast Guard upon request. Did not have most current version of VSP onboard. 33 CFR 104.120(a)(1)

The VSO must ensure that at least one security drill is conducted at least every 3 months. Discovered no 2020 documentation of security drills. 33 CFR 104.230(b)(1)

Security exercises must be conducted at least once each calendar year, with no more than 18 months between exercises. No documented security exercises onboard. Vessel must complete in 07 days. 33 CFR 104.230(c)(1)

All machinery installed shall be maintained to afford adequate protection against fire, explosion, machinery failure and personnel injury. Grey water backup holding tanks discovered with unsecured tops and piping. 46 CFR 182.01-5 [Old T]

Openings through which conductors enter shall be adequately closed. Missing cover to junction box discovered in overhead of emergency exit of the aft engine room. 46 CFR 120.200

All escape hatches must be marked on both sides in clearly legible letters at least 2 inches high: "EMERGENCY EXIT KEEP CLEAR". Emergency exit of forward engine room not labeled from outside. 46 CFR 122.606

The method of opening a means of escape must be obvious, rapid, and of adequate strength. Loose insulation in engine room overhead inhibits proper use of emergency exit. 46 CFR 116.500(i)

At each vessel inspection, all observed unsafe practices and hazardous situations must be corrected. Numerous flammable materials stored near machinery and over piping in 3rd, 4th, 6th, and 8th compartments. 46 CFR 115.830(a)

At each inspection, spaces may be examined to see that there is no excessive accumulation of oil that might cause a fire hazard. Oil accumulation in second compartment. 46 CFR 115.850

As appropriate for the intended voyage, a vessel must carry an adequate and up-to-date Coast Guard Light List. No updated light list onboard. 46 CFR 121.420(a)(3)

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Four of the vessel's hydrostatic release units are expired. Provide liferaft servicing report for proof of delay. 46 CFR 180.130

Every portable fire extinguisher must be supplied with a suitable bracket which will hold the extinguisher securely in its stowage location on vessels or boats. One bracket in the pilothouse and one bracket in the engine room do not hold the extinguisher securely in its place. 46 CFR 162.028-3

The items contained in first-aid kit shall be as listed in Table 160.041-4(b). First Aid Kit onboard is missing ammonia inhalents and iodine applicator. 46 CFR 160.041-4(b)

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Lifesaving maintenance records were not available for inspection upon request.

Rugs and carpets are not permitted in machinery spaces, high risk service spaces, or areas where the spillage or leakage of flammable or combustible liquids is possible including areas immediately adjacent to bar service areas. Carpet in the vicinity of the bar on second deck must be removed by next year. 46 CFR 116.423 (a)(4)(ii)

The VSO must ensure that at least one security drill is conducted at least every 3 months, except when a vessel is out of service due to repairs or seasonal suspension of operation provided that in such cases a drill must be conducted within one week of the vessel's reactivation.
