TAILS OF THE SEA

The intent of this part is to insure that the scantlings, material, fastenings, workmanship, and arrangement of the vessel are such construction to provide for the safe operation of the vessel in accordance with the terms and conditions of it's Certificate of Inspection giving special consideration to provisions of a seaworthy hull. Multiple cracks were identified in the vicinity of the fore peak deck area around the tow/anchor bit. Interior inspection resulted in several cracked welds around the bit structure and the deck stiffeners, and a full thickness crack through the deck. O/O to submit a repair proposal to cognizant OCMI and carry out approved repairs prior to conducting passenger operations. 46 CFR 177.01-5

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. Fuel shutoff valve did not close/seat all the way, which could allow fuel flow after actuated. Owner/Operator shall show full actuation of valve to stop fuel flow. 46 CFR 119.455(b)(3)

When using DSC techniques, coast stations and ship stations must use maritime mobile service identities (MMSI) assigned by the Commission or its designees. The second radio on the PORT side of the pilothouse did not show MMSI. Owner/Operator must provide evidence of MMSI being installed. 47 CFR 80.103(b)

The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. Flame screen on fuel vent needs to be adjusted to provide a proper fit. 46 CFR 119.455(b)(3)

Each ring life buoy on a vessel must be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. 01 of the 03 life is wasted beyond use and must be replaced. 46 CFR 117.70(b)(1)

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. The plastic receptacles on the vessel must be removed and replaced with noncombustible. 46 CFR 117.70(b)(1)

All cable and wire must be protected from the weather. STBD electrical outlet cover missing, but installed during inspection. 46 CFR 120.340(b)(3)

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. The survival crafts maintenance must be logged IAW manufacturer's instructions. 46 CFR 122.702(a)

For portable fire extinguishers, the inspections, maintenance procedures and hydrostatic pressure tests required by Chapter 4 of NFPA 10,with the frequency specified by NFPA 10. If equipment or records have not been properly maintained, a qualified servicing facility may be required to perform an inspection. Maintenance was being conducted but not documented in a log. This corresponds with Monthly maintenance under NFPA 10. 46 CFR 115.810(b)(1)

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations. Submit weight change calculations to the Marine Safety Center for stability review for new generator installation. Prior to carriage of passengers.

Each generator and motor must be designed for an ambient temperature of 50 °C (122 °F). Fill out and return repower worksheet to local OCMI for new generator installation. Prove generator meets 50C temperature rating. Prior to carriage of passengers.

The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Prove proper operation of new generator under load, test built in safeties such as low lube oil pressure alarm, high jacket water temperature alarm, and overspeed shutdown/alarm (if equipped) to the satisfaction of the attending USCG MI. Prior to carriage of passengers.

A vessel owner or operator may use an Alternative Security Program as approved under subsection 101.120 if the Alternative Security Program is implemented in its entirety. Vessel has not completed an exercise or drills in accordance with their ASP.

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screens shall consist of a single screen of corrosion resistant wire of at least 30x30 mesh. The flame screens or flame arresters shall be of such size and design as to prevent reduction in the net cross-sectional area of the vent pipe. No flame screens or arresters were fitted on the discharge ends of the vent pipes for the fuel tanks. 46CFR182.20-35(d)

A vessel must carry adequate and up-to-date: (1) charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar; (3) Coast Guard Light list. Vessel had outdated charts, U.S. Coast Pilot, and light lists.

Vsl is broadcasting no static draft information.

Obtain DVPT approval letter from MSC.

Provide copy of approved letter from MSC for Design Verification Test Procedures (DVTP) and Periodic Safety Test Procedures (PSTP) for vessel.

Provide approved DVTP's and PSTP's for the 4 new engines and controls.

Prior to carrying passengers, prove enrollment in an approved drug and alcohol program.

Prior to carrying passengers properly repair the fire main piping in the machinery space in the overhead IWO the steering gear pumps.

Prior to carrying passengers, conduct underway man overboard, fire and abandon ship drills.

Prior to carrying passengers conduct fire and man overboard drills in the presence of a Coast Guard Marine Inspector.

Prior to carrying passengers provide valid documentation for drug and alcohol program.

Demonstrate satisfactory underway operation of vessel, MOB and firefighting drills

Perform annual drug testing compliance audit with CG inspector

Provide designation letter for "Senior Deckhand " and ensure deckhand is qualified I.A.W. NVIC 1-91

Provide updated navigation publications

Clean and service vessel's hot air furance.

PROPERLY LABEL UPPER DECK CAPACITY, IN PERSONS, AT THE ENTRANCE TO THE UPPER DECK.

PROVIDE COPY OF FCC STATION LICENSE.

MAKE PROPER REPAIRS TO LOOSE PORT AND STARBOARD RUDDER PACKING PLATES AND FLANGE.

CONDUCT UNDERWAY DRILLS NOT LATER THAN 01JUN96, OR PRIOR TO NEXT OPERATING SEASON.

SUBMIT REPAIR PROPOSAL FOR PERMENANT REPAIRS TO FRACTURE OF BOTTOM SHELL PLATE AFT OF STBD INBOARD INBOARD ENGINE.

CONDUCT UNDERWAY FIRE AND BOAT DRILLS.

RENEW 3 WATER LIGHTS FOR VESSEL'S LIFE FLOATS.

REPLACE LEAKING UNION TO RAW WATER INTAKE LINE FOR PORT GENERATOR.

INSTALL DRIP SHIELD PROTECTION FOR 110AC PANEL IN ENGINE ROOM.
